Uterine arteriovenous fistula treated with repetitive transcatheter embolization: case report.

Uterine arteriovenous fistula (AVF) is a rare entity, but may lead to life-threatening hemorrhage. Although transcatheter embolization, surgical ligation, or hysterectomy would be considered for treatment of uterine AVF, there is poor knowledge as to how gynecologists can manage the uterine AVF with multiple large inflow arteries. Herein we report a uterine AVF successfully treated using multiple-step transcatheter embolization. The patient, a 58-year-old postmenopausal woman with a history of dilation and curettage, had intermittent massive uterine bleeding. Radiologic imaging revealed the pr…

Right pulmonary artery to left atrial fistula: a rare presentation.

Right pulmonary artery to left atrial fistula is a rare congenital vascular anomaly which usually presents with cyanosis, clubbing, dyspnea, and signs and symptoms of a right to left shunt. Paradoxical embolism to the brain resulting in cerebral abscess formation and death is a rare and ominous complication that has been described. We describe an unusual presentation with abdominal pain resulting from splenic infarction.

15-year history of spontaneous appendico-vesical fistula (case report).

Appendicovesical (AVF) fistula is rare complication in the modern era. Only a few cases of AVF were described in the literature of the last decade. AVF is meanly associated with a history of appendicitis. The guiding symptoms are coprosuria and pneumaturia, with recurrent infection of the urinary tract. The accurate pre-operative diagnosis is difficult to make. It usually took at least one year from the onset of symptom to confirm diagnosis. It has been reported a few cases of 15 year history of undiagnosed AVF. We represent a case of EVF, spontaneously developed after treated acute appendicul…

Vesico-Appendiceal Fistula Misdiagnosed as Meckel's Diverticulum: A Laparoscopic Approach.

A 26-year-old man complained of a vague low abdominal discomfort for the previous 2 months. Radiologic evaluations demonstrated that there was tubular structure connected with the right side wall of the bladder, suggesting Meckel's diverticulum with fistula formation to the bladder as well as a mass-like bladder wall thickening. With an impression of Meckel's diverticulum with fistula with the bladder, laparoscopic surgery was performed to confirm a diagnosis and to manage the Meckel's diverticulum with fistula with the bladder. The distal tip of the appendix was firmly attached to the right s…

[Benign intracranial hypertension in a hemodialysis patient: consider the fistula].

UNLABELLED: Benign intracranial hypertension (BIH) is a syndrome characterized by the abnormal elevation of the intracranial pressure in the absence of a mass lesion or hydrocephalus, and with normal composition of the cerebrospinal fluid. We report a case of BIH in a chronic hemodialysis patient with no signs of intraocular inflammation or neurologic abnormalities. Treatment with acetazolamide followed by corticosteroids failed to improve the signs of BIH. However, after correction of the radiocephalic arteriovenous fistula of the left upper extremity, which demonstrated excessive flow, BIH s…

Diffusion-weighted MRI for evaluating perianal fistula activity: feasibility study.

PURPOSE: To assess the feasibility of using diffusion-weighted magnetic resonance (MR) imaging (DWI) for evaluating perianal fistula activity. MATERIALS AND METHODS: This study retrospectively assessed 24 patients with clinically suspected perianal fistula who underwent a total of 28 MR examinations after conservative treatment with antibiotics. DWI was performed at 1.5 T, using b-values of 0 and 1000 s/mm(2). Apparent diffusion coefficient (ADC) maps were created and ADCs of the lesions were measured. Lesions were classified into two groups based on the need for surgery and surgical findings…

Successful treatment of bronchoesophageal fistula with esophageal and bronchial stenting.

Bronchoesophageal fistula is reported in 5-10% of patients with esophageal cancer. In most of these cases, the insertion of a single stent, either a tracheobronchial or an esophageal stent, is sufficient to seal off the fistula. In this case we describe a 67-year-old man with esophageal cancer and complications of bronchoesophageal fistula, which resulted in repeated pneumonia and acute respiratory failure. Initially, two expandable metallic membranous esophageal stents were placed to cover the fistula. However, the esophageal stent failed to stop the air leak and dislodged into the stomach. T…

Necrotizing pneumococcal pneumonia with bronchopleural fistula among children in Taiwan.

BACKGROUND: Severe necrotizing pneumococcal pneumonia may progress to the development of bronchopleural fistula (BPF). The purpose of this study was to describe the clinical courses and identify risk factors for the development of bronchopleural fistula in children with pneumococcal pneumonia. Histopathologic features of children receiving surgical resections of the lung because of BPF were analyzed to explore the pathogenesis of destructive lung disease caused by Streptococcus pneumoniae. METHODS: A total of 112 cases of culture-proven pneumococcal pneumonia were identified between January 2…

Anatomic characteristics of anal fistula on three-dimensional anorectal ultrasonography.

BACKGROUND: Surgery for anal fistula is often associated with continence disorders due to the transection of sphincter muscles. Extensive knowledge of anal canal anatomy and anal fistula can help prevent this outcome. OBJECTIVE: This study aimed to correlate the anatomical conformation of the anal canal, the fistula track, and the internal opening according to sex and hemicircumference (anterior vs posterior) by use of 3-dimensional ultrasonography. METHODS: One hundred sixty-five patients with fistula were evaluated with 3-dimensional ultrasound and grouped according to sex, fistula type, i…

Mechanism of non-specific-fistula-in-ano: Hormonal aspects-Review.

Abscesses could occur anywhere, but quite few that could proceed forward and develop fistulae. Crypto glandular abscess formation concept has been forming the basis for almost all designed strategies for management of cases of idiopathic fistula-in-ano for ages. Yet, this hypothesis failed to explain the reason(s) for the recorded lower incidences of idiopathic anal fistula in females than in males. Gram negative bacteria are largely the causative agents for these anal glands' abscesses that processed further and developed fistulae. Lipopolysaccharide component (LPS) of the bacterial cell wall…

Why do we have so much trouble treating anal fistula?

Anal fistula is among the most common illnesses affecting man. Medical literature dating back to 400 BC has discussed this problem. Various causative factors have been proposed throughout the centuries, but it appears that the majority of fistulas unrelated to specific causes (e.g. Tuberculosis, Crohn's disease) result from infection (abscess) in anal glands extending from the intersphincteric plane to various anorectal spaces. The tubular structure of an anal fistula easily yields itself to division or unroofing (fistulotomy) or excision (fistulectomy) in most cases. The problem with this sin…

Splenic arteriovenous fistula: successful treatment with an Amplatz occlusion device.

Splenic artery and vein aneurysm with splenic arteriovenous fistula (SAVF) is a rare entity. We report the case of a 72-year-old woman who presented with signs and symptoms of portal hypertension after a laparoscopic Nissen fundoplication. The diagnosis of a 37-mm SAVF was confirmed by a computed tomographic angiogram. The arteriovenous fistula was successfully treated with placement of a 20-mm Amplatz occlusion device. Surgical ligation and percutaneous embolization have been reported to be equally successful in managing SAVF. We present a review of the literature and report on a novel approa…

Aorto-right ventricular fistula: a complication of aortic valve replacement.

The occurrence of aorto-right ventricular (aorto-RV) fistula after prosthetic aortic valve replacement is rare. Transthoracic echocardiography (TTE) with color-flow Doppler, transesophageal echocardiography (TEE), or both may be required for diagnosis. A 42-year-old woman sought care for palpitations and dyspnea due to atrial flutter 2 weeks after prosthetic aortic valve replacement and graft replacement of the ascending aorta. TTE and TEE revealed left-to-right shunt due to aorto-RV fistula.

Interposition of gracilis muscle for rectourethral fistula repair: case report.

BACKGROUND: Rectourethral fistula is a rare disease with a difficult surgical treatment. The etiology of rectourethral fistula is iatrogenic, secondary to prostate surgery, pelvic radiotherapy and a rare complication of rectal surgery. Surgical treatment options for rectourethral fistula are diverse. Interposition of gracilis muscle has a curative index of 100% with close surveillance, as reported by Nyham. CLINICAL CASE: We present a case of a 58-year-old male with iatrogenic rectourethral fistula secondary to radical prostatectomy and who was treated with interposition of the gracilis muscl…
