Optogenetic Therapy for Restoring Aspects of Visual Function.

BACKGROUND: Retinitis pigmentosa is an inherited degenerative retinal disease that can lead to irreversible blindness. Optogenetic therapy has shown potential for restoring visual function at late stages of the disease. METHODS: In this open-label study, we evaluated the safety of ganglion cell-directed optogenetic therapy in 10 participants with blindness due to advanced retinitis pigmentosa. Each participant received a single intravitreal injection of an adeno-associated viral (AAV) vector encoding the red-shifted channelrhodopsin ChrimsonR in the worse-seeing eye. The primary outcome was s

L-Threonic Acid Inhibits Pathological Retinal Neovascularization by Modulation of CCL3-CCR5 Signaling Pathway.

PURPOSE: Based on the integrated metabolomics data of serum and vitreous humor from diabetic retinopathy, this study aimed to elucidate the inhibitory effects and underlying mechanisms of L-threonic acid (L-ThA) on retinal neovascularization. METHODS: Cross-tissue metabolomic analysis identified consistently altered metabolites in both biofluids. Candidates negatively associated with proliferative diabetic retinopathy (PDR) progression were then selected through mass spectrometry analysis and disease risk assessment. To validate the inhibitory effects of these metabolites on retinal neovascul

A model of progressive photo-oxidative degeneration and inflammation in the pigmented C57BL/6J mouse retina.

Light-induced degeneration in rodent retinas is an established model for of retinal degeneration, including the roles of oxidative stress and neuroinflammatory activity. In these models, photoreceptor death is elicited via photo-oxidative stress, and is exacerbated by recruitment of subretinal macrophages and activation of immune pathways including complement propagation. Existing light damage models have relied heavily on albino rodents, and mostly using acute light stimuli. These albino models have proven valuable in uncovering the pathogenic mechanisms of such pathways in the context of ret

Emergencies in glaucoma: a review.

Most severe eye diseases and injuries ultimately require intervention by an ophthalmologist. The urgency of referral depends on various factors, including level of vision loss, duration of symptoms, and presence of comorbid diseases. Of special importance are five acute eye problems in which emergency management by primary care physicians can be critical to visual outcome: high-velocity injuries, chemical injuries, acute angle-closure glaucoma, arteritic ischemic optic neuropathy, and central retinal artery occlusion. This paper will focus on emergencies in glaucoma, especially primary and sec

Ocular clinicopathologic study of the mitochondrial encephalomyopathy overlap syndromes.

Recent advances in molecular genetics have led to a better understanding of mitochondrially inherited diseases. Mitochondrial encephalomyopathy overlap syndrome is one such group of diseases in which ocular abnormalities are frequently manifest. The authors describe the clinical, molecular genetic, and pathologic findings of two patients with the mitochondrial encephalomyopathy overlap syndrome. The patients shared a similar clinical course with features overlapping the three traditionally distinct clinical phenotypes (the Kearns-Sayre syndrome; the syndrome of mitochondrial encephalopathy, la

Survey of major blinding conditions in Qatar.

Qatar is one of the rapidly developing Gulf States. Its life style is rapidly changing from a seminomadic to a more settled urban one. This is the first study of the causes of economic blindness (6/60) in Qatar. It embraces 3,072 patients (70% males and 30% females). The most common causes in those under 40 years of age were trachoma, infectious diseases, retinal diseases, trauma and congenital malformations. After 40 years, cataract and glucoma occupy the second and third commonest causes after trachoma. Thus, a proper antitrachoma campaign will cut down the incidence of blidness by 50%. Cont…

Quantitative Markers of Neural Changes, Retinal Thickness, and Responses to Electrical Stimulation in Retinal Degeneration.

PURPOSE: Inherited retinal dystrophies demonstrate significant heterogeneity in the severity and distribution of photoreceptor loss. Current quantitative methodologies, however, do not account for this within-eye variation, which may explain conflicting findings regarding the efficacy of interventional strategies. DESIGN: We developed an eccentricity-agnostic measure of photoreceptor loss and determined if accounting for localized changes in retinal degeneration could explain the performance of an implanted retinal prosthesis. SUBJECTS: Hematoxylin and eosin stained, full thickness retinal c

Bilateral idiopathic retinal vasculitis following coxsackievirus A4 infection: a case report.

BACKGROUND: Coxsackieviruses are members of a group of viruses called the enteroviruses, which may cause respiratory and gastrointestinal symptoms, erythema, meningoencephalitis, myocarditis, pericarditis, and myositis. Unilateral acute idiopathic maculopathy caused by coxsackievirus A16 has been associated with hand, foot, and mouth disease, but only a few reports describe retinitis associated with coxsackievirus serotype B3 or B4. We report a case of bilateral multifocal obstructive retinal vasculitis that developed after coxsackievirus A4 infection. CASE PRESENTATION: A 60-year-old woman w

Herbimycin A inhibits angiogenic activity in endothelial cells and reduces neovascularization in a rat model of retinopathy of prematurity.

The pathogenesis of retinopathy of prematurity involves dysregulated angiogenesis resulting in pre-retinal growth of new vessels. Inhibition of tyrosine kinase-dependent pro-angiogenic signals may provide a rational therapeutic approach to the reduction of pre-retinal neovascularization. Vascular endothelial growth factor stimulates endothelial cell mitogenesis, differentiation and migration, by binding and activating the receptor tyrosine kinases vascular endothelial growth factor receptor-1 and vascular endothelial growth factor receptor-2. One of the vascular endothelial growth factor recep

Automatic Diagnosis of Plus Disease in Retinopathy of Prematurity Using Deep Learning.

OBJECTIVES: Plus disease, which can be diagnosed during clinical ophthalmoscopic examinations, is the most important feature in determining retinopathy of prematurity (ROP) requiring treatment. In the current study, we aimed to automatic diagnose and predict plus disease based on deep learning (DL) from retinal images of infants with ROP. METHODS: 600 retinal images from infants screened for ROP were evaluated. Each image was classified as normal, pre-plus and plus disease. After image pre-processing, the images were distributed into groups equal to the number of normal eye images and were us

Mesenchymal drift in ciliopathy iPSC-derived RPE reveals a convergent pathogenic cell state.

Ciliopathies comprise a spectrum of disorders involving mutations in over 150 genes affecting the primary cilium, with retinal degeneration as a prominent feature driven by concomitant developmental and maturation defects in photoreceptors and the retinal pigment epithelium (RPE). Current single-gene-targeted therapeutic approaches are expensive with limited scalability. We hypothesize that downstream of primary cilium dysfunction, mutation-agnostic shared pathways initiate tissue defects. To test this hypothesis, we here develop induced pluripotent stem cell-derived RPE models (iRPE) from nin

The role of microglia in glaucoma - trigger and potential target.

Glaucoma is a neurodegenerative disease characterized by the progressive loss of retinal ganglion cell and optic nerve damage. Recent studies have highlighted the pivotal role of microglia in the onset and progression of glaucoma. This review aims to elucidate the key mechanisms of microglial activation in glaucoma and assess its potential as a therapeutic target for novel treatment strategies. Microglia activation in glaucoma is multifactorial, driven by biomechanical, metabolic, and inflammatory signals. Activated microglia contribute to both neuroinflammatory injury and neuroprotective resp

Neuroprotection in glaucoma: Mechanisms beyond intraocular pressure lowering.

Glaucoma is a common, complex, multifactorial neurodegenerative disease characterized by progressive dysfunction and then loss of retinal ganglion cells, the output neurons of the retina. Glaucoma is the most common cause of irreversible blindness and affects ∼80 million people worldwide with many more undiagnosed. The major risk factors for glaucoma are genetics, age, and elevated intraocular pressure. Current strategies only target intraocular pressure management and do not directly target the neurodegenerative processes occurring at the level of the retinal ganglion cell. Despite strategies
