Surgical ablation of atrial fibrillation during mitral-valve surgery.

BACKGROUND: Among patients undergoing mitral-valve surgery, 30 to 50% present with atrial fibrillation, which is associated with reduced survival and increased risk of stroke. Surgical ablation of atrial fibrillation has been widely adopted, but evidence regarding its safety and effectiveness is limited. METHODS: We randomly assigned 260 patients with persistent or long-standing persistent atrial fibrillation who required mitral-valve surgery to undergo either surgical ablation (ablation group) or no ablation (control group) during the mitral-valve operation. Patients in the ablation group un

Interatrial block and P-terminal force: a reflection of mitral stenosis severity on electrocardiography.

BACKGROUND AND AIM OF THE STUDY: Interatrial block (IAB), a frequently overlooked conduction delay between the atria, is defined as prolonged P-wave on the electrocardiogram (ECG). Previously, IAB has been shown to be an indicator of dilated and impaired left atrial function, and a predictor of both atrial fibrillation and embolic stroke. The study aim was to test the hypothesis that, on ECG, IAB with P-terminal force can reflect the echocardiographic severity of mitral stenosis (MS). METHODS: Patients with rheumatic MS who were referred to a single cardiac center for echocardiography between

Autologous right atrial wall patch for closure of atrial septal defects.

BACKGROUND: We used the right atrial free wall as a patch to close atrial septal defects (ASD) and report its results. METHODS: Between July 1998 and April 2006, 87 patients (mean age, 21.9 +/- 13.9 years; range, 7 months to 54 years), underwent closure of ASD with an autologous right atrial free wall patch. The underlying diagnosis were very large secundum ASD in 51 patients, sinus venosus defect in 15, primum ASD in 5, large defect resulting from excision of a left atrial myxoma in 12, complete atrioventricular canal defect in 1, total anomalous pulmonary venous return with ASD in 2, and Eb

Surgical revision after percutaneous mitral valve repair by edge-to-edge device: when the strategy fails in the highest risk surgical population.

OBJECTIVES: Percutaneous edge-to-edge devices for non-surgical repair of mitral valve regurgitation are under clinical evaluation in high-risk patients deemed not suitable for conventional surgery. To address guidelines for initial therapy decision, we here report on 13 cases of surgery after failed percutaneous edge-to-edge mitral valve repair or attempted repair, and discuss methodology and prognostic factors for operative outcome in this high-risk situation. METHODS: Thirteen patients referred to our cardiothoracic unit after failed percutaneous mitral valve repair or attempted repair usin

Shock--manifestation of pulmonary atresia with aorto-pulmonary collaterals and single ventricle physiology in a 2-day neonate.

BACKGROUND: The combination of pulmonary valve atresia and ventricular septal defect accounts for about 2% of cases of congenital heart disease. Most of the cases have intracardiac anatomy of Tetralogy Fallot and present with cyanosis in neonatal age. OBJECTIVES: To report a case of a newborn with rare combination of pulmonary atresia, ventricular septal defect and single ventricle physiology presenting with shock very early following birth. METHODS AND RESULTS: We describe a newborn infant who developed shock with severe metabolic acidosis and respiratory distress several hours following bi…

Percutaneous balloon valvuloplasty for treatment of congenital pulmonary valvular stenosis in children.

Percutaneous balloon pulmonary valvuloplasty was performed in seven children with moderate to severe valvular pulmonary stenosis (right ventricular to pulmonary artery pressure gradient greater than 50 mm Hg). All patients experienced a decrease in right ventricular peak systolic pressure from 108 +/- 30 to 60 +/- 5.6 mm Hg (p less than 0.001), decrease in right ventricular to pulmonary artery gradient from 90.1 +/- 30 to 38.8 +/- 5 mm Hg (p less than 0.001) and increase in pulmonary valve area from 0.33 +/- 0.06 to 0.55 +/- 0.15 cm2/m2 (p less than 0.001). In the two patients who underwent su…

[Anesthetic management for laparoscopic Nissen fundoplication in a 18 trisomy patient with congestive heart disease].

A 21-month-old patient diagnosed as 18 trisomy, having large ventricular septal defect with pulmonary valve stenosis, and gastroesophageal reflux disease, was admitted for laparoscopic Nissen fundoplication and gastrostomy. Anesthesia was induced with midazolam, fentanyl, and rocuronium, and maintained with sevoflurane and remifentanil. After induction of anesthesia, oxygenation became worse, possibly due to diminished pulmonary blood flow, which was partially resolved with vasopressor administration and fluid infusion. After the abdomen was insufflated, oxygenation improved significantly. We …

Raphal Cord Rupture: A Rare Mechanism of Aortic Regurgitation in Bicuspid Aortic Valve.

A 70-year-old male with chronic aortic regurgitation was referred with abrupt worsening heart failure. Late referral markers were pulmonary hypertension, mitral regurgitation, and tricuspid regurgitation. Evaluation revealed rupture of a raphal cord or fenestrated raphe from the conjoined cusp of a congenitally bicuspid aortic valve, a rare mechanism of aortic regurgitation. (Level of Difficulty: Intermediate.).

Quadrivalvular heart disease of mixed congenital and rheumatic aetiology with concomitant ostium secundum atrial septal defect and Ebstein's anomaly of the tricuspid valve.

We describe a 34-year-old man with an ostium secundum atrial septal defect, Ebstein's anomaly of the tricuspid valve with severe tricuspid regurgitation, congenital valvular pulmonary stenosis, rheumatic mitral stenosis and regurgitation with aortic regurgitation, who presented with decompensated heart failure after developing atrial fibrillation. The complex haemodynamic interplay of these lesions is also discussed.

A case of an infant with flail tricuspid valve due to spontaneous papillary muscle rupture: was neonatal lupus the culprit?

A 3-month-old infant presented in extremis with a flail tricuspid valve. The authors theorized that the genesis of her papillary muscle rupture was perinatal ischemia compounded by worsening pulmonary valvular stenosis leading to excessive fiber tension. Her underlying diagnosis of autoimmune-mediated heart block with endocardial fibroelastosis and prenatal glucocorticoid steroid treatment represents potentiating factors.

Double bioprosthetic valve replacement in right-sided carcinoid heart disease.

A patient with tricuspid and pulmonary regurgitation due to carcinoid syndrome successfully underwent double bioprosthetic valve replacement. This technique avoids anticoagulation treatment in a patient with hepatic dysfunction and facilitates future hepatic de-arterialization as a treatment option in carcinoid disease. Advances in treatment of carcinoid syndrome may have reduced the risk of early bioprosthetic degeneration.

Catheter treatment of congenital heart disease.

Over the last 10 years, several advances have been made in paediatric cardiology and cardiac surgery. However, the major developments have been in non-surgical attempts at transcatheter treatment of congenital heart disease. Initially these concerned some simple defects such as pulmonary valve stenosis but lately much more high-risk and complex defects have been treated.

Cor-triatriatum with atrial septal defect and bicuspid aortic valve diagnosed by cross-sectional echocardiography.

We report a rare case with a combination of cor-triatriatum, large secundum atrial septal defect and bicuspid aortic valve. The diagnosis was made by cross-sectional echocardiography. A defect was present between proximal (accessory) left atrial chamber and right atrium decompressing the accessory left atrial chamber. The large left to right shunt resulted in severe pulmonary arterial hypertension.

Rheumatic heart disease in the South West region of Cameroon: a hospital based echocardiographic study.

OBJECTIVE: Rheumatic heart disease (RHD) prevails as a major public health problem in sub-Saharan Africa. In Cameroon, reports on RHD have been so far limited to a few cities. We sought to describe the demographic, clinical and echocardiographic features of rheumatic heart disease in the Buea Regional Hospital, South West region of Cameroon. Echocardiography reports between June 2016 and June 2017 were reviewed. The diagnosis of RHD was based on the World Heart Federation Criteria for the diagnosis of RHD. RESULTS: A total of 669 echocardiograms were performed over the 1 year study period. Tw

Current management of calcific aortic stenosis.

Calcific aortic stenosis is a progressive disease with no effective medical therapy that ultimately requires aortic valve replacement (AVR) for severe valve obstruction. Echocardiography is the primary diagnostic approach to define valve anatomy, measure aortic stenosis severity, and evaluate the left ventricular response to chronic pressure overload. In asymptomatic patients, markers of disease progression include the degree of leaflet calcification, hemodynamic severity of stenosis, adverse left ventricular remodeling, reduced left ventricular longitudinal strain, myocardial fibrosis, and pu

Modified Fontan procedure for complex congenital heart disease.

The Fontan procedure is frequently used for correction of complex congenital heart disease, but the postoperative morbidity and mortality rates are still high. The results in 35 consecutive patients who underwent the Fontan procedure from January 1980 to June 1991 are reviewed. The patients comprised 26 boys and nine girls aged from 1.7 to 14.8 (mean 6.8) years. The underlying diseases were univentricular heart (19 patients), tricuspid atresia (eight), complete atrioventricular canal (three patients, two of whom had straddling atrioventricular valve) and other conditions (five). Tissue valves in the mitral position. Five years' experience.

Between April 1969 and November 1973 103 patients underwent isolated mitral replacement with three-cusp stented tissue valves. Autologous fascia lata was used in 50 patients, homologous fascia lata in 21, and heterologous pericardium in 32. The early mortality rate (14.6%) was influenced by age, the extent of preoperative cardiac disability, and low cardiac output. The survivors were followed up for periods varying from 8 to 60 months (average 37 months). In general, a factor in late death (13.6%) was high preoperative pulmonary artery pressure. In the autologous fascial series valve failure a

Surgical management of congenital heart defects: current trends.

Surgical treatment for congenital heart disease has become available over the last five decades. Palliative procedures have been designed to improve physiologic abnormalities, for example systemic artery (or venous) to pulmonary artery shunts of various types to increase the pulmonary blood flow, pulmonary artery constriction (banding) to decrease the pulmonary blood flow, and surgical or transcatheter atrial septostomy to augment intracardiac mixing. These can be performed with a low mortality. The majority of congenital heart defects can be corrected by open heart surgical techniques; some r
