The effect of exercise training on delta Vo2/delta WR was studied in 12 patients(11 men, one woman, mean age 62 +/- 9 yr)with chronic heart failure(old myocardial infarction, dilated cardiomyopathy, patients after coronary arterial bypass graft surgery and patients after aortic valve replacement). Cardiopulmonary exercise testing was performed to decide the exercise tolerance and assess the delta Vo2/delta WR. Patients underwent physical training at the anaerobic threshold for 3 months. Cardiopulmonary exercise testing was performed after the second week and after the third month. The anaerobi…

Partial left ventriculectomy (PLV) surgery has gained significant attention worldwide as a possible alternative to heart transplantation or as a therapy to treat heart failure. This procedure offers an attractive choice to patients and physicians because of the lack of need for immunosuppressive medications and a decrease in the risk of rejection associated with transplantation. PLV surgery is currently under investigation in several centers worldwide, but long-term outcomes have yet to be definitively evaluated. The Cleveland Clinic Foundation began performing PLV surgery in May 1996. Since t…

Chronic heart failure (CHF) is present in 1-10% of the whole population. From the drugs used in CHF so far only angiotensin converting enzyme inhibitors and nitrate with hydralazine improved the survival which was caused by beneficial influence of these drugs on neurohormonal factors. There is growing interest in beta-blockers which are believed to stop the progress of CHF. Although the first attempts of using beta-blockers in the treatment of CHF took place in 70-ties, only recent years brought better understanding of mechanisms of their action. The beneficial effect of beta-blockers in CHF i…

This study investigated the effect of chronic digitalis glycoside use on beta-adrenergic sympathetic activities in heart failure. Twenty-two Japanese white rabbits were anesthetized by intravenous injection of chloral hydrate. Aortic regurgitation (AR) was produced by perforating aortic valves in 14 rabbits. Digoxin was given for 1 week to 7 rabbits with AR (AR + Dig) and saline to 7 rabbits with AR (AR + C). Sham operation was performed in the remaining 8 rabbits (S). The left ventricular end-diastolic pressure was higher in AR + C than S (p < 0.05). It was lower in AR + Dig than AR + C (p < …

OBJECTIVES: We sought to test the hypothesis that suppression of chemosensitivity (respiratory response to arterial blood gases) with dihydrocodeine may improve dyspnea and exercise tolerance in patients with chronic heart failure.
BACKGROUND: Exertional dyspnea is a common limiting symptom in patients with chronic heart failure. The mechanisms underlying this symptom are not fully understood but may be related to increased ventilation caused, in part, by the augmentation of chemosensitivity. Suppression of chemosensitivity with mild opiates may thus improve this symptom as well as exercise t…

HISTORY AND CLINICAL FINDINGS: A 58-year-old woman was admitted because of jaundice, ascites and marked oedema. For three years she had suffered from nervousness, decreasing fitness and weight loss, which had been assumed as due to chronic alcoholism. Liver biopsy revealed extensive fibrosis, in part with early cirrhotic transformation. This was followed by cardiac failure with atrial fibrillation (ventricular rate 140/min) and marked pleural effusions. The thyroid was diffusely enlarged and there were signs of exophthalmos.
INVESTIGATIONS: Bilirubin concentration was 3 mg/dl, lactate dehydro

OBJECTIVE: To investigate the effects of prostaglandin E1 (PGE1) by intravenous drip in cor pulmonale patients with heart failure.
METHODS: Thirty caseas of cor pulmonale with heart failure were treated with PGE1 at a dose of 200 micrograms.d-1 for 5 days and in 30 patients with cor pulmonale heart failure (no PGE1) and in 20 healthy subjects as controls. The level of serum endogenous digoxin-like factor (EDF), serum lipoid superoxide (LPO), serum superoxide dismutase (SOD) and blood PaO2, PaCO2 were determined. Their symptoms and signs were observed before and after treatment.
RESULTS: The …

A 64-year-old man, who had had bilateral intermittent claudication and leg pain for five years, was admitted because of sudden onset of severe leg pain, and acute respiratory failure. Laboratory data showed markedly elevated serum CK of 73,050 IU/L, and urinary myoglobin of 430,000 ng/ml. A diagnosis of rhabdomyolysis was made. Renal dialysis was required for the next two days because of acute renal failure. The aortogram was performed on the 32nd day and disclosed complete obstruction of the abdominal aorta immediately distal to the bilateral renal arteries. Good collateral flow was noted to

A case of a pregnant woman who underwent urgent cesarean section is presented. The patient had severe hyperthyroidism, preeclampsia, and congestive heart failure, which had not been treated until 36 weeks of gestational age. At 38 weeks, fetal distress occurred and an urgent cesarean section was performed successfully under epidural anesthesia with preoperative treatments using iodide, hydrocortisone and propylthiouracil. The patient required postoperative intensive care for heart failure. Thyroid function of the neonate was almost normal. No abnormality was observed except low birth weight.

BACKGROUND: Quality of life is an important outcome to measure in patients with end-stage heart disease who are awaiting heart transplantation. The purposes of this study were threefold: (1) to assess life satisfaction in multiple areas, (2) to examine correlations between life satisfaction and demographic, physiologic, and psychosocial variables, and (3) to identify predictors of quality of life in patients with advanced heart failure who were awaiting heart transplantation.
METHODS: Data were collected from a convenience sample of 359 adult heart transplant candidates from a midwestern and …

Furosemide is frequently administered intravenously to patients with chronic heart failure. However, use of diuretics may cause neuroendocrine activation and by itself may not consistently afford diuresis. Atrial natriuretic factor (ANF) in pharmacologic doses is a vasodilator and has favorable neuroendocrinologic effects in patients with congestive heart failure. To examine whether exogenous ANF might enhance the effects of acute furosemide injection, we studied 14 patients with chronic stable heart failure and measured the effects of the combination of ANF and furosemide on hemodynamics, neu…

A new (quality of life) visual analogue scale of dyspnoea has been designed to evaluate the severity of dyspnoea in patients with congestive heart failure (CHF). The index has 10 numerical values, rated on a scale from 0-10, each value corresponding to an intensity of breathlessness. Since dyspnoea is a primary symptom that restricts the quality of life in patients with congestive heart failure, the new scale provided a simple, easily comprehensible and inexpensive instrument to assess the severity of breathlessness. We also report the sensitivity of the visual scale of dyspnoea before and aft…

We operated on a 47-year-old male for Tetralogy of Fallot with severe right heart dysfunction (RVEF 25%, RVEDP 15 mmHg, massive TR) and right heart failure (ascites and edema). The operation was completed successfully. However, persistent severe arrhythmia occurred in the CCU and assisted circulation was used for 6 days until the patient's condition stabilized. Arrhythmia diminished and hemodynamic status improved gradually. The patient improved sufficiently to be able to eat and read books. Unfortunately, he died of sudden ventricular arrhythmia on 69 POD. It was suspected that the right vent…

The possibilities of the classification developed by Lang, Strazhesko, Vasilenko and that of heart failure functional classes, which has been adopted by the New York Heart Association (NYHA), to evaluate the status of patients with severe heart failure are compared in this communication. A total of 1619 patients with Stages IIB-III heart failure of various origin (coronary heart disease, dilated cardiomyopathy, rheumatic heart disease, myocarditis, primary pulmonary hypertension) were studied. To assess the patients' status the attending physicians used 7 grades: (1) satisfactory; (2) close to…
