Obesity hypoventilation syndrome: Current status and future directions for optimizing care of a complex and diverse condition (a narrative review).

The global obesity pandemic contributes to an increase in the prevalence of obesity hypoventilation syndrome (OHS). OHS is associated with poor prognosis and early mortality. Definitions of OHS and disease severity classifications differ between international guidelines, and consideration of polysomnographic features is often lacking. To address this, the European Respiratory Society has proposed a severity classification approach. It is also important to consider the possibility that patients with OHS might have multiple factors contributing to hypercapnia, including obesity-related changes i

Alkindi Sprinkle for Pediatric Patients With Primary Adrenocortical Insufficiency: A Narrative Review.

Adrenocortical insufficiency, also known as adrenal insufficiency (AI), is an endocrine disorder characterized by inadequate production of adrenal hormones, including glucocorticoids and mineralocorticoids (MCs). The condition can be categorized as primary, secondary, or tertiary AI, depending on the location of the defect. Classical symptoms of AI include weakness, fatigue, abdominal pain, tachycardia, hypotension, electrolyte imbalances, and hyperpigmentation. In children, the most common cause of AI is classical congenital adrenal hyperplasia, which results from a deficiency in the 21-hydro

The disproportionately large contribution of the Māori and Pacific Islander community to the healthcare burden of gout in Western Sydney.

BACKGROUND: Gout is a common chronic inflammatory disorder due to monosodium urate deposition, which results in severe inflammatory arthritis. It is particularly common in those of Māori or Pacific Islander heritage. There is a significant number of this at-risk ethnic group in western Sydney. AIMS: To determine the healthcare burden of gout in Western Sydney. METHODS: We characterised patients managed in the emergency departments (EDs) of the four Western Sydney Local Health District (WSLHD) hospitals and those admitted for gout as the primary or secondary diagnosis from 1 January 2017 to 3

Fluid management in children with severe dengue: a narrative review.

Dengue is a mosquito-borne arboviral infection of increasing public health importance. Globally, children account for a significant proportion of infections. No pathogen-specific treatment currently exists, and the current approach to reducing disease burden is focused on preventative strategies such as vector control, epidemiological interventions, and vaccination in selected populations. Once infected, the mainstay of treatment is supportive, of which appropriate fluid management is a cornerstone. The timely provision of fluid boluses has historically been central to the management of septic

Parallel Assessment of Chemotherapy Adherence and Supportive Therapy Adherence on Occurrence and Minimization of Adverse Drug Reactions Among Cancer Patients: A Clinical-Based Observational Study.

Background: Cancer is a disease that is inevitably treated using chemotherapy, but the cytotoxic drugs used in the treatment have the potency to cause adverse drug reactions (ADRs). Thus, supportive therapy plays an essential role in managing the untoward effects of the cancer drugs in patients. This highlights the importance of medication adherence in managing the disease, mitigating and preventing the occurrence of chemotherapy-induced ADR without compromising the health status of the cancer population. Objective: To assess the adherence to chemotherapy and supportive therapy and to evaluate

Emergency response to the explosive growth of health care wastes during COVID-19 pandemic in Wuhan, China.

During the Coronavirus Disease 2019 (COVID-19) as a worldwide pandemic, the security management of health care wastes (HCWs) has attracted increasing concern due to their high risk. In this paper, the integrated management of HCWs in Wuhan, the first COVID-19-outbreaking city with over ten millions of people completely locking down, was collected, investigated and analyzed. During the pandemic, municipal solid wastes (MSWs) from designated hospitals, Fangcang shelter hospitals, isolation locations and residential areas (e.g. face masks) were collected and categorized as HCWs due to the high in

Health system barriers to care for lesbian, gay, bisexual, transgender, and intersex populations in Nepal: A qualitative multi-stakeholder study.

Lesbian, gay, bisexual, transgender, and intersex (LGBTI) individuals face persistent barriers to healthcare in many settings. However, little is known about how health-system factors shape inequities in Nepal. This study aimed to examine the experiences of care, barriers to inclusive services, and opportunities to strengthen the health system for LGBTI individuals in Nepal. We conducted in-depth qualitative interviews using an interview guide with 28 participants-LGBTI community members, healthcare providers, leaders of LGBTI-led organizations, and policy stakeholders-in Nepal. Data were anal

MAFLD in Vietnam: a neglected public health challenge requiring urgent policy action.

Metabolic dysfunction-associated fatty liver disease (MAFLD) is rapidly emerging as a major public health challenge in Vietnam, driven by rising rates of obesity, type 2 diabetes, and lifestyle changes. Although it contributes significantly to morbidity, mortality, and economic burden, MAFLD remains under-recognized within national health strategies and is largely neglected in non-communicable disease (NCD) frameworks. This narrative review synthesizes epidemiological, clinical, and health systems data on MAFLD in Vietnam, drawing on studies published between 2015 and 2024 alongside internatio

Training competencies and recommendations for the next generation of public health genetics: Reflections from current leaders in the field.

Public health genetics training programs must evolve to meet the changing public health landscape and ever-growing data availability. Yet, the field's interdisciplinary nature poses challenges to training program development. Here, we conducted focus groups (n = 7) with public-health-genetics-related professionals (n = 55) to learn about the skills and knowledge needed for a career in public health genetics. Additional discussion topics included perspectives on curriculum structure, analytic skills, and unneeded content. Focus group transcripts underwent thematic analysis, from which competenc

Hearing loss and cognitive decline: Prioritizing equity in a world in which hearing health matters.

Alzheimer's disease and related dementias (ADRDs) and age-related hearing loss are the intersection of two major public health challenges. With age as the primary risk factor for both disease processes, the burden of ADRDs and age-related hearing loss is growing, and each field maintains significant barriers to broadscale identification and management that is affordable and accessible. With the disproportionate burden of ADRDs among racial and ethnic minority older adults and existing disparities within hearing care, both areas face challenges in achieving equitable access and outcomes across

Mycobacterium paratuberculosis zoonosis is a One Health emergency.

A singular pathogen has been killing animals, contaminating food and causing an array of human diseases. Mycobacterium avium subspecies paratuberculosis (MAP) is the cause of a fatal enteric infectious disease called Johne's (Yo'-nees), a disorder mostly studied in ruminant animals. MAP is globally impacting animal health and imparting significant economic burden to animal agriculture. Confounding the management of Johne's disease is that animals are typically infected as calves and while commonly not manifesting clinical disease for years, they shed MAP in their milk and feces in the interval

Predictors of Posttraumatic Stress Disorder, Anxiety Disorders, Depressive Disorders, and Any Mental Health Condition Among U.S. Soldiers and Marines, 2001-2011.

Understanding mental health disorder diagnosis and treatment seeking among active-duty military personnel is a topic with both clinical and policy implications. It has been well documented in military populations that individual-level military experience, including deployment history and combat exposure, influences mental health outcomes, but the impact of unit-level factors is less well understood. In the current study, we used administrative longitudinal data to examine a comprehensive set of unit- and individual-level predictors of posttraumatic stress disorder (PTSD), non-PTSD anxiety diso

Key components of a service model providing early childhood support for women attending opioid treatment clinics: an Australian state health service review.

AIMS AND OBJECTIVES: To report the findings of a service review--specifically the strategy to provide early childhood services 'on site' at opioid treatment clinics to address access difficulties. BACKGROUND: Child and family health nurses are skilled in the assessment and support of families during early childhood. However, women with a history of substance abuse are often cautious when engaging with universal and other health services, with the result that the infant may miss recommended developmental screening and early referral to improve health outcomes. DESIGN: In 2006, an internal rev
