Crohn disease or intestinal fistulising tuberculosis? Diagnosis difficulties in a case treated with infliximab and corticosteroids.

UNLABELLED: Crohn disease represents an idiopathic inflammatory bowel disorder with possibility of local and extra intestinal complications. Infliximab therapy is one of major therapeutic alternatives but this treatment may by followed by several possible infections, especially the reactivation of a latent tuberculosis. We present a case of a 28-year-old patient who was admitted in our hospital with clinical signs of bowel obstruction. Colonoscopy revealed close stenosis al the level of splenic flexure. Surgery was soon performed for the imminence of occlusion and pathology has revealed chroni

Unilateral absence of the lunate sulcus: an anatomical perspective.

The present study examines the gross anatomical features of anomalous lunate sulcus detected incidentally in a cadaveric brain and discusses its clinical importance. The absence of lunate sulcus was carefully studied in a dissected brain specimen. The absence of lunate sulcus was observed unilaterally on the right side of a cadaveric brain specimen. The lunate sulcus was clearly appreciated on the left side whilst on the right side it was absent. The right hemisphere of the cerebellum was also bigger in size as compared to the left. The absence of lunate sulcus is a rare finding, which may be

Peutz-Jeghers syndrome: case report and literature review.

Periorificial lentiginosis, also knew as Peutz-Jeghers Syndrome (PJS), is an autosomally dominant inherited condition determined by a mutation localized at 19p13.3 responsible for mucocutaneous pigmentation and gastrointestinal polyps. Skin- and mucosal pigmentation may be present at birth but usually occur in early childhood, and occasionally may develop later. Round, oval or irregular patches of brown or almost black pigmentation 1 to 5 mm diameter, irregularly distributed over the oral mucosa, gums, hard palate and lips (especially the lower) are observed. The pigmented maculae on the face,

The evaluation of interstitial Cajal cells distribution in non-tumoral colon disorders.

Interstitial cells of Cajal (ICC) are pacemakers that generate electric waves recorded from the gut and are important for intestinal motility. The aim of the study was to evaluate the distribution of interstitial cells of Cajal in colon specimens from patients with idiopathic chronic pseudo-obstruction and other non-tumoral colon disorders as compared with samples from normal colon. The distribution pattern of ICC in the normal and pathological human colon was evaluated by immunohistochemistry using antibodies for CD117, CD34, and S-100. In two cases with intestinal chronic idiopathic pseudo-o

The immunohistochemical expression of E-cadherin in gastric cancer; correlations with clinicopathological factors and patients' survival.

BACKGROUND: The aim of our study is to investigate the immunohistochemical expression of E-cadherin in gastric carcinomas and in surrounding mucosa (normal or with lesions of chronic atrophic gastritis, intestinal metaplasia or dysplasia). MATERIAL AND METHODS: We included 61 patients with gastric cancers operated in Emergency County Hospital Timisoara. We analyzed the E-cadherin immunohistochemical expression, the correlation with clinical and pathological factors and the outcome of the patients. The positive homogeneous pattern of staining for the cellular membranes is considered normal. Th

Accessory lateral pectoral nerves supplying the pectoralis major.

Certain surgical treatment failures of the brachial plexus lesions are due to the presence of anatomical variations. Anatomical knowledge of the pectoral nerves is of clinical importance during mastectomy, neurotization or other surgical procedures in the axilla. The present case describes three nerves arising from the lateral cord of brachial plexus supplying the pectoralis major muscle one below the other which was observed during routine dissection for undergraduate students. Knowledge of certain variations as in the present case may be advantageous for cosmetic augmentations during radical

The angiogenesis in colorectal carcinomas with and without lymph node metastases.

UNLABELLED: Many clinical trials revealed that the anti-angiogenic treatment could improve prognosis in patients with metastatic colorectal carcinomas (CRC), when added to standard chemotherapy. In this paper, we tried to find out if the microvascular density (MVD) determined with CD31, CD105 was correlated with lymph node status, and if the intensity of angiogenesis was different in right versus left colon segments. We studied 187 CRC, with and without lymph node metastases, 128 from left and 59 from right colon. RESULTS: In the right colon, the MVD was higher in the cases where the lymph no

Giant intracranial endolymphatic sac tumor (ELST). Case presentation and histogenetic considerations.

We present a giant tumor of the skull base compressing the brain in a 40-years-old man. The tumor was policystic at imaging. Its histopathology, immunohistochemical profile and long evolution suggest an endolymphatic sac tumor (ELST), a rare case of neoplasia. Since the patient had multiple otolaryngological procedures in his medical history, a possible traumatic pathogenesis could be suspected. On the other way, some immunohistochemical aspects found in our case may imply a histogenesis divergent from that currently accepted. This could be from either the organ of Corti or some local cells th

Congenital arteriovenous communication in the arm: a cadaveric study.

An abnormal communication between an artery and a vein is known as arteriovenous malformation (AVM) or arteriovenous fistula (AVF). The AVM or the AVF might be congenital in origin or even acquired. The arteriovenous communications are usually surgically made in patients undergoing repeated hemodialysis, while suffering from any chronic renal disease. The abnormal arteriovenous communications may be asymptomatic in nature. The arteriovenous communications might be an incidental finding during any anatomical dissections or medico-legal autopsies. The present study reports the presence of BBC on

Massive diminution of the cervical spine: one case report.

The authors report a 13-year-old female with shunted hydrocephalus who when evaluated for neck pain, was found to have not only fusion of cervical vertebrae but significant diminution of the cervical spine. Following our review of the classification of KFA and our case report, the current interpretation of Klippel and Feil's original description of KFA should be re-evaluated, and/or a new class of KFA used, that of complete or partial cervical vertebrae agenesis and associated abnormalities, should be considered in the classification of this anomaly.

A review of literature along with a cadaveric study of the prevalence of the Giacomini vein (the thigh extension of the small saphenous vein) in the Indian population.

Short saphenous has most variable anatomy in the inferior extremity which become more relevant especially in varicose vein surgery. Present study on cadavers was undertaken to report the prevalence of the thigh extension of the short saphenous vein (Giacomini vein) in the Indian population and to discuss it with the prior studies reported in the literature on other populations. Dissection was performed on both the sides of 50 embalmed cadavers to study the prevalence of the Giacomini vein. Mode of termination of the proximal extension of the short saphenous vein in the thigh was studied for di

Unilateral duplication of palmar arterial arches--a case report.

Recent progress in hand surgery has engendered a need for precise knowledge of anatomical variations of blood supply of hand. The present paper describes an unusual organization of palmar arterial arches and attempts to discuss its clinical implications. Double superficial palmar arch of the mediano-ulnar variant was observed with a double deep palmar arch of equal caliber, formed by splitting of deep branches of radial artery and ulnar artery. The superficial palmar branch of radial artery was absent.

An accessory iliacus muscle: a case report.

We present a case of an anomalous accessory iliacus muscle in the iliac fossa which gets originated from the iliac crest and inserting along with iliopsoas, and appear to compress the L4 root of femoral nerve. During the routine dissection of a male cadaver aged 58 years, we found an accessory iliacus muscle. The L2 and L3 nerve roots joined the L4 root distal to the accessory iliacus muscle. The L4 root of the femoral nerve supplied accessory iliacus muscle. Accessory iliacus muscle might cause tension on the femoral nerve resulting in referred pain to the hip and knee joints and to the lumba

Morphopathological changes induced by the Obstetrical Antiphospholipid Antibody Syndrome in fetal adnexa and uterus.

The Obstetrical Antiphospholipid Antibody Syndrome (OAAS) is representing a separate entity of the global Antiphospholipid Antibody Syndrome (APS), focusing the pregnancy morbidity. OAAS is generating morphopathological changes in almost all components of the gestational biologic transitory system (GBTS): placenta, umbilical cord or uterine wall. The most important, serious and lengthened anomalies are occuring in placenta. Our research has been developed on a group of 68 patients diagnosed with OAAS, initially using the Sapporo criteria and later using the "Sydney" ones. There have been morph

A "conservative" method of thoracic wall dissection: a proposal for teaching human anatomy.

The common methods of dissection exposing the thoracic organs include crossing of the wall together with wide resection of its muscular planes. In order to preserve these structures, a little demolishing technique of the thoracic wall is proposed, entering the thoracic cavity without extensive resection of the pectoral muscles. This method is based on the fact that these muscles rise up from the wall, like a bridge connecting the costal plane with the upper limb, and that the pectoralis major shows a segmental constitution. SUPERIOR LIMIT: Resect the sternal manubrium transversely between the

Correlations among the serum levels of some interleukins and the histopathological aspects in chronic viral hepatitis C.

UNLABELLED: PREMISES AND OBJECTIVES: The evolution of the infection with the hepatic virus C depends on the defense of the organism, found under the control of a network of cytokines and chemokines. The mechanisms that are causing both viral persistence as well as hepatic pathology are not entirely elucidated. We have proposed to study the amount in which different categories of cytokines are incriminated in the pathogenesis of the chronic liver disease, as well as the eventual correlations between the serum levels of these cytokines and certain histopathological aspects in the chronic viral h

The split flexor digitorum superficialis.

Variations of the muscles of the anterior forearm are common. We report the cadaveric findings of an unusual variant of flexor digitorum superficialis (FDS). The deep part of the FDS was found to be split and showed two distinct fusiform muscle bellies. The medial belly originated from the common flexor tendon from the medial condyle of the humerus and continued as a thin tendon at the middle of the forearm to insert onto the fifth digit. The lateral belly blended proximally with the deep surface of the superficial portion of the FDS and ended in a relatively thicker tendon, which was inserted

The middle perforated substance of the diencephalon.

The diencephalon, upper brain stem and other basal brain structures are supplied chiefly by penetrating branches of the cerebral arteries. We examined the retrochiasmatic space between the superior border of the pons and posterior edge of the optic chiasm in six randomly selected adult fresh brain specimens. Lateral or anterolateral to the mamillary bodies, two small quadrangular spaces (2.5 x 3.5 mm) were found that were limited laterally by the junction of the optic tract and crus cerebri. These spaces were pierced on each side by 1 to 5 small penetrating branches (premamillary arterial comp
