Effect of high-protein meal replacement on weight and cardiometabolic profile in overweight/obese Asian Indians in North India.

The aim of the present study was to evaluate the impact of a high-protein meal replacement (HPMR) on weight and metabolic, lipid and inflammatory parameters in overweight/obese Asian Indians. In this 12-week open-label, parallel-arm randomised controlled trial, 122 overweight/obese men and women were administered either a HPMR or a control diet after 2 weeks of diet and exercise run-in. Body weight, waist circumference (WC), percentage body fat (%BF), fasting blood glucose, post-oral glucose tolerance test (post-OGTT) blood glucose, fasting and post-OGTT serum insulin, lipid profile, high-sens

Regular long-term red blood cell transfusions for managing chronic chest complications in sickle cell disease.

BACKGROUND: Sickle cell disease is a genetic haemoglobin disorder, which can cause severe pain, significant end-organ damage, pulmonary complications, and premature death. Sickle cell disease is one of the most common severe monogenic disorders in the world, due to the inheritance of two abnormal haemoglobin (beta globin) genes. The two most common chronic chest complications due to sickle cell disease are pulmonary hypertension and chronic sickle lung disease. These complications can lead to morbidity (such as reduced exercise tolerance) and increased mortality. This is an update of a Cochrane…

Circulatory endotoxin concentration and cytokine profile in response to exertional-heat stress during a multi-stage ultra-marathon competition.

Exertional-heat stress has the potential to disturb intestinal integrity, leading to enhanced permeability of enteric pathogenic micro-organisms and associated clinical manifestations. The study aimed to determine the circulatory endotoxin concentration and cytokine profile of ultra-endurance runners (UER, n=19) and a control group (CON, n=12) during a five stage 230km ultra-marathon (mean ± SD: 27h38min ± 3h55min) conducted in hot and dry environmental conditions (30ºC to 40ºC and 31% to 40% relative humidity). Body mass and tympanic temperature were measured, and venous blood samples were ta

[BNP measurement for perioperative management].

BACKGROUND: It is difficult to evaluate the tolerance to anesthesia of patients undergoing operations, who have risk factors of congestive heart failure, such as hypertension, old age or various cardiac diseases. BNP (B type natriuretic peptide) is a useful biomarker as a screening tool for LV dysfunction. Therefore we hypothesized that the measurement of BNP may be useful for perioperative management of these patients. METHODS: Subjects were 101 (58 male and 43 female) gastro-intestinal cancer patients, aged 30 to 91 years (mean 63.9 +/- 12.4) scheduled for intraperitoneal surgery. All patie

Cancer-related fatigue. Clinical practice guidelines in oncology.

These guidelines propose a treatment algorithm in which patients are evaluated regularly for fatigue using a brief screening instrument, and are treated as indicated by their fatigue level. The algorithm's goal is to identify and treat all patients with fatigue that causes distress or interferes with their daily activities or functioning. Management of fatigue begins with primary oncology team members who perform the initial screening and either provide basic education and counseling or expand the initial screening to a more focused evaluation for moderate or higher levels of fatigue. At this

Modern medical management of obesity: the role of pharmaceutical intervention.

The medical model of obesity treatment--combining diet, exercise, and behavior modification with antiobesity agents--suffered a setback when fenfluramine and dexfenfluramine were withdrawn from the market because of an association between these medications and valvular regurgitation. The Food and Drug Administration has recently approved sibutramine (Meridia), a norepinephrine and serotonin reuptake inhibitor that was originally developed as an antidepressant, but which has also been shown to reduce weight. In a 1-year placebo-controlled trial, 65% of patients receiving 15 mg sibutramine daily

Voice Therapy for Puberphonia in the Adolescent Male: A Clinical Case Study.

CLINICAL SCENARIO: Puberphonia (also known as mutational falsetto) occurs when an individual maintains prepubescent speaking fundamental frequency (F0) after the laryngeal changes associated with puberty have taken place. Voice therapy with a speech-language pathologist (SLP) is the primary treatment for puberphonia; however, resources describing treatment techniques specific to this patient population are lacking. CLINICAL QUESTION: What diagnostic and treatment procedures are employed in the course of treatment for an adolescent presenting with puberphonia? What factors influence interventi

Photobiomodulation therapy and postoperative recovery after abdominoplasty: a non-randomized pilot study.

Photobiomodulation (PBM) has been proposed as a non-invasive therapeutic strategy to support wound healing and modulate oxidative and inflammatory responses. However, clinical data evaluating its effects in the postoperative period of abdominoplasty remain limited. This non-randomized clinical trial included 31 female patients undergoing abdominoplasty, divided into two groups: PBM intervention (n = 15) and control (n = 16). Salivary samples were collected at three time points: preoperative (T0), 48 h postoperative (T1), and 7 days postoperative (T2). Inflammatory markers (TNF-α, IL-10), oxida

App-Based Myofunctional Therapy to Improve Adherence and Outcomes of Mandibular Advancement Devices in Moderate Obstructive Sleep Apnea: Multicenter Randomized Trial Protocol.

BACKGROUND: Mandibular advancement devices (MADs) are a valid alternative for patients with moderate obstructive sleep apnea (OSA) who do not tolerate continuous positive airway pressure. Despite their efficacy, both adherence and long-term results remain a challenge. Myofunctional therapy (MT) has shown potential to improve upper airway muscle tone and reduce OSA severity, but its role as an adjunct to MAD treatment is still unclear. This protocol evaluates whether app-based MT (Airway Gym) can improve adherence and clinical outcomes in patients with moderate OSA treated with MADs. We describ

Autologous P63+ lung progenitor cell transplantation in idiopathic pulmonary fibrosis: a phase 1 clinical trial.

BACKGROUND: In idiopathic pulmonary fibrosis (IPF) patients, alveolar architectures are lost and gas transfer function would decline, which cannot be rescued by conventional anti-fibrotic therapy. P63+ lung basal progenitor cells are reported to have potential to repair damaged lung epithelium in animal models, which need further investigation in clinical trials. METHODS: We cloned and expanded P63+ progenitor cells from IPF patients to manufacture cell product REGEND001, which were further characterized by morphology and single-cell transcriptomic analysis. Subsequently, an open-label, dose-

Novel insights into the effects of JMV2894, a growth hormone secretagogue, in Duchenne muscular dystrophy: A preclinical study in the D2-mdx mouse model.

Growth hormone secretagogues (GHSs) are gaining interest as promising therapeutics for Duchenne muscular dystrophy (DMD) due to their broad activity profile and proven benefits in other muscle-wasting conditions. We previously found that JMV2894 - a pseudopeptide GHS - exerts functional, anti-inflammatory, and antifibrotic benefits in classic BL10-mdx mice, supported by in silico predictions of its interaction with ADAMTS-5 and matrix metalloproteinase 9 (MMP-9), both overactivated in DMD. This led us to test JMV2894 in the D2.B10-Dmdmdx/J (D2-mdx) mouse model, chosen for its hyper-fibrotic an
