The chamber-scarification test for irritancy.

A procedure has been designed (the chamber-scarification test), which possesses greatly increased sensitivity for assessing the irritancy of topically applied materials. A forearm test site is criss-cross scarified by drawing a 30-gauge needle over the skin with just enough pressure to cleave the epidermis without drawing blood. The test agent is applied in an aluminum chamber once daily for 3 days. The advantages of the method over conventional patch testing are: enhanced capacity to measure mild irritants, reduced time (3 days versus 10 to 21 days), less effort, less cost, less discomfort fo

Anteromedial facet coronoid fractures: an algorithmic approach for assessment and management based on 3 dimensional computed tomography humeral subtraction.

BACKGROUND: Isolated coronoid fractures without concomitant radial head injury raise suspicion for an anteromedial facet (AMF) fracture as a result of a varus posteromedial rotatory instability (VPMRI) mechanism. However, not all isolated coronoid fractures involve the AMF, nor are they all the result of a VPMRI mechanism. AMF fractures as a result of a VPMRI mechanism have been reported to have specific radiographic features (medially oblique, concave, with extension to the sublime tubercle). We hypothesized that a detailed assessment of the pattern of AMF fracture utilizing three-dimensional

Pattern of bone mineral density in patients with sporadic idiopathic hypoparathyroidism.

OBJECTIVE: Measurement of bone mineral density (BMD) in patients with hypoparathyroidism directly addresses the effect of chronic under-exposure of bone to PTH. Because post-thyroidectomy hypoparathyroidism is potentially complicated by the pre-existence of thyrotoxicosis and the need for postoperative thyroxine replacement, we have studied a large group of patients with sporadic hypoparathyroidism who have been followed up in our endocrine clinic. Studies conducted in limited number of patients with sporadic idiopathic hypoparathyroidism (SIH) have suggested an increase in BMD in such patient

Randomized controlled clinical trial of surgery versus no surgery in patients with mild asymptomatic primary hyperparathyroidism.

Parathyroidectomy is the definitive therapy for patients with symptomatic primary hyperparathyroidism. However, the role of surgery in mild asymptomatic primary hyperparathyroidism remains controversial. Accordingly, we conducted a prospective, randomized, controlled clinical trial of parathyroidectomy to determine the benefits of surgery vs. adverse effects of no surgery. Fifty-three patients were randomly assigned to either parathyroidectomy (n = 25) or regular follow-up (n = 28). Bone mineral density (BMD), biochemical indices of the disease, quality of life, and psychological function were

Unveiling the Uncommon: Bilateral Absence of Musculocutaneous Nerve in Cadaveric Examination.

The brachial plexus is a network of nerves crucial to the innervation of the upper limb. The anatomy of the brachial plexus has been thought to be consistent across persons and is studied as such. Instances of deviations of this plexus pose significant challenges in both clinical diagnosis and surgical interventions. The musculocutaneous nerve, originating from the lateral cord of the brachial plexus, allows for movement of key muscles of the arm and cutaneous sensation to the forearm. Our present case reveals a unique bilateral absence of the musculocutaneous nerve. We aim to elucidate the an

Nivolumab-Induced de novo Discoid Lupus Erythematosus.

An increasing number of checkpoint inhibitor-induced subacute cutaneous lupus erythematosus events have been reported. We present the first case of nivolumab-induced discoid lupus erythematosus in a patient with hepatocellular carcinoma. The patient presents with violaceous hypopigmented plaques on the pinna bilaterally, hypopigmented plaques with central hyperpigmentation on the posterior neck, and other hypopigmented plaques on the face, forearms, and hands. For management, nivolumab was held for 2 months, and Plaquenil and topical steroids were added. Nivolumab was resumed with no further p

Coexistence of two accessory flexor pollicis longus heads or coexistence of two-headed flexor pollicis longus with an unrecognized anatomical structure?

The flexor pollicis longus (FPL) is located in the anterior compartment of the forearm. It is morphologically variable in both point of origin and insertion. An additional head of the FPL can lead to anterior interosseous syndrome. This report presents a morphological variation of the FPL (additional head in proximal attachment and bifurcated tendinous insertion in distal attachment) and an unrecognized structure that has not so far been described in the literature. This structure originates in six heads (attached to the FPL or interosseous membrane) that merge together, and inserts on to the

[Report of the first case of tinea corporis bullosa by Trichophyton tonsurans].

Blister formation in tinea corporis is rare. Bullous tinea is usually evoked by zoophilic dermatophytes. A 20-year-old woman presented in our out-patient department with a painful pruritic bullous skin eruption at the left forearm. Clinically, a 4 × 3 cm symmetrical plaque with sharp borders and peripheral versiculation and serous crusts was seen. Histologically there was a marked spongiotic dermatitis with fungal elements in periodic acid stain (PAS). By fungal culture, PCR and gene sequencing Trichophyton (T.) tonsurans was identified. To the best of our knowledge, this is the first publishe

Secondary Syphilis with Pulmonary Involvement.

A 39-year-old Japanese man presented to our hospital complaining of left chest pain and rash on the hands and feet. Plain thoracic computed tomography (CT) revealed multiple nodular shadows in the left lower lobe of the lung. A diagnosis of secondary syphilis was made based on the appearance of the rash and positive serologic tests for syphilis. The patient was started on amoxicillin but was switched to minocycline due to amoxicillin-induced rash on both forearms. Thoracic CT after five months of treatment revealed that the multiple lung nodular shadows had contracted, and secondary syphilis w

D-penicillamine in the treatment of eosinophilic fasciitis: case reports and review of the literature.

Eosinophilic fasciitis (EF) is a rare disease characterized by symmetrical thickness and hardening of the skin, especially localized to forearms and thorax, with eosinophilia. Corticosteroids represent the first-line therapy, even if some patients are scarcely responsive and/or may develop important side effects due to long-term treatment. Here, we describe three cases of EF, two of them refractory to previous steroid therapy, successfully treated with D-penicillamine. The present clinical observations together with the updated review of the literature suggest usefulness of D-penicillamine in

Baller-Gerold syndrome associated with dextrocardia.

Baller-Gerold Syndrome (BGS) is a rare autosomal recessive disorder that is apparent at birth. The disorder is characterized by distinctive malformations of the skull and facial area and bones of the forearms and hands. We report a 4 year old boy in whom the clinical features of craniosynostosis and bilateral absent thumbs and radii led to a diagnosis of Baller-Gerold syndrome. Physical examination revealed that the heart was localized to the right side. Echocardiography confirmed dextrocardia. Dextrocardia has not previously been reported with Baller-Gerold syndrome. To the best of our knowle

[Adaptive training with electromyographic feedback after elimination of traumatic defects of wrist and fingers].

The objective of this work was to analyse the efficiency of adaptive training with electromyographic biological feedback in the early period after reconstruction of fingers depending on the character of wrist defects, the method of their correction, and etiology of the injury. Unique techniques were proposed for the training. Their effectiveness was evaluated based on clinical observations, results of electromyographic and biomechanical studies that demonstrated a significant increase of EMG amplitudes for different groups of wrist and forearm muscles during basic thumb movements (p = 0.05-0.0

Allotransplant of microencapsulated parathyroid tissue in severe postsurgical hypoparathyroidism: a case report.

The last therapeutic alternative in severe postsurgical hypoparathyroidism is allotransplantation of microencapsulated parathyroid cells. With this technique, it is possible to implant cells or tissue of parathyroid origin to replace them in such patients, without immusupression. We report an allotransplant of parathyroid tissue in a patient with continous endovenous requirement of calcium to survive. The microencapsulation was carried out with a commercial sodium alginate. We implant 23 microspheres in the nondominant forearm and 40 microspheres in the leg in a second attempt. In this article

[Clinic peculiarities of secondary flexion contractur formation in the proximal interphalangeal joint of the triphalanger fingers in conditions of posttraumatic chronic ischemia].

Occurrence of the proximal interphalangeal (PIP) joints contracture is a characteristic consequence of traumas of a forearm and a hand, complicated by the phenomena of a chronic ischemia. Their formation leads to occurrence of considerable functional deficiency of a hand, especially at involving in process of several fingers, and treatment is a complicated problem. Secondary contractures of PIP joints at a chronic ischemia of a hand and fingers differ high severity level, firmness, are characterized by fast formation and progressing. Elimination of the primary factor of blocking of a kinematic

[Disorders of the distal radioulnar joint following fractures of the distal end of the radius].

After a fracture of the distal radius, whether healed in an anatomic position or malunited, many patients complain about problems on the ulnar side of the wrist with pain and decreased range of forearm rotation. In addition many patients are unhappy with the unpleasant appearance of the wrist joint. The complaints are related to tears of the triangular fibrocartilaginous complex, instability, and/or incongruity of the distal radioulnar joint and degenerative changes. Malunion of the distal radius must be taken into account when discussing treatment options. The purpose of this paper is to desc

Case report: the split flexor carpi ulnaris as a local muscle flap.

The flexor carpi ulnaris is a useful local muscle flap in the forearm and elbow. It is, however, an important palmar flexor and ulnar deviator of the wrist, and functional loss may arise from the use of this muscle in its entirety. The flexor carpi ulnaris is made up of two distinct neuromuscular compartments. This arrangement allows for splitting of the muscle and the potential use of the larger ulnar compartment as a local muscle flap while maintaining the humeral compartment as an ulnar deviator and palmar flexor of the wrist. We report two cases illustrating the clinical use of the split f

Acute volar and dorsal compartment syndrome after a distal radius fracture: a case report.

We present a case of an acute volar and dorsal compartment syndrome of the forearm secondary to a distal radius fracture. The patient sustained a low-energy injury (fall) and presented within 12 hours with signs and symptoms of a compartment syndrome prior to any treatment. Emergent volar and dorsal fasciotomies were performed, and the fracture was reduced and pinned. The fracture and fasciotomies healed without complication, and the patient was doing well on the last follow-up examination. To our knowledge, this is the first report in the literature of a volar and dorsal compartment syndrome

A safe technique for distal biceps repair using a suture anchor and a limited anterior approach.

We describe a safe and simple method of repairing an acute rupture of the distal biceps tendon using a single limited (3 cm) anterior approach, a suture anchor, and the use of a plastic sheath (arthroscopy cannula or barrel of a 2 ml syringe) to protect the adjacent neural structures and to remove the need for extensive dissection and retraction in order to protect these structures. We have used this technique on three patients to date and all have regained a full range of flexion and extension at the elbow and pronation and supination of the forearm at a minimum of 6 months follow-up. [A brachial form of motor neuropathy caused by lead poisoning].

We report the case of a 48-year-old patient who developed acute onset bilateral, peripheral motor dysfunction in the arms and forearms. Clinical history, electromyography and laboratory tests confirmed the diagnosis of peripheral neuropathy caused by lead poisoning from the domestic water supply. The EMG showed axonal alterations. Improvement was observed after treatments with chelating agents and removal of the source of poisoning. This case provides the opportunity to recall the traditional 1889 Dejerine classification of lead-related peripheral neuropathies and is a warning signal that lead
