Pathology of spinal cord injuries due to fracture-dislocations of the thoracic and lumbar spine.

The pathology of intervertebral disc injuries, hyper-extension injuries and spinal cord injuries due to fracture-dislocations of the cervical spine has been reported previously. Between 1957 and 1987, autopsy was performed on 4 patients with fracture-dislocation of the thoracic spine and on one patient with that of the lumbar spine. This paper presents the results of a clinicopathologic study conducted on two patients with a fracture-dislocation of the thoracic spine and on one patient with that pathology of the lumbar spine. The methods employed in the present study are the same as those that…

High resolution GRASE MRI of the brain and spine: 512 and 1024 matrix imaging.

OBJECTIVE: Our goal was to implement 512 and 1024 matrix MRI of the brain in clinically acceptable imaging times. MATERIALS AND METHODS: With use of the GRASE (gradient-SE) imaging technique, 3 signals are refocused from each of 16 SEs, giving a total of 48 echoes per echo train, for a speed advantage of 1/48 over conventional SE imaging. Images in 1024 matrices are acquired in 2D Fourier transform (FT) multisectional MRI. In related experiments, multislab 3D FT GRASE imaging is performed using 16 partitions/slab, providing thinner 1 to 2 mm sections. In all imaging experiments, higher spatia

Placement of pedicle screws in the thoracic spine. Part II: An anatomical and radiographic assessment.

We used computerized tomographic scans and subsequent dissections to evaluate the position of ninety pedicle screws that had been inserted bilaterally into the fourth through twelfth thoracic vertebrae of five fresh-frozen cadavera. The screws had been inserted by five experienced spine surgeons without the use of radiographs or imaging studies. Of the ninety screws, thirty-seven were found to have penetrated the cortex of the pedicle. Twenty-one screws had penetrated the medial cortex and entered the spinal canal, and sixteen had penetrated the lateral cortex. The aorta and the esophagus were…

Surgical treatment of degenerative spondylolisthesis in the lumbar spine: no reposition without prior decompression.

In the present note, the cause of root compression in cases with degenerative lumbar spondylolisthesis is discussed in order to clarify the best method of surgery and to avoid iatrogenic damage of the cauda equina if a transpedicular fusion is going to be performed. The root compression is due to osteophytes growing out from the upper articular facets and from the upper ridge of the lamina of the lower vertebra. Therefore, the repositioning of the anteriorly displaced vertebra by the instrumentation could lead to the squeezing of the cauda equina between the withdrawn vertebral body and the de

Posterior arthrodesis and instrumentation in the immature (Risser-grade-0) spine in idiopathic scoliosis.

We performed a retrospective study of the long-term results of posterior instrumentation and arthrodesis of the spine in forty-three patients who had idiopathic scoliosis and a Risser grade of 0 at the time of the operation. The average age of the patients was 12.4 years (range, 6.7 to 15.5 years) at the time of the operation. The triradiate cartilages were open in twenty-three patients and closed in twenty. At the time of the latest follow-up evaluation (average duration of follow-up, four years; range, two to eleven years), seventeen patients had a Risser grade of 5; twenty-two, 4; two, 3; o…

[Significant injury of the neck including the cervical spine and soft tissue in suicidal strangulation].

A 46-year-old man committed suicide by jumping from a height of app. 4.5 m with a running noose round his neck. The neck was nearly cut through by the tightened ligature, so that there was only a small "bridge" of soft tissue between head and trunk.

Pedicular transvertebral screw fixation of the lumbosacral spine in spondylolisthesis. A new technique for stabilization.

Controversy exists about which method is most appropriate for treating moderate- to high-grade spondylolisthesis and symptomatic pseudarthro … f the lumbosacral junction. The technique is safe, effective, and employs instrumentation common to spine surgery. Its use in high-grade slips makes the technique more simple to perform than other methods of lumbosacral stabilization.

[Spinal stenosis and instability of the lumbar spine. Current aspects of pathophysiology and pain mechanisms].

The pathophysiology of spinal stenosis and segmental instability is discussed. The critical magnitude of the cross-sectional area of the cauda equina in the lower part of the vertebral canal is related to the dynamic loading of the spine, nerve impulse conduction and nerve root nutrition. Present knowledge of segmental instability is discussed, and an account is given of findings obtained during dynamic measurements with an intervertebral motion device (IMD), which indicate pathological motion patterns to differ from one pair of vertebrae to another.

[Hormonal disorders in women with prosopalgia in osteochondrosis of the cervical spine and the possible means for their correction].

Radioimmunoassay was employed to determine basal levels of LH, FSH, prolactin, total testosterone and 17 beta-estradiol in plasma from 52 pre- or postmenopausal women with prosopalgia consequent to cervical osteochondrosis. The measurements were made before and after laser treatment. Different from the control, basal levels of the hormones and the trends in their changes due to laser therapy depended on the patients' age and the disease stage.

[Postlaminectomy kyphosis of the cervical spine complicating spinal cord tumor in the foramen magnum].

Postlaminectomy kyphosis is rarely encountered as a sequela of decompression of the upper cervical spine, especially in adults. We present a case of disabling cervical kyphosis which developed after laminectomy for excision of a foramen magnum tumor and was treated successfully by occipito-vertebral fusion. A sixty-one-year old female was treated by laminectomy of the first through to the third cervical spine for a foramen magnum meningioma. One year later, the laminectomy was extended downward to include the fourth through to the seventh cervical spine because of neurological deterioration. S…

[The possibilities and outlook for the surgical treatment of complicated injuries and tumors of the spine by total vertebrectomy].

On the basis of experimental and clinical investigations the authors consider the removal of the vertebra body as the whole block to be the optimal variant. Shortening of the vertebral column is permissible for the approximation of ends of the spinal cord or its roots for suturing without intention. The use of vertebrectomy in 9 patients with long-standing dislocation fractures and defects of the spinal cord tissues from 1 to 12 cm and in 4 patient without tumors of the vertebrae has shown that the operation with removal of as many as 3 vertebrae is quite realizable technically. However, posit

[Experiences with the Steffee variable screw placement system in fractures of the thoracic and lumbar spine].

The variable screw placement (VSP) system was first used in orthopaedic surgery. In recent years, this rigid screw-plate system has been implanted with increasing frequency in patients with traumatic instabilities of the thoracolumbar spine. At the University Hospital in Graz, 20 patients have been operated on in the Department of Surgery using this system, and 15 of them have been followed up some months after the hardware removal. The results are compared with those achieved in patients whose instabilities were stabilized with other implants.

[Effects of inflammation-induced spinal fusion of the lumbar area on movement of the lumbar spine].

Lateral x-rays of the lumbar spine in standing position and functional flexion-extension roentgenograms of 21 patients with spontaneous fusion within the lumbar area after spondylitis were checked related to kyphosis of the block, lordosis and mobility of the lumbar spine and the single discs, and the appearance of degenerative spondylolisthesis. 76% demonstrated degenerative spondylolisthesis of the neighbouring discs, the half of them more than 5 mm with a significant higher number of segmental loosening of the disc caudal to the fusion. The kind of instability is a pseudo-spondylolisthesis …

Spondylodesis augmented by transpedicular fixation in the treatment of olisthetic and degenerative conditions of the lumbar spine.

Fifty-one patients having lumbar or lumbosacral posterolateral fusion with pedicle screw internal fixation were reviewed retrospectively two years after the fusion by an independent observer. In 44 patients, the underlying condition was lytic or degenerative spondylolisthesis. The internal device was removed one year after the fusion. The fusion rate was 94%. The subjective disability was assessed using the Oswestry disability index. The mean Oswestry score was 38% preoperatively, and 24% two years after the fusion. Preoperatively, only every fifth patient was able to work; two years postopera

Characterization of cultured cells derived from ossification of the posterior longitudinal ligament of the spine.

Ossification of the posterior longitudinal ligament of the spine (OPLL) is a common cause of spinal canal stenosis and myelopathy in Orientals. OPLL is characterized by heterotopic new bone formation in ligamentous tissue. To investigate the pathogenesis of OPLL, human posterior longitudinal ligament cells were cultured and their in vitro morphological and biochemical characteristics were studied. Cell cultures from control subjects with normal spinal ligaments did not show any osteoblastic properties. In contrast, cell lines (OG1-OG5) obtained from an OPLL patient showed several different phe…

Neurologic complications and lumbar laminectomy. A standardized approach to the multiply-operated lumbar spine.

Even the careful and knowledgeable spine surgeon will encounter a variety of neurologic complications during and after routine lumbar laminectomy. These include dural and nerve root injuries; cauda equina syndrome; and formation of scar tissue, extradural and intradural (arachnoiditis). The surgeon must be prepared to identify each of these problems and deal with them effectively at the time of the procedure and in the immediate postoperative and follow-up periods. The physician evaluating the multiply-operated lumbar spine patient must use an organized approach. The origin of the problem in m…
