Vitamin D insufficiency increases risk of chronic pain among African Americans experiencing motor vehicle collision.

African Americans experience an increased burden of motor vehicle collision (MVC), post-MVC musculoskeletal pain, and vitamin D insufficiency. In this prospective multicenter study, we tested the hypothesis that African Americans (n = 133) presenting to the emergency department after MVC with low peritraumatic vitamin D levels would have worse chronic musculoskeletal pain outcomes compared to individuals with sufficient vitamin D. Vitamin D levels were assessed in the early aftermath of MVC through enzyme-linked immunosorbent assay, and pain severity was assessed using the 0 to 10 numeric rati

Antiphospholipid Syndrome With Pulmonary Embolism, Cerebral Venous Sinus and Cardiac Thrombosis in a 17-Year-Old.

A 17-year-old boy initially presented with syncope, fever, headache, and vomiting. Examination revealed neck stiffness and bilateral papilledema. Imaging identified superior sagittal and left transverse sinus thrombosis. One year later, he developed chest pain, seizures, and bilateral pulmonary emboli, with chocardiography demonstrated intracardiac thrombi. Surgical excision confirmed fibrinous thrombi. Despite anticoagulation, recurrent intracardiac thrombi and pulmonary hemorrhage occurred. Thrombophilia workup confirmed antiphospholipid syndrome (APS). This rare case highlights APS-related

Internal carotid artery dissection associated with an elongated hyoid bone in a patient with vascular Ehlers-Danlos syndrome.

Hyoid bone-related carotid injury is a rare cause of neurovascular events. This report describes a case of a young, healthy male presenting with neck pain followed by left-sided hemiparesis. The patient was diagnosed with a transient ischaemic attack attributed to structural damage of the vascular surface of the right internal carotid artery as a direct result of continuous compression by an elongated hyoid bone. We describe a successful diagnosis using a series of manoeuvres during a six-vessel cerebral angiogram. Genetic testing later confirmed the diagnosis of vascular Ehlers-Danlos syndrom…

Cavernous Sinus Thrombosis Associated With Contralateral Pansinusitis: A Case Report.

A healthy woman in her 60s presented acutely with right-sided periorbital edema, erythema, neck pain, and fever. Initial examination and investigations suggested right-sided orbital cellulitis with contralateral pansinusitis. Despite treatment with IV antibiotics, her symptoms worsened, and inflammatory markers continued to rise. Further imaging revealed thrombosis of the right superior ophthalmic vein extending into the cavernous sinus and right jugular vein. She underwent urgent left functional endoscopic sinus surgery followed by long-term anticoagulation. After six months of anticoagulatio…

Bilateral Renal Lymphangiomatosis.

Lymphangiomatosis is a rare and benign mesenchymal disorder where there is developmental malformation and dilatation of lymphatic channels. The most common sites for lymphangiomatosis are the cervical neck and axilla, but it can also occur in the mediastinum, retroperitoneum, mesentery, omentum, colon, and pelvis. Renal lymphangiomatosis is an even rarer multicystic disorder which may be hilar, pericalyceal, paracalyceal, peripelvic, or para pelvic. We report a case of 24-year-old female with bilateral perirenal lymphangiomatosis. Her complain was bilateral flank pain and bilateral perinephric

Cervical Spine, Upper Extremity Neuropathies, and Overuse Injuries in Cyclists.

Upper extremity nerve injuries in cyclists include carpal tunnel syndrome and ulnar neuropathy at the wrist. Electromyography and nerve conduction studies aid in the diagnosis of neuropathies. Diagnostic ultrasonography or MRI can also be helpful for evaluation. Overuse injuries in the upper extremity includes biker's elbow, or a tendinopathy of the common flexor or extensor tendons, which is more common in mountain biking. Neck pain is also a common issue for cyclists. Treatment of these conditions varies from conservative management to surgical options, but a bicycle fit assessment is recomm…

Post-COVID-19 Kawasaki-Like Syndrome.

A 42-year-old man was referred for a week history of severe dysphagia, odynophagia, fever (39 °C), fatigue, abdominal pain, pharyngeal swelling, and multiple neck lymphadenopathies. The medical history reported a mild form of COVID-19 one month ago. The biology reported an unspecified inflammatory syndrome. The patient developed peritonitis, myocarditis, and hepatitis. A myocardium biopsy was performed. A diagnosis of Kawasaki-like disease (KLD) was performed. The occurrence of KLD in adults is rare but has to be known by otolaryngologists regarding the otolaryngological clinical presentation

Asymptomatic Internal Carotid Aneurysm: An Uncommon Disease of the Carotid Arteries.

Despite a low-incidence extracranial carotid artery aneurysm (ECAA) disease has important clinical repercussion that obliges understanding and knowledge of correct treatment. The 2 dominant etiologies are atherosclerotic degeneration and pseudoaneurysm. The natural history of ECAAs is understood. Neck pain, a pulsatile mass and central or peripheral neurological manifestations are the most common symptoms. Recommendations for diagnosis and treatment are not uniform and still under discussion, representing a challenge for clinicians. We discuss a case of 2.5 cm asymptomatic saccular atheroscler…

Subacute thyroiditis associated with COVID-19.

We report a case of a hospitalised patient with COVID-19 who developed subacute thyroiditis in association with SARS-COV-2 infection. The patient presented with tachycardia, anterior neck pain and thyroid function tests revealing hyperthyroidism together with consistent ultrasonographic evidence suggesting subacute thyroiditis. Treatment with corticosteroids resulted in rapid clinical resolution. This case illustrates that subacute thyroiditis associated with viruses such as SARS-CoV-2 should be recognised as a complication of COVID-19 and considered as a differential diagnosis when infected p…

Wild Poliovirus Type 1 in Oman: A re-emerging threat that requires urgent, targeted and strategic preparedness.

Poliomyelitis, also known as polio, is a highly infectious viral disease, predominantly affecting children under five years old. The virus is transmitted from person-to-person and mainly spreads through the fecal-oral route. The virus multiplies in the intestine, from where it can invade the nervous system via the bloodstream, potentially causing paralysis. Polio symptoms include fever, fatigue, headache, vomiting, neck stiffness and pain in the limbs. The disease causes permanent paralysis in one out of 200 infections. Currently, there is no cure for polio; it can only be prevented by immunis

Bezold's abscess: A rare complication of suppurative mastoiditis.

Bezold's abscess is a suppurative complication of mastoiditis rarely seen in the current era of antimicrobials. It can lead to seriously sequelae if not diagnosed and treated early. We describe a patient with recurrent bilateral otalgia who had received multiple courses of antimicrobials who presented with severe headache, neck pain and right sided hearing loss. Imaging studies showed chronic mastoiditis and formation of a Bezold's abscess. Drainage was performed but symptoms persisted. Our report highlights the need for further evaluation of patients presenting with complicated mastoiditis an…

[Two cases of leptospirosis with involvement of the central nervous system].

A 26-year-old woman developed fever, myalgias and headaches two weeks after having been scratched by a rat, which she kept as a pet. She had limited renal involvement and marginal pleocytosis in the cerebrospinal fluid (CSF). A 51-year-old man had just returned from a holiday with water sports activities and was admitted with fever, severe headaches, neck pain, myalgias and signs of acute renal failure and pleocytosis in the CSF. Both patients had positive Leptospira DNA polymerase chain reaction in their CSF and were treated to full remission.

[A rare complication of transrectal electrostimulation for sperm retrieval].

In men who wish to have children after a spinal cord injury, assisted ejaculation is frequently needed for sperm retrieval. Transrectal electrostimulation (TES) is often used for this purpose. Typical side effects of TES are autonomic dysreflexia or pain. In a 33-year old man with complete tetraplegia below C6 since 2004, TES caused massive leg spasticity, leading to transcervical fracture of the femoral neck. This previously unreported complication of TES demonstrates that, in men with long-term chronic tetraplegia, spasticity and osteoporosis, TES in anesthesia should be taken into considera

Subcutaneous Emphysema in Acute Asthma: A Cause for Concern?

Pneumomediastinum has been described in patients with asthma. In this case report, we describe a young patient who presented to our medical assessment unit with an asthma exacerbation and progressive dyspnea. The patient developed pneumomediastinum, a rare complication of an asthma exacerbation. Pneumomediastinum is usually characterized by chest pain, dyspnea, and neck swelling caused by subcutaneous emphysema. Although the condition is usually benign and treatment is primarily supportive, surgical intervention may be needed if the patient develops hemodynamic compromise or respiratory failur

From four-parathyroid gland exploration to a minimally invasive technique. Minimally invasive parathyroidectomy as a current approach in surgery for primary hyperparathyroidism.

Complete surgical resection of hyperfunctioning parathyriod tissue is essential for the treatment of primary hyperparathyroidism. During recent years, minimally invasive surgery has been successfully applied in neck exploration, because of significant developments of guidance by intraoperative scans, the use of quick, intraoperative PTH assay, and also preoperative imaging procedures such as high resolution ultrasonography and sestamibi scintigraphy. The results of operations which are performed with minimally invasive techniques are comparable to those of conventional surgery, and provide adv

Clinical reasoning: a 42-year-old man who developed blurred vision and dropped his iPod while jogging.

A 42-year-old man noted sudden onset of blurriness in his left eye and dropped his iPod from his right hand while jogging. In the emergency room, it was noted that visual blurring resolved with right eye closure, but his ophthalmologic examination was otherwise normal. He had subtle right nasolabial fold flattening and right arm pronator drift. His examination was otherwise normal. He reported no headache, neck pain, prior trauma, prior transient neurologic deficit, or palpitations. He took no medications and did not smoke, drink alcohol, or use illicit drugs.

Clinical practice. Giant-cell arteritis and polymyalgia rheumatica.

A 79-year-old woman presents with new-onset pain in her neck and both shoulders. She takes 7.5 mg of prednisone per day for giant-cell arteritis. Occipital tenderness and diplopia developed 11 months before presentation. At that time, her erythrocyte sedimentation rate was elevated, at 78 mm per hour, and a temporal-artery biopsy revealed granulomatous arteritis. The diplopia resolved after 6 days of treatment with 60 mg of prednisone daily. Neither headache nor visual symptoms developed when the glucocorticoids were tapered. How should this patient’s care be managed?

Bilateral subcapital femoral neck fractures secondary to transient osteoporosis during pregnancy: a case report.

Transient osteoporosis during pregnancy is a rare, self-limiting disease. We report on a 36-year-old woman who had bilateral subcapital femoral neck fractures during the 6th month of pregnancy. The diagnosis was made 4 days after delivery, because radiography was declined by the patient for fear of radiation. Fixation was not feasible owing to bone resorption, and 2-stage bipolar hemiarthroplasty was therefore performed. Magnetic resonance imaging is the best non-invasive investigative tool for pregnant women with hip pain. Early detection can prevent complications and resorting to major surge

Subcutaneous emphysema, pneumopericardium, pneumomediastinum and pneumoretroperitoneum secondary to sigmoid perforation: a case report.

A 50-year-old woman presented with chronic epigastric abdominal pain and constipation. She underwent diagnostic upper and lower endoscopy for further evaluation. Several hours following the procedure, she developed chest and subcutaneous emphysema of her upper chest, neck, and face. A chest X-ray demonstrated marked subcutaneous emphysema, pneumopericardium, and pneumomediastinum. A CT scan revealed a small leak at the rectosigmoid junction. Because the patient did not have peritoneal signs, she was treated conservatively and discharged on hospital day seven. The complications of both esohagog

CSF bone cyst in a cervical vertebra.

A 43 year male patient presented with neck pain, upper limb paraesthesia and right foot weakness. A MRI scan revealed a bone cyst involving C4 spinous process, lamina, pedicles and the posterior part of the vertebral body causing focal spinal stenosis. A C4 laminectomy and C3-C5 lateral mass screw fixation was done. Intra-operatively the cyst was found to contain CSF with an associated dural defect. A CSF cervical vertebral cyst has not been previously reported in the literature. The clinical presentation, radiological features and management of this CSF vertebral cyst is discussed.
