[Endometriosis management in the emergency department: a diagnostic and therapeutic challenge].

Endometriosis affects at least one in ten women of childbearing age, yet it is often overlooked in emergency care. Clinicians should consider this diagnosis for any woman presenting with menstrual or ovulatory cyclic pain, dyspareunia, or chronic pelvic pain. In case of worsening pain in a patient with endometriosis, an underlying acute condition must be ruled out before considering an "endometriosis flare-up". Effective pain management remains the cornerstone of emergency care in endometriosis. It also plays a critical role in preventing premature surgical intervention, whose iatrogenic conse

Abdominal Pregnancy: Challenges in Recognition and Diagnosis in the Emergency Department.

An abdominal pregnancy (AP) is a unique type of ectopic pregnancy that occurs when the embryo implants within the peritoneal cavity. It is an obstetrical emergency that can be fatal to both the mother and the fetus. Being able to identify such an occurrence in the emergency department requires physicians to understand certain risk factors and clinical findings and appreciate diagnostic challenges. This report discusses the case of a young female who presented with abdominal pain and vaginal bleeding in what was believed to be an intrauterine pregnancy and later discovered to be an AP. It also

Macrophage immunometabolism: emerging targets for regrowth in aging muscle.

The recovery from muscle atrophy is impaired with aging as characterized by improper muscle remodeling and sustained functional deficits. Age-related deficits in muscle regrowth are tightly linked with the loss of early pro-inflammatory macrophage responses and subsequent cellular dysregulation within the skeletal muscle niche. Macrophage inflammatory phenotype is regulated at the metabolic level, highlighting immunometabolism as an emerging strategy to enhance macrophage responses and restore functional muscle regrowth. Accordingly, metabolic targets with an emphasis on glycolytic, hypoxia, a

Epistaxis and Hypertensive Emergency as the First Signs of Lupus Nephritis.

Systemic lupus erythematosus is a multisystem autoimmune disease that predominantly affects young females. This case report describes a rare presentation of a hypertensive emergency as the initial manifestation of lupus nephritis (LN) in a 64-year-old female with normal serum creatinine at presentation. The patient initially presented with epistaxis and severe hypertension (221/127 mmHg). Further evaluation revealed non-nephrotic range proteinuria and microscopic hematuria. Autoimmune studies and renal biopsy confirmed diffuse proliferative LN (Class IV). Treatment with corticosteroids and cyc

Is WALANT anesthesia safe and comfortable in emergency hand surgery?

The use of Wide-Awake Local Anesthesia No Tourniquet (WALANT) is well documented in elective surgery but remains understudied in emergency hand surgery. The emergency setting presents distinct challenges, including preoperative pain, patient stress, and inflamed or damaged soft tissue. This study aims to evaluate the efficacy and safety of the WALANT procedure for soft tissue and bone surgery in acute hand and wrist injuries. A prospective cohort study was conducted, involving 245 hand injuries in 207 patients. Pain scores during injection were 3.7/10 for wounds and 3.8/10 for bone procedures.

Predictors of response to emergent therapies in paediatric immune thrombocytopenia.

Emergent therapies (ET), which include intravenous immunoglobulin (IVIG), corticosteroids and intravenous anti-Rho(D) immunoglobulin, are used to treat acute episodes of bleeding in children with immune thrombocytopenia (ITP). There are currently no known biomarkers or clinical features predictive of treatment response to any specific ET. Thus, the treatment of ITP remains largely trial and error, exposing patients to potentially ineffective medications, which are often expensive and associated with adverse effects. We therefore sought to identify the predictors of treatment response through c

Neuritin: a multifaceted neuroprotective factor with emerging applications for neurodegeneration.

Neuritin is a conserved, activity-regulated gene encoding a glycosylphosphatidylinositol-anchored protein, crucial for neural development, synaptic plasticity, and neuroprotection. Identified via activity-dependent gene screening in the rat hippocampus, neuritin promotes neurite outgrowth, dendritic arborization, and synaptic maturation with neural activity. In this review, we summarize recent findings regarding neuritin's signaling pathways, neuroprotective, neuroregenerative, and neuromodulatory properties, with a focus on its therapeutic potential to counter neurodegeneration in various con

Heterotopic ossification: Current developments and emerging potential therapies.

This review aimed to provide a comprehensive analysis of the etiology, epidemiology, pathology, and conventional treatment of heterotopic ossification (HO), especially emerging potential therapies. HO is the process of ectopic bone formation at non-skeletal sites. HO can be subdivided into two major forms, acquired and hereditary, with acquired HO predominating. Hereditary HO is a rare and life-threatening genetic disorder, but both acquired and hereditary form can cause severe complications, such as peripheral nerve entrapment, pressure ulcers, and disability if joint ankylosis develops, whic

Current and emerging bone resorption inhibitors for the treatment of osteoporosis.

INTRODUCTION: Osteoporosis is a metabolic skeletal disease characterized by low bone mass and strength, and increased risk for fragility fractures. It is a major health issue in aging populations, due to fracture-associated increased disability and mortality. Antiresorptive treatments are first line choices in most of the cases. AREAS COVERED: Bone homeostasis is complicated, and multiple factors can compromise skeletal health. Bone turnover is a continuous process regulated by the coupled activities of bone cells that preserves skeletal strength and integrity. Imbalance between bone resorpti

Vasospastic angina: Pathophysiology, diagnosis, and emerging therapeutic approaches.

Vasospastic angina (VSA) is a distinct endotype of ischemia with non-obstructive coronary arteries characterized by transient coronary artery spasm and myocardial ischemia in the absence of significant fixed stenosis. It is an underdiagnosed and often challenging condition that can lead to recurrent angina, myocardial infarction, and sudden cardiac death. VSA arises from a multifactorial interplay of endothelial dysfunction, vascular smooth muscle hyperreactivity, inflammation, and autonomic dysregulation. While calcium channel blockers and nitrates remain the mainstay of therapy, there is a g

Emerging manifestations of IL-17 immunomodulation in the gastrointestinal tract.

Interleukin 17 (IL-17) is a proinflammatory cytokine that is commonly dysregulated in autoimmune diseases. Biologics targeting IL-17 (ixekizumab, secukinumab, brodalumab, bimekizumab) have been approved to treat conditions including psoriasis, psoriatic arthritis, and ankylosing spondylitis. Although inflammatory bowel disease (IBD) is characterized by dysregulated IL-17 signaling, IL-17 inhibitors have not shown benefit for IBD patients and are paradoxically associated with new-onset colitis. Here, we searched our biopsy archives and consult material to identify patients on IL-17 inhibitors w

Emerging biological therapies for psoriatic arthritis: A systematic review.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory condition characterized by joint involvement, enthesitis, dactylitis, and skin psoriasis. The pathophysiology of PsA is complex, driven by dysregulated immune responses, making targeted therapies essential for managing symptoms. Despite the success of biologic disease-modifying anti-rheumatic drugs (bDMARDs) like tumor necrosis factor (TNF) inhibitors, many patients experience suboptimal responses or adverse effects, necessitating the development of new therapies. Emerging biologics targeting interleukin (IL)-17, IL-23, and Janus

Management of patients with juvenile idiopathic arthritis in the emergency department.

Juvenile idiopathic arthritis (JIA) is the most common pediatric rheumatic disease. Children may present to the emergency department during the initial presentation of JIA or due to disease-related complications. Differentiating JIA from emergent causes of joint pain, including severe infections and malignancies, can be challenging. This issue reviews the clinical presentation of JIA, provides guidance for differentiating JIA from conditions with similar presentations, and offers recommendations for management of JIA and JIA-related complications in the emergency department.

Management and Treatment of an Emergent Nonpuerperal Uterine Inversion With a Leiomyoma.

Uterine inversion is a rare medical emergency that occurs when the uterus inverts and, at times, protrudes from the vagina. Uterine inversions can be puerperal, related to childbirth, or nonpuerperal, often secondary to tumors within the uterine cavity. Our case presents a nonpuerperal uterine inversion caused by a pedunculated submucosal leiomyoma. The case follows a patient from her initial presentation through the resolution of her condition with a vaginal myomectomy and abdominal hysterectomy.
