The osteochondral fractures (OCFs) of the patella are most common injuries of adolescence, usually associated with acute dislocation of patella. Although the acute cases are managed with a variety of options ranging from either conservative or removal or refixation, the latter seems to be the best option. While fixation using screws or bioabsorbable pins has been a standard practice, complications remain a concern. The use of suture fixation for treatment of such low-profile OCFs has gained popularity in recent times. However, each author has used the sutures in a random manner without a prede

There is a wide range of elbow injuries including chronic injuries such as lateral epicondylitis, medial epicondylitis, and cubital tunnel syndrome, or acute injuries such as elbow fracture-dislocations and distal biceps tendon ruptures. Combinations of acute and chronic elbow injuries have been reported including country club elbow and terrible triad of the elbow which are important to identify to properly treat. Thus, we report for the first time a new elbow injury triad termed Olympic elbow in a 65-year-old man comprising lateral epicondylitis, cubital tunnel syndrome, and a distal biceps t

Charcot arthropathy is a progressive condition primarily affecting the lower limbs in patients with diabetes mellitus. It is a rare complication of diabetic neuropathy and if left untreated can lead to severe limb destruction necessitating major amputation. Here, we report the case of a 41-year-old female who presented with rapidly progressive Charcot foot over a 10-day period, necessitating open reduction and internal fixation of Lisfranc-type fracture dislocations. Her presentation with a rapidly progressing red, swollen foot with a blister on the plantar aspect prompted initial treatment on…

We present a case report of a patient with acute upper and lower limb ischemia due to paradoxical embolism. A 67-year old woman without history of venous thromboembolism suffered dislocated patellar fracture requiring surgery in November 2017. Two months after surgery she presented to the emergency room with bilateral pulmonary embolism, occlusion of the left subclavian artery, left common femoral artery and superior mesenteric artery. Transesophageal echocardiography detected patent foramen ovale. Vascular surgeon decided against embolectomy, interventional radiologist against pharmacomechani

An 18-year-old military cadet presented to a direct-access physical therapy clinic 3 days after sustaining a lateral patellar dislocation that self-reduced with extension of the knee. Suspecting a loose body, the physical therapist ordered radiographs, which did not demonstrate any associated fractures. Rapid disposition was required to determine her potential to participate in mandatory Army field training, so the patient was referred to an orthopaedic surgeon, who ordered magnetic resonance imaging to assess for intra-articular pathology. Magnetic resonance imaging demonstrated a full-thickn

Treatment of patients presenting with type I aortic dissection is known to be a surgical challenge. The Djumbodis bare metal stent was developed to ease the burden of a reconstructive intervention on the aortic arch, to decrease the duration of the operation, and to minimize complications associated with artificial circulation. Currently, insufficiently explored remains the problem of complications occurring due to the technological fatigue of the metal and possible stent migration. Described herein is a clinical case report regarding migration of a Djumbodis stent, diagnosed 6 months after im

We reviewed the short-term outcome of 171 patients treated with total hip replacement for femoral neck fractures using the lateral (94 patients) or posterior approach (77 patients). The Sernbo score and the Abbreviated Mental Test Score were used to assess patients' pre-injury functional and cognitive status respectively. Patients were uniformly treated with regard to the type of prosthesis, size of femoral head and rehabilitation. They were followed up to a mean of 25 months (range : 13-42 months). The dislocation rate in the lateral group was 2.1% versus 0% in the posterior group. There was

The aim of this study was to evaluate complications related to prosthesis design in patients managed with cemented total knee arthroplasty using anterior-posterior glide mobile-bearing prosthesis. One hundred eighty-three total knee arthroplasties were performed using this prosthesis on 146 patients, with a minimum of 7 years of follow-up. There were late dislocations of the bearing in 2 knees, anterior soft tissue impingements in 6 knees at an earlier follow-up, excessive anteroposterior translation in 5 knees, and a fracture of the femoral component induced by osteolysis in 1 knee. Impact forces centered over the lateral orbital rim may cause fracture dislocations of the frontal process of the zygoma and the orbital plate of the sphenoid bone. These two major components of the lateral orbital wall may be impacted separately or as a unit into the posterior orbit to cause severe injuries to the eyeball or optic nerve. These ophthalmic injuries may be worsened by attempts to manipulate the bony fragments through the standard periorbital incisions. A safer approach is the temporal approach that exposes the entire lateral zygomatico-orbital region through a hemicoronal incisi…

Lowe oculocerebrorenal syndrome (OCRS), as previously described in the literature, consists of a well-defined constellation of clinical findings involving the eyes, cerebrum, and kidneys. However, the only musculoskeletal abnormalities reported in patients with OCRS have been joint hypermobility, recurrent fractures, rickets, tenosynovitis, and joint effusions. No other specific orthopedic problems have been described. This study presents new clinical findings of scoliosis, kyphosis, platyspondyly, dislocated and/or subluxed hips, and cervical spine abnormalities in six patients with OCRS trea

The spinous process (SP) is often neglected in the radiologic examination of the spine. Proper coning decreases scattered radiation to the SP's in the lateral view. Lateral zonography (thick cut tomography) is occasionally needed in the thoracic region. In the anteroposterior view paying attention to alignment of the SP's and the distance between them will help in the diagnosis of fracture and/or dislocation. Examination of the spinolaminal line (i.e. the line joining the base of the SP's) on the lateral view is also important in looking for the alignment of the vertebra in trauma. Local absen

BACKGROUND: The influence of surgical approach on risk of early postoperative mechanical complications after total hip arthroplasty (THA) continues to be a focus of debate. We performed the first single-institution study on risk of early operative and nonoperative mechanical complications after THA based on approach, with the hypothesis that there would be no clinically significant difference with modern surgical methods.
METHODS: A retrospective study was conducted on 16,186 consecutive THA performed from 2010 to 2016. Revision or conversion THA and cases performed for hip fracture, with rec

Traumatic lateral spondyloptosis is mostly a lateral shearing injury that must be tremendous enough to completely disrupt the strong musculoligamentous and bony structures. This injury has only been described at single levels in the lumbar spine. Lateral expulsion of a vertebral body from the spinal column due to 2-level adjacent spondyloptosis has not been previously reported. This 16-year-old girl was referred to our center for the management of an extremely unusual L2-5 fracture-dislocation. Motor deficits were incomplete and sacral sensation was spared. BACKGROUND: The lateral impaction force against the shoulder often causes multiple lesions to the forequarter including the chest wall. The purpose of this study is to describe the injury patterns and associated pathology in patients with combined chest wall and shoulder girdle injuries- forequarter lateral implosion injury and organize the findings into a classification framework to predict morbidity.
METHODS: Three hundred fifty-four consecutive patients with two or more rib fractures and ipsilateral shoulder girdle injury from a Level 1 trauma center, between 2018-2022, were retrospectivel

BACKGROUND: The choice between operative or nonoperative treatment is questioned for partial articular fractures of the radial head that have at least 2 millimeters of articular step-off on at least one radiograph (defined as displaced), but less than 2 millimeter of gap between the fragments (defined as stable) and that are not associated with an elbow dislocation, interosseous ligament injury, or other fractures. These kinds of fractures are often classified as Mason type-2 fractures. Retrospective comparative studies suggest that operative treatment might be better than nonoperative treatme

PURPOSE: To discuss when and how to operate on thoracic and lumbar spine fractures.
PATIENTS AND METHODS: We retrospectively studied 77 consecutive patients with thoracic and lumbar spine fractures treated from 2000 to 2011; 28 patients experienced high-energy spinal trauma and 49 low-energy spinal trauma. Mean follow-up was 5 years (1-11 years). Surgical treatment was done in 15 patients with neurological deficits, and in 16 neurologically intact patients with fractures-dislocations, burst fractures and fractures with marked deformity. Non-surgical treatment was done in 46 neurologically int

AIM: A review of condylar fractures treated ''non-surgically'' and ''surgically'' over the last 5 years (1996-2001) at the Maxillofacial Surgery Division of the Brescia Civil Hospital (Italy) is presented. The non surgical and surgical treatments carried out, as well as their effectiveness and therapeutic aims, are illustrated.
METHODS: From June 1996 to June 2001, at the Maxillofacial Surgery Division of the Brescia Civil Hospital, 179 fractures of the mandibular condyle were treated. Of the 179 cases treated, a distinction was immediately made between ''non-surgical'' (103 cases) and ''surg

Increasing numbers of total hip arthroplasties in combination with increasing age and growing daily activities of the elderly lead to increasing numbers of periprosthetic fractures and revision arthroplasties in osteoporotic bone. The prosthesis nail is a hybrid of a hip prosthesis and an intramedullary nail allowing immediate full weight bearing and early rehabilitation. The prosthesis nail consists of three self-locking components: a distally locked intramedullary nail, different lengthening modules, and a hip prosthesis module. From 1992 to 1999, 28 prosthesis nails were implanted in 26 pat

PURPOSE OF THE STUDY: Most pertrochanteric fractures can be successfully fixed with osteosynthesis. Osteosynthesis fails however is a small number of patients who require re-operation for implantation of a total hip prosthesis. This situation occurs in particular when the material has penetrated the acetabulum and in elderly subjects. Although this type of arthroplasty is routine practice, few series have been reported. We present here outcome and complications of total hip arthroplasty after failure of per- and sub-trochanteric fracture fixation.
MATERIAL AND METHODS: Between 1990 and 2000,

PURPOSE: Revision total joint arthroplasty cases including those complex enough to require limb-salvage procedures are expected to significantly increase. Total femoral replacements represent a limb-preserving procedure with potential utility for these complicated cases. This review seeks to summarize the outcomes of total femoral replacements when used in the revision arthroplasty setting.
METHODS: A systematic review was performed by searching Embase, Ovid MEDLINE, PubMed, Wiley Cochrane Library: Central Register of Controlled Trials, and Thompson Reuters Web of Science: Citation Index on F
