Genome-wide linkage of obstructive sleep apnoea and high-density lipoprotein cholesterol in a Filipino family: bivariate linkage analysis of obstructive sleep apnoea.

Increasing evidence supports an association between obstructive sleep apnoea (OSA) and metabolic syndrome (MeS) in both children and adults, suggesting a genetic component. However, the genetic relationship between the diseases remains unclear. We performed a bivariate linkage scan on a single Filipino family with a high prevalence of OSA and MeS to explore the genetic pathways underlying these diseases. A large rural family (n = 50, 50% adults) underwent a 10-cM genome-wide scan. Fasting blood was used to measure insulin, triglycerides, total cholesterol and high density lipoprotein (HDL) cho…

Effects of mandibular advancement on upper airway dimension and collapsibility in patients with obstructive sleep apnea using dynamic upper airway imaging during sleep.

OBJECTIVES: The objective of this study was to evaluate the effects of mandibular advancement on oropharyngeal dimension and collapsibility and reveal the predominate site of change produced by mandibular advancement in patients with obstructive sleep apnea (OSA). STUDY DESIGN: Sixteen adults (13 males and 3 females) with symptomatic mild to severe OSA participated. Custom-made silicone mandibular positioners were used to keep the mandible at 67% of maximum advancement. Changes in the oropharyngeal size and collapsibility with mandibular advancement were evaluated using ultrafast computed tom…

Comparison of sleep variables between chronic widespread musculoskeletal pain, insomnia, periodic leg movements syndrome and control subjects in a clinical sleep medicine practice.

BACKGROUND: Between 50% and 89% of chronic pain patients report unrefreshing sleep. The aim of this retrospective analysis was to compare the sleep of normal subjects with the sleep of a clinical population presenting musculoskeletal chronic widespread pain (CWP), psychophysiological insomnia and restless legs syndrome/periodic limb movements during sleep (RLS/PLMS) in order to identify sleep variables that may explain the poor sleep complaints of CWP patients. METHODS: Sleep data from 10 normal subjects and 37 patients (mean age 55+/-3 yo), matched for age and sex, were retrieved from our sl…

The sleep-wake circuit plays a role in temporal distortions.

Normally individuals give a fairly accurate subjective estimate of short seconds-to-minutes time interval durations. However, in some circum … demonstrating such a neural circuit. Here we propose that, unexpectedly, actions of the well-known sleep-wake neural circuit are ultimately responsible for many of these known temporal distortions given two basic hypotheses about the underlying electromagnetic nature of the brain. We review this concept, including the theoretical existence of an imagin …

A crosstalk between the immune system and obstructive sleep apnea.

Obstructive sleep apnea (OSA) is characterized by recurrent upper airway obstruction during sleep. Its hallmark pathophysiological features, intermittent hypoxia (IH) and sleep fragmentation (SF), trigger cellular and molecular responses that disrupt immune homeostasis. Recurrent hypoxia-reoxygenation promotes oxidative stress and activates hypoxia-inducible factor-1α (HIF-1α), nuclear factor-κB (NF-κB), and the NLRP3 inflammasome, contributing to sustained inflammatory signaling, endothelial dysfunction, and alterations in circulating and tissue-resident immune cells. This review integrates e…

Sex differences in sleep regularity and its associations with mortality.

In characterizing sleep and circadian health, the day-to-day regularity of sleep-wake timing strongly predicts health outcomes, outperforming short sleep duration in prospective associations with mortality and new-onset disease (1, 2). It is unknown whether biological (e.g., sleep and circadian physiology) and sociocultural (e.g., exposures that affect sleep-wake timing) sex differences lead to differences in day-to-day sleep-wake regularity or modify its prospective associations with health outcomes. Here, we present findings from a UK Biobank study of 506,582 person-days of accelerometer rec…

Implication of the thalamus in sleep alterations observed in Alzheimer's disease.

Alzheimer's disease (AD), the most common neurodegenerative disorder, is frequently accompanied by sleep disturbances. Alterations of sleep, in both quality and quantity, contribute to the progression of AD neuropathology. The thalamus, through its diverse nuclei, plays a central role in sleep regulation and sleep-dependent memory consolidation, making it a relevant structure to investigate sleep disturbances associated with AD. Structural studies reveal that AD-related changes emerge early within the thalamus: Higher-order nuclei show atrophy, degeneration, and tau and Aβ accumulation. AD is …

Artificial intelligence for surgical education: The never-sleeping attending.

Artificial intelligence is rapidly emerging as a transformative tool in surgical education, with early studies demonstrating its potential to make training more efficient, equitable, and scalable. Anticipated applications include developing personalized learner profiles and employing predictive modeling to accelerate learning curves. The early success of artificial intelligence largely stems from its ability to analyze learner data objectively and at a scale beyond human capability. It also enables asynchronous and remote learning, thereby expanding access and availability. Furthermore, artifi

The Interaction between Sleep and Development on Wake EEG Oscillations.

The amount of time previously spent awake or asleep strongly impacts the sleep electroencephalogram (EEG), especially slow waves during nonrapid-eye-movement (NREM) sleep. These effects on the sleep EEG meaningfully interact with age and to a lesser extent developmental disorders such as attention-deficit hyperactivity disorder (ADHD). We aimed to determine whether EEG oscillations during wakefulness were likewise affected by the interaction of sleep and development, using data collected from 163 participants aged 3-25 years old (62 female). We analyzed age- and sleep-dependent changes in two …

Sleep Interventions and the Prevention of Pediatric Anxiety: A Systematic Review.

PURPOSE OF REVIEW: Anxiety disorders are prevalent in children, adolescents, and young adults. As many youth either do not receive adequate intervention or do not achieve remission, prevention of anxiety disorders is a public health priority. Sleep problems are a modifiable risk factor that precedes anxiety. This systematic review includes recent pediatric sleep interventions and anxiety outcomes, and introduces anxiety sensitivity as an understudied mechanism. RECENT FINDINGS: In 21 intervention and experimental studies, sleep intervention reduced anxiety symptoms/disorders and the onset of …

Sleep in Trans and Non-Binary Individuals: A Question of Gender?

Sleep problems are increasingly prevalent in society. One group that is disproportionately affected includes transgender and non-binary individuals, whose sex assigned at birth does not match their identified gender. This mismatch can lead to gender minority stress and increases the risk of mental disorders. However, research on sleep in this population remains extremely limited, especially when considering potential effects of gender-affirming care, including gender-affirming hormone therapy. Therefore, we conducted an anonymous, Germany-wide online study with 469 trans and non-binary individ…
