Study of middle ear disease using tympanometry in general practice.

Tympanometry was used to provide evidence of middle ear effusions in a prospective study of middle ear disease in 264 children aged 3 months … he initial examination compared with African and West Indian children, as were those children whose siblings had a positive history of otitis media compared with those whose siblings had no such history. Children under 3 years were more likely to have evidence of an effusion than older children. Middle ear effusion as shown by tympanometry was not associat …

This strain of rat exhibits a heritable … slices corrected for background was expressed as pmol DHT bound/mg protein. Compared to normal male siblings, cytosol ABP levels on the ectopic side in unilateral ectopic rats were 78% and 93% lower than normal in epididymal heads and tails, respectively; whereas similar values for either side in bilateral ectopic rats were 88% and 99%. The above differenc …

The pattern of genetic transmission of the leukocyte defect in fatal granulomatous disease of childhood.

The leukocyte-phagocytic function test which was found to be abnormal in boys with fatal granulomatous disease of childhood has been found to be abnormal to an intermediate extent in their mothers. Nine of nine mothers were shown to be abnormal, whereas none of eight fathers and none of five healthy brothers exhibited a defect. 10 of 16 female siblings were abnormal to the same degree as their mothers, as were all three maternal grandmothers available for study. Assuming that this intermediate functional defect represents the heterozygous state, the nine family pedigrees are entirely compatibl…

Non-Rotavirus infection causing apnea in a neonate.

Apnea in a premature infant is not always due to immaturity and caffeine is not always the answer. We report a case of apnea in a preterm infant who presented at two weeks of life with increase in frequency of apnea that did not respond to caffeine. Family history was significant for diarrhea in a sibling. Stool PCR was positive for Norovirus Genogroup II. Enteric isolation was instituted and the apnea resolved spontaneously with conservative management. Re-emergence of apnea or persistent apnea necessitates further investigation to elucidate the etiology.

Bone marrow donation in childhood: one donor's perspective.

Allogeneic bone marrow transplantation is an established treatment modality for leukemia, hematologic disorders and inborn errors of metabolism. while the focus of attention is the transplant recipient, the bone marrow donor (often a sibling) is both physically and emotionally involved in the process. Few reports have been written from the donor's perspective, however. One of the authors of this report (G.P.) was a marrow donor as a child, and he reflects on this experience at the completion of his first year of training in medical school.

Spastic paresis in two littermate pups caused by Toxoplasma gondii.

The clinical and pathological findings in 2 sibling Bloodhound pups with spastic paresis of the pelvic limbs due to toxoplasmosis are described. Macropathology consisted of bilateral atrophy of the musculature of the affected limbs. Histopathology revealed meningo-encephalitis involving the cerebellum and medulla oblongata, diffuse meningomyelitis, radiculoneuritis of the lumbar spinal nerves and polymyositis and atrophy of skeletal muscle. Epizootological investigations failed to determine the source of the infection. Dissemination of the infection by a route other than the haematogenous path

[Neo-natal neutropenia induced by anti-NA 2 allo-immunization. Serological and clinical study of 2 cases in a family].

A severe neutropenia in a 2 day old newborn drew attention to anti-HLA and NA 2 antigranulocytic antibodies in the maternal serum. Close supervision from birth of a younger sibling demonstrated a neutropenia on the 5th day of life. Serological investigation showed the anti-NA 2 antibody to be responsible in both cases. A good result with total blood replacement of leucocyte-free blood in the second case would advocate this treatment in cases of severe immunological neutropenia.

The interrelationship between hypocomplementaemia, partial lipodystrophy and mesangiocapillary glomerulonephritis.

A further case of mesangiocapillary glomth reduced serum C3 levels is reported. The father of the patient was also found to have C3 deficiency. This lends support to the hypothesis that C3 deficiency may be the primary disorder relating these conditions and there is evidence in this case that the deficiency may be genetically determined. It is suggested that the complement state of close relatives, particularly younger siblings, of patients with these disorders should be investigated and, if abnormalities are found, they should be followed-up in order to elucidate the role of hypocomplementaem…

Pendular Nystagmus: a Novel Feature of ANO10-Related Disorders.

ANO10-related ataxia is characterized by cerebellar and pyramidal signs, with prominent oculomotor abnormalities, including saccadic abnormalities, strabismus, and ptosis. While nystagmus is frequently observed, the specific subtype of horizontal pendular nystagmus has not been emphasized. This report describes two siblings with early-onset spastic-ataxia with horizontal pendular nystagmus, ultimately diagnosed with SCAR10. This report details the ocular abnormalities spectrum of ANO10-related ataxia by highlighting horizontal pendular nystagmus as a significant clinical feature in early-onset…

The Many Lives of Staph: Staphylococcal Scalded Skin Syndrome in Two Brothers Secondary to Staphylococcus aureus-Associated Ecthyma Gangrenosum.

Staphylococcal scalded skin syndrome (SSSS) is a rare, infectious entity whereby the systemic distribution of Staphylococcal toxins results in the development of multiple symptoms, the most notable of which is a desquamative rash. Ecthyma gangrenosum, a skin infection most frequently occurring in immunocompromised individuals, is similarly uncommon. Herein, we present the case of two young siblings presenting with SSSS following the development of ecthyma gangrenosum. Prompt treatment of the Staphylococcal infection led to resolution of symptoms and recovery. Our case highlights the need to be…

A pseudo-homozygous missense variant and Alu-mediated exon 5 deletion in FARS2 causing spastic paraplegia 77.

FARS2-associated hereditary spastic paraplegia, later onset spastic paraplegia type 77, is a rarely neurodegenerative disease. Here, we reported two affected siblings in an autosomal recessive spastic paraplegia family with a pseudo-homozygous missense variant and Alu-mediated exon 5 deletion in FARS2. Both patients gradually developed altered gaits and weakness in both lower limbs. In our literature review, spastic paraplegia type 77 shows high heterogeneity in clinical manifestations. Our study broadens the scope of pathogenic mechanisms of SPG77 resulting from compound heterozygous mutation…

Identification of a Novel Mutation of Extracellular Matrix Protein 1 Gene in a Chinese Family with Lipoid Proteinosis.

Lipoid proteinosis (LP) is a rare autosomal recessive disorder caused by mutations in extracellular matrix protein 1 (ECM1), a glycoprotein expressed in skin. Whole-exome sequencing (WES) was used to investigate two Chinese siblings with suggestive clinical features of LP. They shared one known (c.960G>A) and one novel (c.1081G>T) pathogenic variant in ECM1 gene, inherited from their unaffected parents. The novel mutation (c.1081G>T) led to a termination codon at position 361 and caused nonsense-mediated mRNA decay and lost the function. Our finding expands the genetic etiology spectrum of LP.

Collagen VI-Related Myopathy Caused by Compound Heterozygous Mutations of COL6A3 in a Consanguineous Kurdish Family.

Collagen VI-related myopathies are caused by mutations of COL6A1, COL6A2, and COL6A3 and present with a wide phenotypic spectrum ranging from severe Ulrich congenital muscular dystrophy to mild Bethlem myopathy. Here, we report a consanguineous Kurdish family with 3 siblings affected by autosomal-recessive Bethlem myopathy caused by compound heterozygous mutations of COL6A3. We found the previously described missense mutation c.7447A > G/p.(Lys2483Glu) and a novel large deletion encompassing the exon 1-39 of the COL6A3 gene. Apart from the classical clinical symptoms, all patients had keratoco…

Delayed diagnosis of holocarboxylase synthetase deficiency in three patients with prominent skin findings.

Holocarboxylase deficiency (HLCSD) is caused by biallelic pathogenic variants in HLCS and is associated with poor feeding, emesis, lethargy, seizures, life-threatening metabolic acidosis, and hyperammonemia. Skin involvement in HLCSD is typically described as scaly, erythrodermic, seborrhea-like, or ichthyosiform, but there is a paucity of reports. We report three patients, including two siblings, with HLCSD and significant cutaneous manifestations including ichthyosiform dermatitis and a presentation with features of annular pustular psoriasis. In this report, we show that persistent, unexpla…

Familial writer's cramp: a clinical clue for inherited coenzyme Q10 deficiency.

The spectrum of coenzyme Q10 (CoQ10) deficiency syndromes comprises a variety of disorders, including a form of autosomal recessive cerebellar ataxia (ARCA2) caused by mutations in the AarF domain-containing kinase 3 gene (ADCK3). Due to the potential response to CoQ10 supplementation, a timely diagnosis is crucial. Herein, we describe two siblings with a novel homozygous ADCK3 variant and an unusual presentation consisting of isolated writer's cramp with adult-onset. Cerebellar ataxia developed later in the disease course and remained stable during the follow-up. This report highlights that A…

Fetal cerebral hemorrhage due to X-linked GATA1 gene mutation.

We report a multiplex family with a GATA1 gene mutation responsible for a massive fetal cerebral hemorrhage occurring at 36 weeks. Two other stillbirth cousins presented with fetal hydrops and congenital hemochromatosis' phenotype at 37 and 12 weeks of gestation. Molecular screening revealed the presence of a c.613G>A pathogenic allelic variation in exon 4 of GATA1 gene in the 3 male siblings and their carrier mothers. The diagnosis of a GATA1 gene mutation may be suspected in cases of male fetuses with intracerebral bleeding, particularly if a history of prior fetal loss(es) and mild maternal…

A newly distal hereditary motor neuropathy caused by a rare AIFM1 mutation.

In two siblings, who suffer from an early childhood-onset axonal polyneuropathy with exclusive involvement of motor fibers, the c.629T>C (p.F210S) mutation was identified in the X-linked AIFM1 gene, which encodes for the apoptosis-inducing factor (AIF). The mutation was predicted as deleterious, according to in silico analysis. A decreased expression of the AIF protein, altered cellular morphology, and a fragmented mitochondrial network were observed in the proband's fibroblasts. This new form of motor neuropathy expands the phenotypic spectrum of AIFM1 mutations and therefore, the AIFM1 gene …

TFG associated hereditary spastic paraplegia: an addition to the phenotypic spectrum.

Hereditary spastic paraplegias (HSPs) constitute movement disorders with extreme lower limb spasticity caused by axonopathies of the upper motor neurons. We describe two siblings affected with a recessive form of movement disorder. Whole-exome sequencing revealed a homozygous missense mutation c.64 C>T (p.Arg22Trp) in TFG as cause of the disorder. Comparison of the phenotype of the patients of this study, with that reported previously, revealed differences in the severity of the disorder as well as new clinical findings. These include presence of clonus, undeveloped speech, and sleep disturban…
