[Multimodal diagnostic of CSNB1 with NYX gene mutation].

This article presents the case of a young male patient with complete congenital stationary night blindness (CSNB1). The informative value of the general medical history and clinical findings for the diagnosis was impaired due to language barriers and low compliance. Full-field electroretinography and optical coherence tomography help to define particular hereditary retinal dystrophies. Molecular genetic analysis by next generation sequencing as a part of multimodal diagnostics finally uncovered a rare, causal missense mutation in the nyctalopin (NYX) gene.

Bilateral giant macular schisis in a case of Goldmann-Favre syndrome.

We describe a case of a 23-year-old man with giant macular schisis which can be seen in Goldmann-Favre syndrome. Associated history of decreased night vision and an enhanced S cone electroretinogram helps in confirming the diagnosis. Swept source optical coherence tomography of the same helps in confirming the schisis and delineates the extent and level of schisis. Other differential diagnosis of bilateral macular schisis in a young man could be juvenile X-linked retinoschisis.

Spontaneous improvement of macular traction retinal detachment associated with myopic macular schisis.

High myopia can be associated with a range of pathologic changes within the macula that are now easily appreciated with optical coherence tomography. In the setting of high myopia and a macular traction retinal detachment, the expectation is for progressive worsening over time, and surgical intervention is often undertaken early. The authors present a case of spontaneous improvement of myopic macular detachment, which illustrates the potential value of an initial period of observation in this clinical setting.

Abnormality in the external limiting membrane in early Stargardt disease.

Stargardt disease (STGD1) is caused by mutations in the ABCA4 gene. It has previously been reported that abnormalities in STGD1 may be detectable in the photoreceptors using spectral domain-optical coherence tomography (SD-OCT) prior to the detection of retinal pigment epithelium abnormalities. We present a 5-year-old asymptomatic girl with normal appearing fundi who possessed pathogenic ABCA4 variants on both chromosomes and where thickening of the external limiting membrane was the only abnormality detected on SD-OCT.

[Diagnostic Approaches to the Cornea (Part 2) - From Conventional Techniques to Advanced Imaging].

The cornea plays a pivotal role in visual quality, accounting for most of the eye's refractive power. It is also frequently affected by a wide range of pathological conditions. Accurate diagnostics are therefore fundamental for making therapeutic decisions. This review provides an overview of established and emerging methods for examining the cornea, ranging from corneal pachymetry, assessments of the endothelium and nerves, anterior segment optical coherence tomography, and biomechanical measurements. Advances in high-resolution imaging and multimodal platforms have significantly improved the

[Research progress on the structure and innervation of extraocular muscles].

In addition to the abnormalities of the extraocular muscles themselves and the nervous system innervating the extraocular muscles, the tissue pulley abnormalities around the extraocular muscles may lead to the occurrence of strabismus. In recent years, researchers have shown the intramuscular nerve distribution of the extraocular muscles using the Sihler technique. With the continuous progress of imaging technology, magnetic resonance imaging, computed tomography, anterior segment optical coherence tomography and ultrasound biomicroscopy can be used to observe the attachment sites of the extra

Ocular toxocariasis.

Ocular toxocariasis is an uncommon worldwide parasitic infection that affects mostly children and is found in both rural and metropolitan areas. In many parts of the world, parasitic infections of the eye are a major cause of blindness. The diagnosis of toxocariasis is essentially clinical, based on the lesion morphology and supportive laboratory data such as serum enzyme-linked immunosorbent assay (ELISA) titers and ELISA Toxocara titers on aqueous humor; other diagnostic methods are imaging studies including optical coherence tomography, fluorescein angiography, computed tomography, and ocul

Congenital iris ectropion associated with juvenile glaucoma.

Congenital iris ectropion is an uncommon malformation and no reports exist about the use of modern technologies in this pathology. The authors describe a case of unilateral and isolated congenital iris ectropion associated with juvenile glaucoma in a healthy and completely asymptomatic 6-year-old girl with an unusual form of anisocoria. Optical coherence tomography and confocal scanning laser tomography showed a progressive glaucomatous neuropathy. A trabeculectomy without antimetabolites was performed and intraocular pressure normalized without other medications during a follow-up of 2 years.

Multimodal Imaging in a Case of Idiopathic Neuroretinitis.

A 37-year-old woman presented with painful visual loss in the left eye for 2 weeks. The best-corrected visual acuity was 20/200. Ophthalmic examination of the left eye revealed vitreous cells, optic disc swelling, serous retinal detachment, and macular star-figure hard exudates. Swept-source optical coherence tomography showed both inner and outer retinas were swollen, the choroid was thickened, and the corresponding retinal pigment epithelium was elevated. Wide-field indocyanine green angiography disclosed multiple hypofluorescent spots in the mid-periphery. Taken together, an involvement of

Long-range frequency-domain optical delay line based on a spinning tilted mirror for low-cost ocular biometry.

Optical biometers are routinely used to measure intraocular distances in ophthalmic applications such as cataract surgery planning or myopia monitoring. However, due to their high cost and reduced transportability, access to them for screening and surgical planning is still limited in low-resource and remote settings. To increase patients' access to optical biometry we propose a novel low-cost frequency-domain optical delay line (FD-ODL) based on an inexpensive stepper motor spinning a tilted mirror, for integration into a time-domain (TD)-biometer, amenable to a compact footprint. In the prop

Interpreting ocular age estimates: source, output, evidence, and clinical meaning.

Ocular age estimates derived from fundus photographs, optical coherence tomography, ocular fluids, systemic omics, and multimodal models are increasingly reported in years. The eye is unusually tractable for organ-specific aging research because it is paired, repeatedly imageable, locally treatable, and linked to quantitative function. The familiar scale can obscure differences in what was measured, what the model was developed to predict, which quantity is reported, and what the study design can support. Existing frameworks classify visual-aging biomarkers by biological domain and context of

How to diagnose papilloedema.

Papilloedema is optic disc swelling due to raised intracranial pressure (ICP). Typically, this occurs bilaterally, and visual function is usually relatively preserved unless the disease is moderately severe or chronic. The implications of a new finding of papilloedema are potentially serious; however, overdiagnosis of papilloedema can lead to iatrogenic harm from unnecessary investigation or treatment. It is important to consider the differential diagnosis for swollen optic disc appearances to ensure patients are investigated appropriately. We describe an approach to the clinical assessment of

High-resolution visible light OCT of the human retina with combined superluminescent diodes.

High-resolution optical coherence tomography (OCT) requires broadband, spatially coherent light sources. Today, the source of choice for visible light OCT, the supercontinuum (SC), is bulky, expensive, and prone to excess noise. Here we demonstrate high-resolution visible light OCT of the human retina with a combined superluminescent diode (SLD) source. The source is longer in wavelength than the high blue light hazard range but shorter in wavelength than the high photopic efficiency range, ensuring subject safety and comfort. We report an axial resolution of 3.2 µm in the retina. We find that

En face OCT: Breakthroughs in understanding the pathoanatomy of retinal disease and clinical applications.

En face optical coherence tomography (OCT) is a practical and informative imaging modality to noninvasively visualize distinct retinal and choroidal layers by providing coronal images using boundary-specific segmentation. Ongoing research with this method is generating breakthroughs in the illustration of new perspectives of retinal disease. The clinical value of en face OCT as an advanced retinal imaging tool is growing steadily and it has unveiled many new insights into the pathoanatomy of retinal disorders. Moreover, this modality can capture various en face OCT biomarkers that correspond t

Peripapillary intrachoroidal cavitation.

Peripapillary Intrachoroidal Cavitation (PICC) can be appeared as an orangish lesion located at the outer lower edge of the myopic cone and confined to the intrachoroidal space. It is more common in patients with high myopia, older age, and greater axial length. The most accepted pathophysiological mechanism involves traction over a vulnerable sclera tissue at the myopic cone. PICC may present with visual field defects like mild glaucomatous neuropathy. Differential diagnosis with other choroidal pathologies is essential, and OCT-HD shows distinctive features in PICC. Additionally, OCT-A plays

Impact of the Valsalva manoeuvre on the choroid: A systematic review with meta-analyses.

A variety of daily activities can intentionally or unintentionally cause the Valsalva manoeuvre, which induces a physiological response of elevated peripheral venous pressure. Studies have speculated that it may ultimately affect the choroidal anatomy. This is particularly important from a clinical point-of-view since patients occasionally hold their breath while undergoing macular optical coherence tomography (OCT). In this study, we systematically reviewed the literature to understand the impact of the Valsalva manoeuvre on the choroid and conducted meta-analyses on the changes induced in th
