Remission of irreversible aripiprazole-induced tardive dystonia with clozapine: a case report.

BACKGROUND: Aripiprazole can cause irreversible tardive dystonia in some individuals, and additional intervention is sometimes needed. Here, we report the first case of aripiprazole-induced irreversible tardive dystonia in which complete recovery of motor function was achieved using the antipsychotic drug clozapine. CASE PRESENTATION: A 24-year-old man with bipolar disorder was treated with aripiprazole and gradually developed tardive dystonia. Thorough medical and neurological examinations were performed to rule out other possible causes of tardive dystonia. Clozapine was administered when t

Prevalence and correlates of psychotic like experiences in a nationally representative community sample of adolescents in Ireland.

Adolescent psychotic like experiences (PLEs) are an important area of research, yet only a small number of community surveys have investigated their psychosocial correlates. This study presents the prevalence and correlates of three types of PLEs in a nationally representative community sample of 12-19 year olds in Ireland (N=5910). Correlates are considered across five domains: demographic, stressful life experiences, emotional/behavioral problems, substance use, and personal resources. Auditory hallucinations were reported by 13.7% of participants, 10.4% reported visual hallucinations and 13

Neuroanalysis: a method for brain-related neuroscientific diagnosis of mental disorders.

BACKGROUND: As an Ancient Chinese proverb says "The beginning of wisdom is to call things by their right names" thus we must start calling mental disorders by the names of their underlying brain disturbances. Without knowledge of the causes of mental disorders, their cures will remain elusive. METHODS: Neuroanalysis is a literature-based re-conceptualization of mental disorders as disturbances of brain organization. Psychosis and schizophrenia can be re-conceptualized as disturbances to connectivity and hierarchical dynamics in the brain; mood disorders can be re-conceptualized as disturbance

Cognitive impairment in patients with a schizoaffective disorder: a comparison with bipolar patients in euthymia.

OBJECTIVES: Several studies have shown persistent neurocognitive impairment in patients with a bipolar affective disorder (BD) even in euthymia as well as in patients with a schizoaffective disorder (SAD). The aim of our study was to compare the neuropsychological performance between these two groups. Confounding variables were controlled to enhance our understanding of cognitive dysfunction in both BD and SAD. METHODS: Several domains of neurocognitive function, executive function, memory, attention, concentration and perceptuomotor function were examined in 28 euthymic SAD patients and 32 B

Bullying victimization in youths and mental health problems: 'much ado about nothing'?

Bullying victimization is a topic of concern for youths, parents, school staff and mental health practitioners. Children and adolescents who are victimized by bullies show signs of distress and adjustment problems. However, it is not clear whether bullying is the source of these difficulties. This paper reviews empirical evidence to determine whether bullying victimization is a significant risk factor for psychopathology and should be the target of intervention and prevention strategies. Research indicates that being the victim of bullying (1) is not a random event and can be predicted by indi

Neuropsychiatric complications of medical and surgical therapies for Parkinson's disease.

This review deals with the range of neuropsychiatric problems that may arise from the use of medical and surgical therapies in the treatment of Parkinson's disease. As new approaches emerge, these problems are diversifying. Well-recognized drug-related complications include hallucinations and psychosis and the so-called dopamine-dysregulation syndrome. The etiology of these problems has not been fully established and is not clearly dose related, while the management can be difficult and needs to be tailored to the individual patient. Cholinergic and dopaminergic drugs may both influence cognit

What risk factors tell us about the causes of schizophrenia and related psychoses.

Genetic, epidemiologic, and molecular studies concur that liability to schizophrenia is transmitted through the inheritance of a number of genes of relatively small effect, some of which are shared with other psychoses. Each of these susceptibility genes causes minor deviations that are relatively innocent in themselves, for example, increased lateral ventricular volume, schizotypal personality, or subtle cognitive difficulties. However, when an individual is unlucky enough to inherit several of these traits, their cumulative effect, often compounded by environmental hazards, propels that pers

Is puerperal psychosis bipolar mood disorder?: a phenomenological comparison.

The most widely held current view on puerperal psychosis (PP) is that it is a mood disorder; some researchers have even suggested that it may be linked to bipolar mood disorder (BMD). To compare the phenomena of PP and BMD, 20 patients with PP were compared with 20 concurrent age-matched women with BMD, using the Structured Clinical Interview for DSM-III-R (SCID). To exclude patients with possible unipolar disorder, subjects with depression were not considered for inclusion in the study. A significantly greater proportion of the PP group experienced delusions of control, auditory hallucination

[Vocational promotion of psychiatrically handicapped patients in a Frankfurt am Main vocational training center].

In 1988, the Frankfort on Main Vocational Retraining Centre had decided in favour of increased, on-going admission of psychiatrically disabled adults, offering opportunities for their vocational education. The centre's objective is to enable psychiatrically disabled rehabilitees to successfully complete formal occupational training in as "normal" a setting as possible. This means: the Centre is a vocational rehabilitation facility, and is not directed at therapy or care. Psychiatrically disabled rehabilitees are accepted for up to 10 percent of available places in the various programme types.

Effects of dexamethasone on the availability of L-tryptophan and on the insulin and FFA concentrations in unipolar depressed patients.

Several authors have shown that the availability of L-tryptophan (L-TRP) in the serum is lower in patients with major depression than in controls. It has recently been reported that the administration of a dose of dexamethasone sufficient to cause cortisol suppression also caused significant decrements in the availability of L-TRP. In order to elucidate the putative pathophysiological mechanisms underlying this decreased L-TRP disposition, the authors examined 37 depressed women categorized according to the DSM-III. L-TRP, the sum of five competing amino acids (CAA), the L-TRP/CAA ratio, and i

Classifying depression by mental state signs.

The possibility that separation of a categorical depressive disease ('melancholia') from remaining depressive disorders can be improved by assessment of mental state signs was examined in patients treated by representative Sydney psychiatrists and patients referred to a specialised mood disorders unit. A set of signs, principally assessing retardation, was derived within the two samples by principal-components and latent-class analyses. Scores were significantly correlated with clinical, DSM-III, and RDC diagnoses, and appeared independent of severity, suggesting that melancholia can be define

Treating depression in schizophrenic patients.

Successful intervention in the depressive reactions of schizophrenic patients depends on accurate diagnosis, careful observation, the use of many modalities of treatment, and the maintenance of therapist optimism. Therapy needs to be pragmatic and focused on the special needs of the individual patient.

[Termination of pregnancy on psychiatric grounds (author's transl)].

303 psychiatric opinions were given between 1947 and 1969 regarding the termination of pregnancy. 45,2 per cent of the applications were supported, 53,4 per cent were rejected. In 10 per cent of the cases neurological disorders were present. 79 per cent of the women had psychiatric disorders among which reactive depressions had a definite majority of 89 per cent. Endogenous psychoses only represented a marginal group. Among the reactive depressive group young unmarried women in their first pregnancy with environmental conflicts and an increased readiness to express suicidal tendencies could be

Note on decision time/movement time relationships in normal and depressed subjects.

It has been suggested that under normal conditions, there is no relationship between decision and movement time components of response latency. However, for normal Ss, induced elevation of decision time produces conpensatory reductions in movement time. Psychomotor retardation, commonly observed in depressive patients, shows in the main, elevations for decision time. Such patients should also produce compensatory reductions in movement time. This was found not to be so. While 15 controls exhibited no relationship between decision and movement times, 30 depressive Ss exhibited a positive one. T

Dynamics and management of self-induced eruptions.

Self-induced eruptions are always expressions of an emotionally disturbed person. They cover a wide variety of injuries and include aggravation of preexisting dermatoses, neurotic excoriations, mucocutaneous changes from compulsive movements, factitial dermatitis and trichotillomania. The emotional disturbances in such patients range from mild psychoneuroses to severe psychoses. Attention should be focused primarily on the emotional disturbance. The management of psychotic patients is the domain of the psychiatrist. But those unsuitable for psychiatric care and persons with mild psychoneuroses

Polygenic predisposition to transdiagnostic symptom dimensions and treatment outcomes across psychiatric disorders.

BACKGROUND: Mood and psychotic disorders, including major depressive disorder (MDD), bipolar disorder (BD), and schizophrenia (SCZ), show overlapping symptoms that challenge diagnostic boundaries and may inform personalised treatments. We examined the contribution of genetic liability to transdiagnostic symptom dimensions in treatment outcomes across disorders using polygenic scores (PGSs). METHODS: We analysed two MDD cohorts (total N = 2548), one BD cohort (N = 755), and one SCZ cohort (N = 449). Outcomes included treatment resistance, symptomatic remission, and change in functioning define…
