The occurrence of epidermoid cysts in the oral cavity is extremely rare. These cysts generally present slow and progressive growth and often are not diagnosed until the second or third decade of life. Epidermoid and dermoid cysts in the floor of the mouth are painless, doughy or fluctuant lesions and cause no symptoms until they are large enough to interfere with speech or eating. This article presents the case of a 20-year-old man with swelling in the floor of his mouth that was causing difficulties with speech and swallowing. After surgical removal of the cyst, histopathologic examination co

Superior laryngeal neuralgia is a rare condition. We report a case of superior laryngeal neuralgia with deviated hyoid bone. A 50-year-old woman presented with paroxysmal and lancing pain localized to the area around the thyrohyoid membrane. Pain was induced and worsened by swallowing. A three-dimensional image showed deviation of the hyoid bone. General and neurological examinations were normal. After spraying of the larynx with 4% lidocaine, the pain improved. Superior laryngeal neuralgia was diagnosed, and the condition was treated with carbamazepine. Superior laryngeal neuralgia is rare bu

Amyotrophic lateral sclerosis is a progressive degenerative neuromuscular disease of insidious onset. It involves upper and lower motor neurons and causes both spastic and atrophic muscular symptoms. More than one fourth of patients have complaints relating to the head and neck (bulbar palsy); thus, the otolaryngologist may be the first physician to see them. In bulbar forms of Amyotrophic Lateral Sclerosis, voice and/or swallowing difficulties are often the initial signs of disturbance. Predominant symptoms are slurred speech, hoarseness, dysphagia, and dyspnea. Muscular weakness, atrophy, an

A 5-year-old asymptomatic boy was shown to have bowel loops in the thoracic cavity incidentally in a chest radiograph. A barium swallow confirmed the diagnosis of Morgagni hernia. Laparoscopic repair under CO2 pneumoperitoneum was performed. Anesthesia was induced and maintained with air, oxygen and sevoflurane. After pulling the transverse colon and the greater omentum into the abdomen, it was found that a part of the liver was also herniated into the right sternocostal hiatus (Larry hernia). The patient showed uneventful recovery. However, we should realize that dissection of adhesions betwe

A 3-year-old girl with severe dysphagia secondary to caustic stricture of the cervical esophagus (about 4 cm in length) underwent esophagoplasty by a new technique following failed attempts at esophagoscopy and dilatation. This procedure results in a neoesophagus in the region with a lumen approximately twice its previous diameter, which will further increase in size gradually after a number of dilatations. The postoperative course was uneventful. She had almost no dysphagia following four successive dilatations 1, 2, 3 and 5 months after surgery. Barium swallow revealed an acceptable lumen. S

Thiopental, propofol, and diazepam-ketamine were compared for evaluation of laryngeal function in dogs. There was no significant difference among the three protocols in time to observation of normal function after drug administration or in the occurrence of swallowing, laryngospasm, or breathing. Jaw tone was significantly greater with diazepam-ketamine. Exposure of the larynx was excellent in five dogs and moderate in three dogs, each receiving thiopental or propofol. Exposure was excellent in one dog, moderate in six dogs, and poor in one dog receiving diazepam-ketamine. Exposure of the lary

When a patient presents with persistently unilateral head or face pain, cluster headache and trigeminal neuralgia should be considered. Diagnosis is based on the patient's history; anatomical studies are performed only to rule out problems other than tumor or stroke. A patient who presents with pain in the pharynx, tonsils, and ear--particularly if it is initiated by swallowing, yawning, or eating--may have glossopharyngeal neuralgia. Treatment with carbamazepine is indicated; if the patient does not respond to this drug, the diagnosis is doubtful. Several effective treatments are available fo

We report the use of a cuffed oropharyngeal airway (Copa), in a patient with an acute respiratory failure from a cardiogenic pulmonary oedema, for continuous positive pressure ventilation. Considering the ease of use and the lack of laryngeal stimulation, this device can be considered for mechanical ventilation in selected cases with acute cardiac failure. There are two contra-indications: prolonged mechanical ventilation, because of the lack of airway protection from gastro-oesophageal reflux, and normal consciousness, as the patient cannot swallow. This device can be considered when starting

The surgical technique, indications, and results of the infrahyoid muscle flap are presented. This flap is fed by the superior thyroid vessels and innervated by the ansa cervicalis. The flap is indicated in case of medium-sized defects in the floor of the mouth, the tongue, the buccal mucosa, and the lateral pharyngeal wall. The advantages of the technique presented include the rapid flap elevation close to the original operating field, the ability to use the motor capability of the flap, and the ability to combine it with other local flaps--for example, the platysma flap. This technique appea

This study is based on the videofluorographic exploration of deglutition in 14 patients who were treated by supracricoid laryngectomy. The choice of this population rests on two criteria: a 1-year postoperative delay, and absence of residual deglutition disorders elicited by patient history. Asymptomatic aspiration was seen in 6 cases. In the cricohyoidoepiglottopexies (CHEP), aspiration occurred uniquely in patients who did not recuperate satisfactorily from epiglottic dynamics. The deglutition sequelae are less invalidating relative to the cricohyoidopexies (CHP), with a possible recuperatio

The histologically benign hypopharyngeal lipoma is a potentially fatal tumour because of the risk of upper airway obstruction. It may be asymptomatic or present with symptoms ranging from vague foreign-body sensation to sore throat, dysphagia or dysphonia. The diagnosis may be suggested by indirect or fibreoptic laryngoscopy. Lateral neck soft tissue X-ray and barium swallow may help but CT imaging of the pharynx enables a more precise preoperative diagnosis. Treatment is by surgical excision of the lesion either perorally, endoscopically or via a lateral pharyngotomy. Long-term follow-up is r

Tumour or congenital lesions are less common, whether intrinsic (stenosis, fistulas, duplications), or extrinsic (vascular malformations, mediastinal tumours). Radiological investigatio

The symptom dysphagia is defined and an interdisciplinary team approach emphasized. A step like diagnostic approach in oropharyngeal dysphagia is described, including transnasal pharyngo-laryngo-fiber endoscopy, videofluoroscopy and the "modified barium swallow." First, all of the organic changes causing oropharyngeal dysphagia must be recognized, both "typical ENT diseases" and malignant tumors of the oropharynx and hypopharynx, which are usually diagnosed only at an advanced stage. Of particular interest is postoperative dysphagia with or without aspiration. If there is no evidence of an org

Five horses with squamous cell carcinoma of the stomach are described. The clinical findings included anorexia, weight loss, abdominal distension, abnormal chewing and swallowing behaviour and abdominal masses palpable per rectum in four cases. Haematological studies revealed a normocytic anaemia in three horses, and neutrophilia and hypoalbuminaemia in two. Analysis of peritoneal fluid revealed abnormal effusions in all five horses and neoplastic cells were identified in three of them. The tumours originated in the cardia and metastases were present in all the horses. Three of them had many s

Malignant fibrous histiocytoma has been the most common soft-tissue sarcoma of late adult life. However, it is relatively uncommon in the head and neck. Furthermore, there has been no report of malignant fibrous histiocytoma of the hypopharynx in the literature. The first case of malignant fibrous histiocytoma of the hypopharynx is presented. The patient, a 70-year-old male, underwent extended laryngectomy and elective neck dissection with primary tracheoesophageal shunt operation and pharyngeal myotomy for voice restoration following total laryngectomy. For more than 2 years, there has been n

In general practice, a material of 202 patients seeking advice on account of difficulty in swallowing were included consecutively in an investigation with the object of assessing the value of a rapid test, Respiralex, for the diagnosis of group A streptococci as compared with culture. The true positive and negative diagnostic frequencies were 0.89 and 0.94, respectively. Absence of agreement was found most frequently when culture rendered poor growth. It is concluded that Respiralex is such a reliable method of examination in the diagnosis of group A streptococci, that it is recommended for us

Extracorporeal membrane oxygenation (ECMO) for survivors of surgical repair of congenital diaphragmatic hernia was used in 14 neonates. All showed marked esophageal dilatation on postoperative chest radiographs; the dilated esophagus simulated an air- or fluid-filled mass. All patients were later shown to have marked gastroesophageal reflux as well. The finding suggests a problem in swallowing exists before birth, which may explain the recent correlation between diaphragmatic hernia and hydramnios. The observation of a mediastinal mass in neonates with congenital diaphragmatic hernia may repre

A case is reported of massive drug self-poisoning (more than 200 pills associated with slices of oranges) in which repeated gastric lavage failed to empty the stomach. An attempt to split up the amalgamated pills, adherent to the gastric mucosa, by fibre-optic gastroscopy failed and a gastrotomy was required. Endoscopy together with gastric lavage is indicated in certain situations to quickly empty the stomach and so shorten the course of the poisoning. Indications are the large number of pills swallowed, a poor result of the treatment, a worsening of the clinical condition despite treatment,

Reconstruction of extended post-irradiation pharyngo-esophagostomy is still a formidable challenge in head and neck surgery. Early rehabilitation of speech, swallowing and chewing and prevention of severe hemorrhage of the carotid artery by soft tissue coverage is mandatory. If previous attempts with local lipocutaneous and myocutaneous flaps have failed, a two stage reconstruction by microsurgical transfer of a jejunal patch in combination with a myocutaneous latissimus dorsi flap will lead to sufficient lining and a permanent closure of the defect. The indication and the technical procedures

Two cases are reported of patients in the third trimester of pregnancy in whom routine ultrasound examination revealed progressive dilatation of the fetal intestines. Intestinal obstruction was suspected and amniocentesis as well as amniography with Urografin were done. In both cases a regression in dilatation of bowel was noted on ultrasonography. The Urografin swallowed by the fetus was deemed to have relieved the obstruction of the fetal intestine. The babies were delivered in good condition and passed large amounts of watery meconium per rectum. In spite of the risks inherent in the use of
