Electronic patient-reported outcomes and toxicities during radiotherapy for head-and-neck cancer.

PURPOSE: To characterize quality of life (QOL) using real-time, electronic patient-reported outcomes (ePROs) and to evaluate adverse events (AEs) and supportive care during head-and-neck radiotherapy (RT) and concurrent chemoradiotherapy (CCRT). METHODS: Sixty-five patients undergoing head-and-neck RT completed electronic, real-time, 12-item linear analog self-assessments (LASA) at baseline, before biweekly appointments, and at the last week of RT. Changes in QOL domains between time points were calculated. Clinical data were collected from the institutional medical record. AEs were recorded

Assessing the Risk of Prehospital Administration of Naloxone with Subsequent Refusal of Care.

BACKGROUND: EMS providers frequently encounter opioid-toxic patients who receive naloxone and then refuse further medical care. Older studies revealed this practice to be safe. In light of the evolving patterns of opioid abuse, this study attempted to determine the safety of this practice. METHODS: This is a retrospective review of all patient encounters by the Los Angeles Fire Department (LAFD) between July 1, 2011-December 31, 2013. All LAFD patient encounters are stored electronically. These electronic records were reviewed for subjects who received naloxone had a documented respiratory ra

Rat Model of Trigeminal Neuralgia Using Cobra Venom Mimics the Electron Microscopy, Behavioral, and Anticonvulsant Drug Responses Seen in Patients.

BACKGROUND: A new animal model of trigeminal neuralgia produced by injecting cobra venom into the infraorbital nerve (ION) trunk in rats had been developed. We tested and extended the model by observing the ultrastructural alterations of neurons and ameliorative effect of pregabalin in cobra venom-induced pain behaviors of rats. OBJECTIVES: The goal of this study was to prove the feasibility of the cobra venom-induced model of trigeminal neuralgia and to demonstrate the demyelination change of ION and medulla oblongata is the major pathological change of trigeminal neuralgia. Evidence-based rules from family practice to inform family practice; the learning healthcare system case study on urinary tract infections.

BACKGROUND: Analysis of encounter data relevant to the diagnostic process sourced from routine electronic medical record (EMR) databases represents a classic example of the concept of a learning healthcare system (LHS). By collecting International Classification of Primary Care (ICPC) coded EMR data as part of the Transition Project from Dutch and Maltese databases (using the EMR TransHIS), data mining algorithms can empirically quantify the relationships of all presenting reasons for encounter (RfEs) and recorded diagnostic outcomes. We have specifically looked at new episodes of care (EoC) f

Effects of human adipose-derived stem cells on the regeneration of damaged visceral pleural mesothelial cells: a morphological study in a rabbit model.

OBJECTIVES: Although an alveolar air leak (AAL) after pulmonary resection is a troublesome complication that diminishes a patient's quality of life and increases medical costs, current treatment and preventive methods for AAL are not effective. Therefore, we transplanted adipose-derived stem cells (ASCs) to the damaged visceral pleura to facilitate the regeneration of mesothelial cells and investigated the possibility of cell therapy as a treatment option for AAL. METHODS: Stem cells were isolated and cultured from discarded fat tissues that were collected after liposuction procedures. Flow c

Automated inhaled nitric oxide alerts for adult extracorporeal membrane oxygenation patient identification.

BACKGROUND: Recently, automated alerts have been used to identify patients with respiratory failure based on set criteria, which can be gleaned from the electronic medical record (EMR). Such an approach may also be useful for identifying patients with severe adult respiratory distress syndrome (ARDS) who may benefit from extracorporeal membrane oxygenation (ECMO). Inhaled nitric oxide (iNO) is a common rescue therapy for severe ARDS which can be easily tracked in the EMR, and some patients started on iNO may have indications for initiating ECMO. This case series summarizes our experience with

Bryan is a 10-year-old boy who is brought to his pediatrician by his parents with concerns about oppositional behaviors. Bryan's parents report that he has always been hyperactive and oppositional since a very young age. He has been previously diagnosed with attention-deficit hyperactivity disorder and has been treated with appropriate stimulant medications for several years; however, despite this, his parents feel increasingly unable to manage his difficult behaviors. He refuses to do chores or follow through with household routines. He refuses to go to bed at night. His family feels unable t

Antihypertensive Medication Class and Incident Alzheimer's Disease and Related Dementias: External Replication Across Two Independent Healthcare Data Sources-the Vanderbilt EHR Synthetic Derivative and US Medicare Claims.

IMPORTANCE: Hypertension is among the most important modifiable risk factors for Alzheimer's disease and related dementias (ADRD), but whether different antihypertensive drug classes confer different cognitive risk, particularly through differential modulation of angiotensin II receptor signaling, remains uncertain. Prior pharmacoepidemiologic evidence is based largely on a single Medicare cohort, and external validation across distinct healthcare data environments is needed. OBJECTIVE: To externally replicate and extend prior findings across two structurally independent healthcare data sourc

Automated Review of Patient Records: Privacy-Preserving Large Language Models for Identifying Incident Nonarteritic Anterior Ischemic Optic Neuropathy at Scale.

PURPOSE: Retrospective identification of acute nonarteritic anterior ischemic optic neuropathy (NAION) cases is critical for research on risk factors. However, reliance on International Classification of Diseases (ICD) 10th edition coding for case identification has limited accuracy, and manual review of longitudinal electronic health records is time-intensive. The purpose of this study is to evaluate automated methods for retrospective identification of acute NAION cases using large language models (LLMs) that preserve patient privacy. DESIGN: Retrospective cross-sectional study. SUBJECTS:

Media-based behavioural treatments for behavioural disorders in children.

BACKGROUND: Prevalence studies show that behaviour problems in children are quite common (10-15% in preschoolers). These problems may manifest as, for example anxiety, sadness, over-activity and tantrums. Some studies have shown that these problems can be persistent, and that they lead to a range of problems in adolescence and adulthood. Many approaches are used to address behavioural problems such as medication, or more usually, psychological treatments either directly with the child and/or his/her family. Behavioural interventions have been shown to be highly effective but access to these tr

Exploring Strategies for a Digital Tool to Support Medication Adherence Among Adolescents and Young Adults Undergoing Hematopoietic Stem Cell Transplant and Their Care Partners: Qualitative Formative Study.

BACKGROUND: Allogeneic hematopoietic stem cell transplant (HCT) is a complex but essential treatment for malignant and nonmalignant conditions, requiring strict posttransplant adherence to immunosuppressant medications to prevent complications such as graft-versus-host disease. Adolescents and young adults undergoing HCT face unique challenges, including balancing growing independence with ongoing reliance on care partners, often parents. Medication adherence in this group is often suboptimal, and few interventions address adolescent and young adult-care partner dyads. To address this gap, we
