Determination of Normal Values of the Basal Angle in the Era of Magnetic Resonance Imaging.

BACKGROUND: The flexion of the skull base (basal angle [BA]) is the inclination between the anterior fossae and the basilar ramp of the occipital and sphenoid bones. An increased BA, termed platybasia, is usually associated with basilar invagination. BA reference values in the magnetic resonance imaging (MRI) era and the diagnosis of platybasia are of clinical importance. The transnasal approach has been the surgical technique of choice to remove the odontoid process in cases of ventral brainstem compression in patients with platybasia. The knowledge of normal BA values has been influenced by …

The clivus: anatomy, normal variants and imaging pathology.

The clivus is one of the most central parts of the skull base. As diseases of the clivus evade clinical evaluation imaging plays a pivotal role in establishing a diagnosis. This article combines the description of anatomy and pathology with an extensive pictorial essay. Starting with the development and normal macroscopic anatomy of the clivus the reader is then introduced to a large variety of normal variations and developmental disorders some of them with clinical significance. Typical examples of non-neoplastic and neoplastic masses of the clivus and their differential diagnoses are provide…

Extensive craniocervical pneumocranium.

The authors report the first case of extensive craniocervical pneumocranium causing mass effect, without any associated extradural pneumatocele. The patient presented with frontal headaches and unusual sensations in his left ear. He was found to have large pneumocranium involving his left frontal, temporal, parietal, and occipital bones as well as the atlas. He underwent a craniectomy with replacement of the pneumatized bone with titanium mesh, and mastoidectomy with obliteration of the mastoid air cells. This case is discussed in the context of the literature to date. A review of the literatu

Prehistoric trepanation in the Cuzco region of Peru: a view into an ancient Andean practice.

In this study, patterns of prehistoric trepanation in the southern highlands of Peru were examined through an analysis of 11 Cuzco-region burial sites. Trepanations were found in 66 individuals, with several individuals exhibiting more than one trepanation, for a total of 109 perforations observed. The predominant methods used were circular cutting and scraping-methods that proved highly successful with an overall 83% survival rate and little ensuing infection. Survival rates showed a significant increase over time, apparently reflecting improvements in trepanation technique through experiment

Cryptic osteoid osteoma of the cranium: case report.

Osteoid osteoma is an uncommon benign tumor of bone that rarely involves the cranium. If often causes local pain that is worse at night, characteristically relieved by aspirin, and does not correlate with the size of the lesion. The diagnosis depends on skull radiographs and computed tomography. This report describes a case in which radiographs showed nothing abnormal. The diagnosis and operative localization were accomplished with radionuclide bone scanning. Thus, the possibility of radiographically cryptic lesions must be considered in the evaluation of atypical head pain syndromes.

A micro-system for cranio-maxillofacial skeletal fixation. Preliminary report.

For application within special anatomical areas, such as the naso-ethmoidal area, the orbit and the frontal sinus wall and for infant cranio-facial surgery a Vitallium micro-plate-system has been developed, with screws of 0.8 mm diameter. The micro-plates, of 0.5 mm thickness, provide rigid fixation of very thin bony structures, and at the same time, minimal interference with the overlying soft tissues.

[Unusual case of depressed fracture of the posterior cranial fossa associated with the syndrome of acute central cervical spinal cord injury].

Linear fractures through the occipital bone are common, whereas depressed fractures in the posterior cranial fossa are rare because the occi … eyes and the ocular movements were normal in all directions except for horizontal nystagmus. Plain skull x-ray revealed a conspicuously depressed fracture in the posterior cranial fossa, but cervical spine x-ray showed neither fracture nor dislocation. Immediately suboccipital craniectomy was done and there were a thin subdural hematoma on the cerebell …

Neuroendoscopic Resection of Trigeminal Schwannoma in the Pterygopalatine/Infratemporal Fossa via the Transnasal Perpendicular Plate Palatine Bone or Transnasal Maxillary Sinus Approach.

BACKGROUND: Both the pterygopalatine fossa (PPF) and the infratemporal fossa (ITF) lie outside the midline of the skull base. Lesions in the PPF or ITF include trigeminal schwannoma (trigeminal schwannoma, TS), which originates from the second or third branch of the trigeminal nerve (maxillary nerve or mandibular nerve). Due to their typically deep anatomic location, lesions in the PPF or ITF can be difficult to treat using traditional surgical approaches. In recent years, because of their advantages, which include the fact that they allow the problem to be observed close up, neuroendoscopic t…

Outcome and complications of extended cranial-base resection requiring microvascular free-tissue transfer.

OBJECTIVES: To determine the complications of extensive cranial-base resection requiring free-tissue transfer (FTT) and the effect of these resections on local control and survival among patients with malignant neoplasms of the skull base. BACKGROUND: Before the advent of FTT, cranial-base surgery was often limited by our inability to adequately repair defects comprising communication between the central nervous system and upper aerodigestive tract. The use of FTT in cranial-base resections was therefore assessed to determine whether the improved procedure (ie, extensive resections) would imp…

Natural and artificial selection generate distinct cranial diversification pathways in bats and domestic dogs.

How far can evolution push a complex structure before hitting developmental limits, and when can it break free to explore new forms? Domestic dogs (Canis familiaris) represent the most extreme skull shape diversity generated within a single species through recent artificial selection, while neotropical leaf-nosed bats (Phyllostomidae) showcase the broadest craniofacial radiation in mammals, shaped over millions of years of natural selection. Because these contrasting systems belong to the same mammalian superorder, they provide an opportunity to test whether skull evolution is constrained by t…

Multimodal intraoperative monitoring of facial nerve function: combined use of blink reflex and corticobulbar motor evoked potentials.

OBJECTIVE: To compare the intraoperative blink reflex (BR) and facial corticobulbar motor evoked potentials (FCoMEPs) in skull base surgery, and to assess their respective and combined value in predicting postoperative facial nerve outcome. METHODS: 80 consecutive patients undergoing skull base surgery were monitored using ipsilateral FCoMEPs and bilateral BR recordings. BR was particularly assessed during stages in which FCoMEPs were limited by stimulus-induced movement. Facial nerve function was evaluated in the immediate postoperative period and at 6 months using the House-Brackmann (HB) s…

Early Versus Late Recurrence in Olfactory Neuroblastoma: A Multi-Institutional Analysis of Predictive Risk Factors.

BACKGROUND: Olfactory neuroblastoma (ONB) exhibits variable recurrence patterns, with significant late recurrences occurring years after tre … . When patients with late recurrence were compared to those who remained disease-free, radiographic skull base bone involvement was the strongest independent predictor (OR 2.90, 95% CI 1.14-8.64, p = 0.025), and this effect strengthened after adjusting for tumor volume (OR 3.80, p = 0.029). Pathologic dural invasion (OR 3.29, p = 0.013), Kadish stage (O …

A novel method for acoustic modeling of cranial bone based on the porosity index.

Transcranial ultrasound stimulation (TUS) has emerged as a clinically validated neuromodulation technique. Particularly, phased array ultrasound can be applied in TUS to focus on the cortex or deep brain non-invasively, such as the ventral intermediate thalamic nucleus (VIM) and Precuneus (PCu) region for the treatment of essential tremor (ET) and Alzheimer Disease (AD). Current TUS treatment planning relies on computed tomography (CT)-derived skull porosity measurements, which involve patient radiation exposure and potential registration errors. This study proposes a Porosity Index (PI)-based…

Investigations of the subarachnoid space as a potential link between aura and headache in migraine: A case-control MRI study.

BACKGROUND: The connection between migraine aura and headache is poorly understood. Some patients experience migraine aura without headache, … culated average distances between the occipital lobes, between the calcarine sulci, and between the skull and visual areas V1, V2 and V3a. We also measured volumes of corticospinal fluid between the occipital lobes, between the calcarine sulci, and overlying visual areas V2 and V3a. We investigated the relationship between headache status, distances and …
