Ruptured duodenal peptic ulcer is a serious complication of ulcer disease that occurs in approximately 5% of cases and accounts for over 70% of deaths associated with peptic ulcer disease. This case report details the management of a 67-year-old male with a ruptured duodenal peptic ulcer who presented to the emergency room with acute onset of severe abdominal pain. The authors report a case of a ruptured breast implant presenting as an infraclavicular mass lesion. The findings of film-screen mammography, plain radiography, computed tomography and initial biopsy were nondiagnostic. Magnetic resonance imaging demonstrated the extent of the abnormality and permitted the correct diagnosis.

The functional status of the brain stem was assessed by recording evoked stem auditory potentials (ESAP) in 76 patients in the acute period of arterial aneurysm rupture. Abnormalities of ESAP were detected in 39.4% of the patients. The determinant of ESAP disorder in aneurysm rupture is the occlusive-dislocation syndrome caused by intracranial hematoma, more than 50 ml in volume, ventricular tamponade or extensive vascular spasm.

A case report of uterine rupture in labor with epidural anesthesia is presented. The woman had good analgesia on the left side, but complained of severe labor pais on her right side. Uterine rupture occurred which was manifested by sudden vaginal bleeding, fainting, low blood pressure and fetal distress. She did not feel any pains typical of uterine rupture. Rupture of the left uterine wall, with a large hematoma in the left parametrium was seen at surgery. It seems the unilateral anesthesia of the left side concealed the early signs of rupture.

Ruptured abdominal aortic aneurysm classically presents with abdominal pain radiating to the back, a pulsatile abdominal mass and circulatory collapse. However, other symptoms may be the only presenting complaint. We report the case of a patient who presented with a history of acute retention of urine relieved by catheterisation, but who then developed clinical features more typical of abdominal aortic aneurysm rupture.

The results of a study after 1 and 2 years of a prospective randomised trial of operative versus conservative treatment of ankle ligament rupture, demonstrate that purely functional orthotic therapy is the method of choice. This relates both to patient need and economical considerations. Traumatic spondylolisthesis of the axis is rarely associated with neurological deficit. The authors describe a case of complete tetraplegia due to spinal cord rupture at T1 without any other osteoligamentous lesion at the level. The mechanism of this lesion is discussed; it seems to be due to hyperflexion, with preceding hyperextension responsible for the C2 pedicular fracture.

A new technique is described for dealing with late-presenting spontaneous esophageal ruptures. This method requires only a short period of general anesthesia to drain the periesophageal abscess by a drainage tube inserted into the abscess cavity from the esophagus with the aid of a gastroscope and fluoroscopy. Gastric fluids are diverted from the esophageal rupture with a gastrostomy, and a jejunostomy is used for enteral feeding. The esophagus is retained, and closure of the fistula with resumption of normal swallowing is documented with serial sinograms.

In the period from 1980 to 1990 operations were carried out on 412 patients with echinococcosis of the liver. Complicated forms were encountered in 108 patients. Rupture of the hydatid cyst into the biliary tract occurred in 72 patients. Spontaneous enterocystoplasty rupture represents a devastating and potentially fatal late complication. Fourteen days after removal of an internal jugular catheter inserted prior to renal transplantation, a patient presented with brisk arterial hemorrhage from the insertion site. The hemorrhage, caused by a ruptured pseudoaneurysm of the costocervical trunk, was controlled by transcatheter embolization with gelatin particles. One year later the patient reported no problems related to the pseudoaneurysm or its management.

During the period January 1984-July 1988, 191 abdominal aortic aneurysms were encountered at Gosford District Hospital, NSW, a hospital that services an ageing population. These aneurysms were either repaired or found as the cause of death at post-mortem. During the study, the rate of elective repair rose from 0.25/month during the first 2 years to 3.67/month in the latter 2.5 years.
