How to set up a clinical research center in Brazil, as an example of a middle-income country.

In health care, innovation is a core part of the process that pushes advances forward. Drug and device development follow a step-by-step process from the discovery of a molecule to the final product. While patent filing and preclinical studies are usually performed by academic centers or start-ups, the clinical development is usually performed by pharmaceutical companies. To assess safety, efficacy and fulfil regulatory demands, clinical trials must be performed in sequential Phase I, II, and III stages prior to market access. In this context, clinical research centers have been established ar

Developing a core outcome set for patient-reported symptom monitoring to reduce hospital admissions for patients with heart failure.

AIMS: In patients with heart failure (HF), hospitalization rates are increasing, particularly for non-HF causes and over half may be avoidable. Self-monitoring of symptoms plays a key part in the early identification of deterioration. Our objective was to develop expert consensus for a core outcome set (COS) of symptoms to be monitored by patients, using validated single-item patient-reported outcome measures (PROMs), focused on the key priority of reducing admissions in HF. METHODS AND RESULTS: A rigorous COS development process incorporating systematic review, modified e-Delphi and nominal

The gut microbiome in colorectal anastomotic leakage: from mechanisms to precision.

Anastomotic leakage after curative colorectal cancer resection remains a frequent and severe complication that increases short-term mortality, worsens long-term oncologic outcomes, and places substantial burdens on individuals and health systems despite advances in surgical technique and perioperative care. Emerging evidence redefines anastomotic failure as not only a technical event but also a biologically driven process in which the gut microbiome regulates inflammation, epithelial repair, and barrier integrity at the healing interface. This review summarizes current data on the dual role of

Temporal dynamics of shame and guilt in adolescent NSSI: an ambulatory assessment study.

INTRODUCTION: Nonsuicidal self-injury (NSSI) is a common clinical concern among adolescents, yet the roles of shame and guilt as proximal drivers of NSSI in daily life remain unclear. Using an ambulatory assessment design, we examined how these self-conscious emotions relate to momentary NSSI urges and how they fluctuate before and after NSSI acts. METHODS: We recruited 25 adolescents (M = 15.9 years, 72% female, 8% male, 20% gender-diverse) who reported shame, guilt, and NSSI urges four times per day for seven consecutive days on their smartphones. NSSI acts were logged using event-contingen

Printed simulation models versus organ models in laparoscopic Roux-en-Y gastric bypass training: a prospective validation study of efficacy, learning curve, and cost-effectiveness.

BACKGROUND: Laparoscopic Roux-en-Y gastric bypass (LRYGB) is a complex bariatric surgical procedure that demands advanced technical skills. However, traditional training models (virtual simulators, cadavers, and porcine organs) are Limited by issues concerning realism, cost, and accessibility. Although 3D-printed models offer promising potential solutions, their application in LRYGB training remains underexplored. METHODS: 3D-printed silicone models of stomach and small intestine were fabricated using patient-derived anatomical data. The physical properties of these models (tensile strength,

Development of a modular patient-reported outcome and experience measure on patient needs and benefits in CLL (PBI-CLL).

BACKGROUND: Chronic lymphocytic leukaemia (CLL) is the most common form of leukaemia in adults in western countries. Asymptomatic patients are under clinical observation; when indication for treatment according to guidelines is met, treatment is initiated. When choosing from the numerous new treatment options, individual patient needs should be considered. To date, no instrument exists to capture these needs. METHODOLOGY: The ePROM was developed based on the Patient Benefit Index (PBI) methodology which captures the importance of treatment goals as well as the achievement of these goals. The

Effect of occupational therapy on the occurrence of delirium in critically ill patients: a systematic review and meta-analysis.

AIM: Delirium poses a major challenge to global health care, yet there is currently a dearth of single effective interventions or medications. Particularly, addressing delirium induced by critical illness is a complex process. Occupational therapy is considered to have a high potential for use in the prevention of delirium, as it involves both cognitive training and training in ADL. To comprehensively analyze the effect of occupational therapy on delirium prevention, we evaluated the effects of occupational therapy vs. standard non-pharmacological prevention on incidence and duration of deliri

Potential negative impact of reputed regulators' decisions on the approval status of new cancer drugs in Latin American countries: A descriptive analysis.

BACKGROUND: Many new cancer drugs are being approved by reputed regulatory authorities without evidence of overall survival benefit, quality of life improvement, and often based on clinical trials at high risk of bias. In recent years, most Latin American (LA) countries have reformed their marketing authorization (MA) rules to directly accept or abbreviate the approval process in case of earlier authorization by the European Medicines Agency (EMA) and the US Food and Drug Administration, mainly. This study assessed the potential impact of decisions taken by EMA regarding the approval of new ca

Health-care providers' perspectives on factors influencing return-to-work after surgery for nontraumatic conditions of the upper extremity.

STUDY DESIGN: This study is a descriptive survey. INTRODUCTION: Health care providers (HCPs) are key stakeholders who facilitate workers' return to work (RTW) following upper extremity surgery. Hand therapists play a major role in this process, yet we do not know if and/or how their perspectives differ from other HCPs. PURPOSE OF THE STUDY: This study examined HCPs' opinion on factors that influence RTW after surgery for nontraumatic upper extremity conditions and whether HCPs from different disciplines differed in their opinion. METHODS: HCPs (occupational therapists, physiotherapists, han

Resilience in patients with amputation because of Complex Regional Pain Syndrome type I.

PURPOSE: Although controversial, an amputation for longstanding and therapy-resistant Complex Regional Pain Syndrome Type I (CRPS-I) may improve quality of life and pain intensity. Resilience, the way people deal with adversity in a positive way may be related to these positive outcomes. This study focused on the relationship between resilience and post-amputation outcomes, i.e. quality of life, pain and recurrence of CRPS-I and psychological distress. METHOD: Twenty-six patients with an amputation related to CRPS-I filled in the Connor-Davidson Resilience Scale (CD-RISC), World Health Organi

Recognizing, managing, and preventing cognitive sequelae after stroke. A narrative review.

Post-stroke cognitive impairment (PSCI) affects up to one-third of stroke survivors and is a major contributor to long-term disability and reduced quality of life. Its heterogeneous phenotype reflects the interplay of acute cerebrovascular injury with chronic vascular pathology, inflammation, blood-brain barrier dysfunction, and, in many patients, coexisting neurodegeneration. Clinical manifestations vary according to lesion location and most commonly involve executive and attentional deficits, with variable impairment of memory, language, visuospatial function, and neuropsychiatric domains.
