Association of right ventricular dysfunction on electrocardiogram with outcomes and ventilatory response in patients monitored by electrical impedance tomography: A cohort study.

BACKGROUND: Mechanical ventilation is essential in critical care but can cause lung injury and hemodynamic compromise, particularly in patients with right ventricular dysfunction (RVD). Electrical impedance tomography (EIT) is increasingly used to guide ventilation, but its role in patients with RVD is not well defined. OBJECTIVES: To evaluate how electrocardiographic (ECG) signs of RVD influence the application and effects of EIT-guided ventilation management. METHODS: This retrospective cohort study (2013-2023) included mechanically ventilated patients who underwent both ECG and EIT. Patie

Electrical impedance tomography to set high pressure in time-controlled adaptive ventilation.

INTRODUCTION: TCAV (Time controlled adaptive ventilation), a combination of settings applied to the APRV (airway pressure release ventilation) mode, provides personalized ventilation tailored to the lung condition in ARDS (acute respiratory distress syndrome). The objective was to evaluate whether electrical impedance tomography (EIT) could serve as a tool for guiding Phigh level in TCAV for ARDS patients. METHODS: Eleven patients with moderate or severe ARDS were enrolled in a prospective single-center study in 2023. Patients were monitored with EIT (PulmoVista 500). Phigh trial was conducte

Mechanical ventilation after cardiac arrest: protecting the brain by protecting the lungs.

PURPOSE OF REVIEW: To review the current knowledge on mechanical ventilation after cardiac arrest, carefully balancing the protection of both the brain and the lungs. RECENT FINDINGS: Although lung-protective ventilation (LPV) strategies are often considered in the general population and widely studied in acute respiratory distress syndrome (ARDS) patients, current knowledge focused on patients after cardiac arrest is unclear. Mechanical ventilation in this unique population should prevent potential brain injury while also avoiding ventilation-induced lung injury. This includes optimizing ven

Continuous positive airway pressure (CPAP) for apnoea of prematurity.

BACKGROUND: Apnoea of prematurity (AoP) is defined as a pause in breathing for 20 seconds or longer, or for less than 20 seconds when accompanied by bradycardia and hypoxaemia, in a preterm infant. An association between the severity of apnoea and neurodevelopmental delay has been reported. Continuous positive airway pressure (CPAP) is a form of non-invasive ventilatory assistance that has been shown to be relatively safe and effective in preventing and treating respiratory distress among preterm infants. It is less clear whether CPAP treatment is safe and effective in the prevention and treat

Intraoperative protective mechanical ventilation in patients requiring emergency abdominal surgery: the multicentre prospective randomised IMPROVE-2 study protocol.

UNLABELLED: IntroductionEmergency abdominal surgery is associated with a high risk of postoperative complications. One of the most serious is postoperative respiratory failure (PRF), with reported rates up to 20%-30% and attributable 30-day mortality that can exceed 20%.Lung-protective ventilation, especially the use of low tidal volume, may help reducing the risk of lung injury. The role of positive end-expiratory pressure (PEEP) and recruitment manoeuvre (RM) remains however debated. We aim to evaluate whether a strategy aimed at increasing alveolar recruitment by using higher PEEP levels an

Ventilation management in Victorian intensive care unit patients without acute respiratory distress syndrome.

BACKGROUND: The setting of tidal volume (VT) during controlled mechanical ventilation (CMV) in critically ill patients without acute respiratory distress syndrome (ARDS) is likely important but currently unknown. We aimed to describe current CMV settings in intensive care units (ICUs) across Victoria. METHODS: We performed a multicentre, prospective, observational study. We collected clinical, ventilatory and arterial blood gas data twice daily for 7 days. We performed subgroup analysis by sex and assessment of arterial partial pressure of carbon dioxide (PaCO2) management where hypercapnia w

Mechanical ventilation strategies for intensive care unit patients without acute lung injury or acute respiratory distress syndrome: a systematic review and network meta-analysis.

BACKGROUND: It has been shown that the application of a lung-protective mechanical ventilation strategy can improve the prognosis of patients with acute lung injury (ALI) or acute respiratory distress syndrome (ARDS). However, the optimal mechanical ventilation strategy for intensive care unit (ICU) patients without ALI or ARDS is uncertain. Therefore, we performed a network meta-analysis to identify the optimal mechanical ventilation strategy for these patients. METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) in the Cochrane Library, EMBASE, MEDLINE, CINAHL,

Pressure-limited sustained inflation vs. gradual tidal inflations for resuscitation in preterm lambs.

Support of the mechanically complex preterm lung needs to facilitate aeration while avoiding ventilation heterogeneities: whether to achieve this gradually or quickly remains unclear. We compared the effect of gradual vs. constant tidal inflations and a pressure-limited sustained inflation (SI) at birth on gas exchange, lung mechanics, gravity-dependent lung volume distribution, and lung injury in 131-day gestation preterm lambs. Lambs were resuscitated with either 1) a 20-s, 40-cmH2O pressure-limited SI (PressSI), 2) a gradual increase in tidal volume (Vt) over 5-min from 3 ml/kg to 7 ml/kg (

Lung-protective ventilation strategies and adjunctive treatments for the emergency medicine patient with acute respiratory failure.

Respiratory failure is a frequent disease process encountered in the emergency department. There is significant need for improvement in the care of patients on mechanical ventilation. If not contraindicated, lung-protective ventilation strategies should be used. It is important to consider pathophysiology (shunting, dead space ventilation, and low venous admixture) when formulating treatment strategies in patients who are difficult to oxygenate or ventilate or when Pao2, Paco2, and pH can only be maintained at unsafe ventilator settings.

Effects of prolonged lung inflation or deflation on pulmonary stretch receptor discharge in the alligator (Alligator mississippiensis).

The American alligator (Alligator mississippiensis) is a semi-aquatic diving reptile that has a periodic breathing pattern. Previous work identified pulmonary stretch receptors, that are rapidly and slowly adapting, as well as intrapulmonary chemoreceptors (IPC), sensitive to CO2, that modulate breathing patterns in alligators. The purpose of the present study was to quantify the effects of prolonged lung inflation and deflation (simulated dives) on pulmonary stretch receptors (PSR) and/or IPC discharge characteristics. The effects of airway pressure (0-20 cm H2O), hypercapnia (7% CO2), and hy

Testosterone conversion blockade increases breathing stability in healthy men during NREM sleep.

STUDY OBJECTIVES: Gender differences in the prevalence of sleep apnea/hypopnea syndrome may be mediated via male sex hormones. Our objective was to determine the exact pathway for a testosterone-mediated increased propensity for central sleep apnea via blockade of the 5α-reductase pathway of testosterone conversion by finasteride. DESIGN: Randomization to oral finasteride vs. sham, single-center study. SETTING: Sleep research laboratory. PARTICIPANTS: Fourteen healthy young males without sleep apnea. INTERVENTION: Hypocapnia was induced via brief nasal noninvasive positive pressure ventila

Positive end expiratory pressure for preterm infants requiring conventional mechanical ventilation for respiratory distress syndrome or bronchopulmonary dysplasia.

BACKGROUND: Conventional mechanical ventilation (CMV) of neonates has been used as a treatment of respiratory failure for over 30 years. While CMV facilitates gas exchange, it may simultaneously damage the lung. Positive end expiratory pressure (PEEP) has received less attention than other ventilation parameters when considering this balance of benefit and possible harm. While an appropriate level of PEEP may exert substantial benefits in ventilation, both inappropriately low or high levels may lead to harm. An appropriate level of PEEP for neonates may also be best achieved by an individualiz

[Effects of different respiratory rates on PaCO2 and plasma potassium concentration].

OBJECTIVE: To observe the effects of different respiration rates on the CO2 partial pressure and plasma potassium concentration during general anesthesia. METHODS: Forty-two surgical patients under general anesthesia with normal preoperative arterial CO2 partial pressure and plasma potassium concentration were randomized into 2 groups. After intubation, all patients were placed on the mechanical ventilator (Ohmeda 210 anesthesia machine) with tidal volume 10 ml/kg and oxygen flow rate 1.5 L/min. Initial respiratory rate of group I patients was set to 12 times per minute for 30 minutes, and th…

[A case of re-expansion pulmonary edema after one lung ventilation for the radical operation of lung cancer].

A 64-yr-old man was admitted to our hospital for the examination of the abnormal shadow in his left lung field, which was diagnosed later as lung cancer. Radical operation was scheduled under combined epidural/general anesthesia. One lung ventilation was performed to facilitate the operative procedure for two hours and fifteen minutes. About two hours after the re-expansion of his left lung, he complained of dyspnea. He was hypoxic and moist rale was audible in his left lung. Cloudy shadow emerged in the left field of his chest X-P. Under bronchofiberoptic observation, excessive serous secreti

Derecruitment of the lung induced by stepwise lowering of positive end-expiratory pressure in patients with adult respiratory distress syndrome.

BACKGROUND AND OBJECTIVE: It has recently been suggested that recruitment proceeds far above the lower inflection point of the elastic pressure-volume (Pel/V) curve of the respiratory system. Accordingly, the value of the lower inflection point as a guide to set the positive end-expiratory pressure (PEEP) has been challenged. Our aim was to evaluate the derecruitment induced by stepwise decreasing PEEP levels. METHODS: Seven consecutive sedated and paralysed patients with acute respiratory distress syndrome were studied. Multiple Pel/V curves of the respiratory system were recorded at PEEP le

Negative pressure ventilation via chest cuirass to decrease ventilator-associated complications in infants with acute respiratory failure: a case series.

Pulmonary and nonpulmonary complications of invasive positive pressure ventilation are well documented in the medical literature. Many of these complications may be minimized by the use of noninvasive ventilation. During various periods of medical history, negative pressure ventilation, a form of noninvasive ventilation, has been used successfully. We report the use of negative pressure ventilation with a chest cuirass to avoid or decrease the complications of invasive positive pressure ventilation in three critically ill infants at two institutions. In each of these cases, chest cuirass venti

Passive body movement and gas exchange in the frilled lizard (Chlamydosaurus kingii) and goanna (Varanus gouldii).

The saccular lung in lizards is large and highly compliant compared with mammalian lungs, and these properties led us to question to what extent body movements could affect pulmonary gas exchange and the partial pressure of arterial blood gases. Specimens of two species of lizards, the frilled lizard (Chlamydosaurus kingii, approximately 600 g body mass) and the goanna (Varanus gouldii, approximately 1400 mass), were anaesthetised, maintained at approximately 36 degreesC and mechanically hyperventilated to lower the arterial partial pressure of carbon dioxide (PaCO2) to below apnoeic threshold

Obstructive sleep apnoea syndrome: is the "half-night polysomnography" an adequate method for evaluating sleep profile and respiratory events?

Recently, to reduce the costs of polysomnography, split-night studies have been introduced into routine practice: the first part of the night is used to make the diagnosis of obstructive sleep apnoea syndrome (OSAS) and the second part to achieve an appropriate level of continuous positive airway pressure. Since this split-night protocol has not yet been validated by the comparison of polysomnographic pictures obtained in the first and second parts of the night, the aim of this study was to evaluate sleep profile and respiratory disturbances in the first part (PSG1) and second (PSG2) portion o
