Perioperative management of hemorrhagic stroke in morbidly obese patients: A narrative review.

BACKGROUND: Intracerebral hemorrhage (ICH) constitutes approximately 10-15% of all strokes and is a major contributor to long-term neurological morbidity and mortality. Morbid obesity (body mass index ≥40 kg/m2) is a well-recognized risk factor for hypertension and hemorrhagic stroke. Paradoxically, some studies report more favorable outcomes among obese patients with stroke - a phenomenon known as the obesity paradox. Nevertheless, specific perioperative anesthetic guidelines for this high-risk population remain inadequately addressed in current literature. METHODS: This narrative review cri

Successful Endovascular Repair of a Thoracic Infected Aortic Aneurysm with Concomitant Liver Abscess: A Case Report.

BACKGROUND Concomitant infected aortic aneurysm and liver abscess is extremely rare and potentially fatal. Their simultaneous occurrence suggests an aggressive disease course and presents substantial diagnostic and therapeutic challenges. Because symptoms are often nonspecific, diagnosis is frequently delayed, which can lead to worse outcomes. Early recognition through timely imaging and a coordinated multidisciplinary approach are essential for optimizing clinical results. CASE REPORT A 65-year-old woman with a history of hypertension and diabetes mellitus presented with a 1-week history of e

Ultrasound-guided thoracic paravertebral block versus erector spinae plane block analgesia for cardiac surgery with median sternotomy: a noninferiority randomised controlled trial.

BACKGROUND: Effective postoperative analgesia after cardiac surgery with median sternotomy is essential to minimise complications and promote recovery. Bilateral thoracic paravertebral block (TPVB) is an established technique in cardiac surgery but carries potential risks. Erector spinae plane block (ESPB) could offer a simpler and safer alternative. METHODS: In this prospective, patient- and assessor-blinded, noninferiority, randomised controlled trial, adults undergoing median sternotomy for planned cardiac surgery were allocated to receive ultrasound-guided single-shot ESPB or TPVB with 20

Quantification of myocardial blood flow using PET: Current status, clinical applications, and future directions.

This review highlights the evolving role of positron emission tomography (PET) in quantifying myocardial blood flow (MBF) and myocardial flow reserve (MFR) and its expanding clinical impact. The relative nature of perfusion assessment with single photon emission computed tomography often underestimates disease in patients with multivessel or microvascular involvement. Positron emission tomography (PET) enables absolute quantification of myocardial blood flow (MBF) and myocardial flow reserve (MFR), which provides deeper insights into coronary physiology. PET-derived MBF and MFR have shown clea

Increased albuminuria is highly prevalent in the general population: prevalence of CKD in the Gutenberg Health Study.

BACKGROUND: Early diagnosis of chronic kidney disease (CKD) is essential to slow progression and delay or prevent dialysis. However, in the absence of specific symptoms, patients and physicians may remain unaware of the disease for a long period. Here we present an analysis from the Gutenberg Health Study, a prospective longitudinal cohort study, to estimate the prevalence of CKD indicators in the population. METHODS: A representative sample of 10 125 individuals underwent extensive medical testing at baseline; 9331 were tested again after 5 years. The estimated glomerular filtration rate (eG

Epidemiological Link Between Post-traumatic Stress Disorder and Cardiovascular Disease: Evidence, Mechanisms, and Clinical Implications.

Post-traumatic stress disorder (PTSD) has emerged as a significant and independent risk factor for cardiovascular disease (CVD), yet its incorporation into routine cardiovascular prevention and care remains limited. This review synthesizes epidemiological evidence showing consistent associations between PTSD and increased incidence of hypertension, coronary artery disease, myocardial infarction, stroke, heart failure, and cardiovascular mortality across diverse populations, including veterans, civilians, women, and disaster-exposed cohorts. The review also highlights the bidirectional relation

A methodology study for sodium quantification in bone and soft tissue based on in vivo neutron activation analysis using a two-compartment model.

Sodium (Na) is an essential mineral for human health regulation and in excess has been associated with many diseases. Its storage has been found in rapidly (soft tissue) and slowly exchangeable pools (bone) in human body. However, Na concentration and metabolism information in human bone and soft tissue cannot be determined using conventional biological urine and blood samples. The aim of this study was to apply a transportable neutron generator based in vivo neutron activation analysis technique to separately quantify Na in bone and soft tissue. Two pigs were fed with low and high Na diet to

Endovascular Treatment Outcomes for TASC C and D Lesions in Chronic Peripheral Arterial Disease: A Retrospective Study and Literature Review.

Background: Peripheral Artery Disease (PAD) of the lower extremities is a prevalent manifestation of atherosclerotic disease, significantly affecting individuals aged 55-70, with a global incidence of 4-12%. Major risk factors include smoking, diabetes mellitus, hypertension, dyslipidemia, and chronic kidney disease, all contributing to endothelial damage and subsequent plaque progression. This retrospective study examines the outcomes of endovascular treatment for TASC C and D lesions, which are complex cases that have historically required surgical intervention. Methods: From June 2022 to Se

Atypical Presentation of Boerhaave Syndrome with Left Shoulder Pain as the Initial Manifestation: A Case Report.

BACKGROUND This report describes a rare spontaneous esophageal rupture presenting as left shoulder pain. Spontaneous transmural rupture, or Boerhaave syndrome, is a surgical emergency with high morbidity and mortality but is often missed due to atypical signs. Mackler's triad of vomiting, acute chest pain, and subcutaneous emphysema raises concern for this diagnosis but appears in only about 50% of cases, emphasizing early recognition and high suspicion in atypical presentations. CASE REPORT We present a rare case of an Asian man in his 90s with hypertension presenting with left shoulder pain

Pseudohyperkalemia in a Patient with Leukemia: A Diagnostic Pitfall in Emergency Medicine.

BACKGROUND: Pseudohyperkalemia is characterized by a falsely elevated serum potassium concentration, often due to cellular disruption during blood sample collection, processing, or transport. Accurate differentiation between pseudohyperkalemia and true hyperkalemia is vital to prevent inappropriate treatment, which can lead to iatrogenic hypokalemia. CASE REPORT: We present a case of a 67-year-old woman with hypertension and T-cell prolymphocytic leukemia who presented to the emergency department with fever and hypotension. Initial laboratory results indicated hyperkalemia, acute kidney injur

Obesity-Related Phenotype of Heart Failure With Preserved Ejection Fraction: A Comprehensive Review.

Heart failure with preserved ejection fraction (HFpEF) is a complex clinical syndrome with an obesity-related phenotype gaining prominence amid the global obesity epidemic. This review explores the distinct pathophysiological mechanisms, diagnostic challenges, and management strategies associated with obesity-induced HFpEF. Obesity contributes to HFpEF through several key mechanisms, including increased blood volume, myocardial hypertrophy and fibrosis, systemic inflammation, and metabolic dysregulation. These factors collectively exacerbate diastolic dysfunction and elevate left ventricular f

Coronary Artery Revascularization in the Older Adult Population: A Scientific Statement From the American Heart Association.

The United States is facing a demographic shift as the population of older adults grows rapidly, with the proportion of Americans ≥65 years of age projected to double by 2060. This aging trend will have far-reaching effects on health care systems, especially because aging is a primary risk factor for cardiovascular disease. Age-related cardiovascular changes, such as increased arterial stiffness, endothelial dysfunction, and reduced elasticity, increase the risk for hypertension, atherosclerosis, and other risk factors. Older adults often experience additional complications, including obesity,

Symptomatic pulmonary cement embolism following PMMA vertebroplasty: Clinical presentation and management.

BACKGROUND: Vertebroplasty, a minimally invasive technique frequently used for spine stabilization and vertebral compression fractures, uses polymethylmethacrylate (PMMA) bone cement. Although usually benign, pulmonary cement embolism (PCE) is an uncommon but potentially dangerous consequence that arises when bone cement enters the pulmonary circulation through the vertebral venous system. Although the majority of PCE cases are underdiagnosed and asymptomatic, the incidence varies between 2.1 % and 26 % of vertebroplasty surgeries. To prevent incorrect diagnoses and needless treatments, early
