Atrophy, hormones, genes and viruses in aetiology germ cell tumours.

This chapter reviews the evidence that testicular germ cell atrophy could be the final common pathway for all of the established and postulated risk factors for induction of testis tumours. The evidence from age incidence studies shows that the tumour peak incidence follows the curve of sexual activity in the male, and this provides the starting point for the argument that this tumour is dependent on endocrine factors for its induction. The data from epidemiological studies confirming the high frequency of atrophogenic events and occurrence of low sperm counts in more than 75% of patients prov

Development and external validation of an XGBoost-based prognostic model for small cell lung cancer with brain metastases.

BACKGROUND: Patients with small cell lung cancer (SCLC) and brain metastases (BM) have poor and heterogeneous survival outcomes, while existing prognostic tools have shown limited discriminatory performance. We aimed to develop an extreme gradient boosting (XGBoost) based prognostic model and perform preliminary external validation. METHODS: Patients with small cell lung cancer and brain metastases (SCLCBM) diagnosed between 2010 and 2020 were identified from the Surveillance, Epidemiology, and End Results (SEER) database for model development and internal testing. External validation was per

Risk factors of breast cancer patients developing multiple primary cancers: a retrospective study and establishing/testing of machine learning models.

BACKGROUND: Breast cancer is a prevalent malignancy globally, with approximately 1 in 10 breast cancer patients at risk of developing additional primary malignant tumors. This study seeks to explore the risk factors linked to the development of multiple primary cancers (MPCs) in breast cancer patients and to develop predictive models to aid in clinical decision-making. METHODS: A cohort of patients from the Surveillance, Epidemiology, and End Results (SEER) database was analyzed to identify key factors contributing to the occurrence of MPCs. Machine learning models, including logistic regress

Transcriptome analysis reveals significant discrepancies between two in vitro models of host-trematode interaction.

Cell models emulating an in vitro parasitic infection can greatly improve our understanding of helminthiases. Nonetheless, it remains challenging to select an appropriate in vitro model to study molecular pathogenesis of infections by trematodes having a complex life cycle. Therefore, adequate models are in high demand. The epidemiologically important foodborne trematode Opisthorchis felineus parasitizes bile ducts of fish-eating mammals, including humans. The human infection leads to chronic inflammation and biliary intraepithelial neoplasia, which is considered precancerous. This study was a

In vivo and in vitro Models of PM2.5 Induced COPD: Focus on the Role of RTA-408.

INTRODUCTION: Inflammation and oxidative stress are important factors in the pathogenesis of Chronic obstructive pulmonary disease (COPD). Current treatments for COPD focus on improving symptoms caused by inflammation rather than curing the disease, therefore, emerging research focusing on upstream pathways may help develop effective treatments. Epidemiological investigations have shown that exposure to fine particulate matter (PM2.5) can cause lung inflammation and oxidative stress through nuclear factor NF-E2-associated factor (Nrf2) pathway, leading to COPD. Nrf2 is an important transcripti

A machine learning-based model for predicting the risk of early-stage inguinal lymph node metastases in patients with squamous cell carcinoma of the penis.

OBJECTIVE: Inguinal lymph node metastasis (ILNM) is significantly associated with poor prognosis in patients with squamous cell carcinoma of the penis (SCCP). Patient prognosis could be improved if the probability of ILNM incidence could be accurately predicted at an early stage. We developed a predictive model based on machine learning combined with big data to achieve this. METHODS: Data of patients diagnosed with SCCP were obtained from the Surveillance, Epidemiology, and End Results Program Research Data. By combing variables that represented the patients' clinical characteristics, we app

Network methods and design of randomized trials: Application to investigation of COVID-19 vaccination boosters.

Network science methods can be useful in design, monitoring, and analysis of randomized trials for control of spread of infections. Their usefulness arises from the role of statistical network models in molecular epidemiology and in study design. Computational models, such as agent-based models that propagate disease on simulated contact networks, can be used to investigate the properties of different study designs and analysis plans. Particularly valuable is the use of these methods to assess how magnitude and detectability of intervention effects depend on both individual-level and network-l

Assessing the drivers of syphilis among men who have sex with men in Switzerland reveals a key impact of screening frequency: A modelling study.

Over the last decade, syphilis diagnoses among men-who-have-sex-with-men (MSM) have strongly increased in Europe. Understanding the drivers of the ongoing epidemic may aid to curb transmissions. In order to identify the drivers of syphilis transmission in MSM in Switzerland between 2006 and 2017 as well as the effect of potential interventions, we set up an epidemiological model stratified by syphilis stage, HIV-diagnosis, and behavioral factors to account for syphilis infectiousness and risk for transmission. In the main model, we used 'reported non-steady partners' (nsP) as the main proxy fo

Modelling exposure heterogeneity and density dependence in onchocerciasis using a novel individual-based transmission model, EPIONCHO-IBM: Implications for elimination and data needs.

BACKGROUND: Density dependence in helminth establishment and heterogeneity in exposure to infection are known to drive resilience to interventions based on mass drug administration (MDA). However, the interaction between these processes is poorly understood. We developed a novel individual-based model for onchocerciasis transmission, EPIONCHO-IBM, which accounts for both processes. We fit the model to pre-intervention epidemiological data and explore parasite dynamics during MDA with ivermectin. METHODOLOGY/PRINCIPAL FINDINGS: Density dependence and heterogeneity in exposure to blackfly (vect

Assessment of traffic-related noise in three cities in the United States.

BACKGROUND: Traffic-related noise is a growing public health concern in developing and developed countries due to increasing vehicle traffic. Epidemiological studies have reported associations between noise exposure and high blood pressure, increased risk of hypertension and heart disease, and stress induced by sleep disturbance and annoyance. These findings motivate the need for regular noise assessments within urban areas. This paper assesses the relationships between traffic and noise in three US cities. METHODS: Noise measurements were conducted in downtown areas in three cities in the Un

Spatial Clustering of Recently Activated Cytotoxic Lymphocytes Improves Association with Overall Survival in Women with High-Grade Serous Ovarian Cancer.

BACKGROUND: A previous study found that adding the spatial context of tumor-infiltrating lymphocytes (TIL) and cytotoxic T lymphocytes (CTL) to abundance improved associations with overall survival (OS) of non-Hispanic Black women with high-grade serous ovarian cancer (HGSOC). This study set out to replicate previous findings in predominantly non-Hispanic White women HGSOC cohorts. METHODS: Multiplex immunofluorescence was used to characterize TILs in HGSOC from women enrolled in three epidemiologic studies (N = 433). Spatial clustering of TILs was derived using a permutation approach for Rip

Clinical characteristics and prognosis of acute myeloid leukemia and plasma cell neoplasms with bidirectional occurrence: a retrospective population-based study.

The bidirectional occurrence of acute myeloid leukemia (AML) and plasma cell neoplasms (PCN) is rare, and clinical characteristics and survival outcomes remain poorly defined. We conducted a retrospective population-based study using the Surveillance, Epidemiology, and End Results (SEER) database (2000-2022). Patients diagnosed with both AML and PCN were identified and categorized based on diagnostic sequence. Overall survival (OS) and cancer-specific survival (CSS) were analyzed using Kaplan-Meier methods and Cox proportional hazards models adjusting for demographic and clinical variables. Th

Modeling epidemic and economic trade-offs under alternative COVID-19 isolation exit scenarios in South Korea.

BACKGROUND: Scientific evidence should guide adjustments to isolation policies as part of coronavirus disease 2019 (COVID-19) exit strategies. However, such evidence was limited when these strategies were implemented. This study retrospectively evaluated the relaxation of isolation policies in South Korea, aiming to generate evidence to support policy decisions in future pandemic scenarios. METHODS: We developed an age-stratified deterministic vaccination-extended Susceptible-Exposed-Infectious-Recovered model to simulate counterfactual scenarios related to isolation policy changes during the

Modelling Xylella fastidiosa Infection Dynamics in Grapevines: A Physiologically Based Approach.

Xylella fastidiosa subsp. fastidiosa (Xff), the causal agent of Pierce's Disease (PD) in grapevine, is a major threat to global viticulture. However, the mechanisms governing infection establishment, persistence, and recovery within grapevine hosts remain insufficiently characterized. We developed a novel physiologically based infection model that explicitly describes the population dynamics and within-plant spread of Xff in grapevine plants as a function of temperature and host phenology. Grapevines are represented as structured hosts composed of two interacting compartments: permanent woody

Environment-based strategy for monitoring early colonization of Lutzomyia longipalpis at the city scale.

The urban expansion of Lutzomyia longipalpis, the principal vector of Leishmania infantum in the Americas, has created new epidemiological scenarios for visceral leishmaniasis, thereby increasing the need for early detection of vector colonization in cities. Most predictive approaches have focused on temporal dynamics driven by climate, whereas tools to prioritize surveillance sites within urban areas are still limited. Here, we developed and evaluated an environment-based strategy to identify critical intra-urban areas for early detection of Lu. longipalpis. We conducted a scoping review to i

Multidimensional Trends and AI-Based Forecasts of HIV Incidence in Korea, 1985-2024.

BACKGROUND: Since the first reported HIV case in Korea in 1985, the incidence of HIV/AIDS has continued to rise, emphasizing the need for effective prevention and management strategies. While global initiatives aim to end the epidemic by 2030, Korea has observed demographic shifts, particularly among young men and foreign nationals, that warrant further investigation. METHODS: We analyzed national HIV incidence data from 1985 to 2024, stratified by nationality, age, transmission route, and testing motivation. To forecast future trends, we employed a stacking ensemble model combining ARIMA, Ran

Identifying risk factors of post-COVID-19 conditions with machine learning and deep learning algorithms.

INTRODUCTION: Post-COVID-19 conditions (PCC) affect millions of people in the United States. Early diagnosis and PCC management requires an understanding of the epidemiology and drivers behind PCC in the real world. METHODS: We applied multiple machine learning and deep learning models to a large electronic health database of patients with a recent COVID-19 infection in the United States from 2020 to 2022 to quantitatively evaluate progression to newly developed PCC and identify the individual-level risk factors for developing new PCC at 60, 74, 90, and 120 days following initial SARS-CoV-2 i
