A retrospective study of complications of therapeutic plasma exchange in myasthenia.

INTRODUCTION: Venous access for therapeutic plasma exchange (TPE) in myasthenia gravis (MG) can be achieved by central venous catheters (CVC) or peripheral veins (PV), and the preferred method varies among providers. We evaluated our institutional experience with TPE venous access method and complications. METHODS: We reviewed all TPE-treated MG patients (2005-2010) through blinded chart review. TPE complications were categorized as serious or minor. Serious complications ended the procedure and/or were potentially life-threatening. RESULTS: A total of 134 MG patients received 230 TPE course…

Electrodiagnosis of disorders of neuromuscular transmission.

This article reviews the use of electrodiagnostic testing in disorders of neuromuscular transmission and discusses the differences between various presynaptic and postsynaptic disorders. Attention is paid to quality control issues that influence the sensitivity of repetitive nerve stimulation and single fiber electromyography. Electrodiagnostic testing, when used as an extension of the clinician's history and physical examination, will provide appropriate direction in establishing the diagnosis.

Combined Guillain-Barré syndrome and myasthenia gravis.

BACKGROUND: Guillain-Barré syndrome and myasthenia gravis both lead to muscle weakness but the two combined is uncommon. Detection of these entities can help identify forms of autoimmune neuromuscular diseases that may respond to immunotherapy. This report sought to characterize the clinical features of these two entities when combined. METHODS: This report is of a case of combined Guillain-Barré syndrome and myasthenia gravis. The clinical features were analyzed and correlated to those published in English literature from 1960 to 2012. Ten reports and 12 cases, including the present case, we…

β-glucans reduce LDL cholesterol in patients with myasthenia gravis.

We aimed at evaluating whether β-glucans could improve low-density lipoprotein (LDL) cholesterol levels and/or glycemic control in patients with myasthenia gravis (MG), in whom statins usually have muscle-related side effects. Fifty-nine MG patients participated in the study and received a daily dietary supplement of 3 g of β-glucans during 8 weeks. Body mass index (BMI) and blood sample analysis of lipid and glucose status were performed before and after 8 weeks intake of β-glucans. In the 52 patients who completed the study, there was a significant reduction in total cholesterol, LDL, ApoA1 …

[Recent clinical trials on treatment of myasthenia gravis].

In the past decade, the therapeutic choices for patients with myasthenia gravis (MG) have not changed, except for the introduction of rituximab. There have been three placebo-controlled randomized trials in the past fifteen years, namely, those on azathioprine (AZP), mycophenolate mofetil (MMF) and tacrolimus. The trial for AZP was carried out on a relatively small number of patients (AZP, 15; placebo, 19), and its outcome showed the therapeutic effectiveness of AZP. The MMF trial was carried out on a large number of patients (MMF, 88; placebo, 88). The result showed no effect on primary and s…

[The MG-QOL15 Japanese version: validation and associations with clinical factors].

We produced a Japanese translation of the 15-item myasthenia gravis (MG)-specific quality of life (QOL) scale (MG-QOL15), assessed its reliability and validity, and examined clinical factors affecting the self-perceived QOL in MG. Consecutive 327 patients with MG seen at six neurological centers were evaluated. All patients completed an MG-QOL15 Japanese version (MG-QOL15-J), the Beck Depression Inventory-Second Edition (BDI-II), and a generic health-related QOL questionnaire, the SF-36. Disease severity was determined according to the MG Foundation of America (MGFA) quantitative MG score and …

[Directions for use of corticosteroids and calcineurin inhibitors against generalized myasthenia gravis: therapeutic strategies that can lead to early improvements and veer away from high-dose oral corticosteroids].

The advent of effective immune treatment has meant that myasthenia gravis (MG) is most often not lethal. However, many MG patients still find it difficult to maintain daily activities due to chronic residual fatigability and long-term side effects of medication, since full remission without immune treatment is not common. Our analysis demonstrated that disease severity, dose of oral corticosteroids, and depressive state are the major independent factors negatively associated with self-reported QOL (MG-QOL15-J score). It is noteworthy that oral corticosteroid, the first-line agent for MG, is ne…

[Plasmapheresis and immunoglobulin therapy].

Plasmapheresis (PP) and intravenous immunoglobulin (IVIG) are both effective for treatment of crisis and acute exacerbation of myasthenia gravis. The 1(st) clinical problem is that effect of both treatments usually lasts for only 2-3 weeks. Additional immunosuppressive treatments may be necessary for prevention of the relapse. The 2(nd) problem is adverse effects and complications associated with each treatment. Hypotension, including shock, thromboembolism, and opportunistic infections are major adverse events of PP. Awareness of these complications and careful practice should be solutions. I…

[Indication of extended thymectomy in patients with myasthenia gravis].

Recently, the prognosis of myasthenia gravis (MG) has improved considerably because the commercial measurement of acetylcholine receptor (AChR) antibody has been introduced and early treatment with steroid at large quantities has been utilized. On the other hand, the efficacy and the safety of thymectomy for MG patients without thymoma are still controversial. A meta-analysis of thymectomy for MG patients without thymoma revealed an uncertain effect. Since 2005, a controlled randomized study for thymectomy in patients with MG has been in progress at 66 centers in 16 countries. This study compa…

[Progress of myasthenia gravis: discovery of Lrp4 antibodies].

Myasthenia gravis (MG) is caused by the failure of neuromuscular transmission mediated by pathogenic autoantibodies (Abs) against acetylcholine receptor (AChR) and muscle-specific receptor tyrosine kinase (MuSK). The seropositivity rates for routine AChR binding Ab and MuSK Ab in MG are 80-85% and 5-10% for MG patients in Japan, respectively. The autoimmune target in the remaining patients is unknown. In 2011, autoantibodies against low-density lipoprotein receptor-related protein 4 (Lrp4) were identified in Japanese MG patients and thereafter have been reported in Germany and the USA. We deve…

[Anti-MuSK antibodies in myasthenia gravis block binding of collagen Q to MuSK].

In myasthenia gravis (MG), 5-15% of patients are positive for MuSK antibodies. MuSK binds to LRP4 and transmits an agrin-mediated signal for clustering of acetylcholine receptor (AChR). MuSK also anchors the collagenic tail subunit (ColQ) of acetylcholinesterase (AChE). MuSK-IgG is a blocking antibody, but its molecular targets remained elusive. As acetylcholine receptor (AChR) deficiency is typically mild and as cholinesterase inhibitors are generally ineffective in MuSK-MG patients, we asked if MuSK-IgG interferes with binding of ColQ to MuSK. The in vitro overlay assay showed that MuSK-IgG …

[The impairment of excitation-contraction (E-C) coupling in myasthenia gravis].

We have developed a new novel method to assess the function of excitation-contraction (E-C) coupling in patients with myasthenia gravis (MG). In our procedure, masseteric compound muscle action potential (CMAP) and mandibular movement-related potentials (MRP) were recorded simultaneously after stimulating the trigeminal motor nerve with a needle electrode. The E-C coupling time (ECCT) was calculated by the latency difference between CMAP and MRP. Bite force was measured using a pressure-sensitive sheet. Our serial studies demonstrate that masseteric E-C coupling is impaired in some MG patients…

[New clinical entity of myasthenia gravis with autoimmune targets of heart and skeletal muscles].

Since previous case reports have shown inflammatory myopathies in a few patients with myasthenia gravis (MG), heart and skeletal muscles are speculated to be autoimmune targets in MG. We screened to investigate the clinical, histological and immunological features of MG patients, who also developed myocarditis and/or myositis at 5 Japanese hospitals. Of 924 MG patients, eight (0.9%) had inflammatory myopathies. The onset age of MG was 55.3 ± 10.3 years. All patients showed severe symptoms with bulbar involvement accompanied by myasthenic crisis in 5 and invasive thymoma in 4. Myocarditis was f…

MiR-320a is downregulated in patients with myasthenia gravis and modulates inflammatory cytokines production by targeting mitogen-activated protein kinase 1.

Myasthenia gravis (MG) are T-cell dependent antibody-mediated autoimmune disorders, microRNAs are important regulators of human autoimmune disease pathogenesis. Here, we investigated the miRNAs expression profiles in MG for the first time and found that miR-320a was significantly downregulated in MG patients compared to normal healthy people. Meanwhile, pro-inflammatory cytokins in MG patients were overexpressed. Furthermore, we identified MAPK1 as a direct target of miR-320a. Downregulation of miR-320a induced the overexpression of pro-inflammatory cytokins through promoting COX-2 expression.…
