EBV can produce a broad range of CNS disorders such as demyelinative diseases, acute encephalitis, cerebellar ataxia, myelitis, or meningitis. The role of EBV itself in acute encephalitis is not completely clear. This report focuses on a fatality involving severe dengue fever and melioidosis in a 28-year-old truck driver residing in Pacoti in northeastern Brazil. He exhibited long-term respiratory symptoms (48 days) and went through a wide-ranging clinical investigation at three hospitals, after initial clinical diagnoses of pneumonia, visceral leishmaniasis, tuberculosis, and fungal sepsis. After death, Burkholderia pseudomallei was isolated in a culture of ascitic fluid. Dengue virus type 1 was detected by polymerase chain reaction in cerebrospinal fluid (CSF); this infection was the cause of death.

Acquired central hypothyroidism (CH) is a rare form of hypothyroidism that results from a variety of conditions affecting the hypothalamus and the pituitary gland. This pathology remains difficult to diagnose in patients with chronic disease. The Authors describe a 21-year-old patient with thalassaemia (TM) who was referred for the evaluation of short stature and hypogonadism, and was found to have CH.This case report stresses the importance of following thyroid function in TM patients and underlines the criteria for diagnosis and treatment.

An 89-year-old woman with a known hiatus hernia presented to the accident and emergency department with acute onset epigastric pain. Our case is a male student which was dead because of traffic accident as passenger in October, 01, 2007. His heart was found to be lacerated and ruptured horizontally from atrio-ventricular region through all layer of the wall at autopsy. There were not any changes of skeletal system. Our case is considered as a rare and interesting case because there was no lesion on the bones, though the case was exposed to so massive trauma that cause rupture of heart from all layers.

OBJECTIVE: A case of a rapidly progressive degeneration of the visual, auditory and corticospinal tract in a patient with a HIV infection is presented.
METHODS AND RESULTS: The HIV-infected patient suffered from severe and rapidly progressive sensorineural hearing loss, blindness, dysarthria, dysphagia and tetraparesis. MRI showed degeneration of the visual, auditory and corticospinal tract. Diffusion tensor imaging showed reduced fractional anisotropy of the corticospinal tract. HAART and other salvage therapies did not save the patient from death 5 months after the onset of the symptoms.

Five 25-yr-old crocodiles (Crocodylus johnstoni and Crocodylus porosus) were diagnosed with Aeromonas hydrophila-associated septicemia accelerated by improper thermoregulation. At necropsy, pulmonary congestion and pleural effusion were the main lesions in the thorax. Necrotizing enteritis, intestinal hemorrhage, fibrinous serositis, hepatitis, and pancreatitis were observed in the abdominal cavities of all five crocodiles. An acute poisoning with nitrobenzene presenting as methaemoglobinemia is an uncommon medical emergency. A young girl with nitrobenzene induced methaemoglobinaemia died despite use of mechanical ventilator, administration of oral methylene blue and parenteral ascorbic acid. Here author highlights the rare occurrence of such cases, methaemoglobin induced severe oxidative stress, unexplained splenomegaly and leucocytosis and the ineffectivity of oral methylene blue and other supportive measures in evading death due to nitrobenzene poisoning.
