Bazex's acrokeratosis paraneoplastica is a clinically well-defined entity consisting of a distal erythemato-squamous eruption and nail dystrophy. It is always associated with neoplasia, mainly squamous carcinomas of the upper digestive and respiratory tracts; therefore, Bazek's acrokeratosis is considered to be a specific marker of internal malignancy. We report a case of renal cell carcinoma occurring with bladder and contralateral adrenal metastasis discovered preoperatively with computerized tomography. Surgical treatment consisted of radical nephrectomy, with preservation of the ipsilateral adrenal gland, contralateral adrenalectomy, and transvesical resection of the bladder neoplasm. The hypothesis for the mechanism(s) of spread and treatment modalities are discussed.

This review covers recent observations (all retrospective) on useful prognostic indicators in patients treated by radiation for this heterogeneous disease, including presenting clinical characteristics, extent of conservative surgery, radiographic studies, and pathologic subtypes. Although imperfect and incomplete, this review offers some criteria to identify which patients will do well and which quite poorly if treated by bladder-preserving full-dose radiation therapy. Potentials and limitations of a new, facultative non-tomographic Fast-Imaging sequence "RARE-MR-Urography" are presented. Its results in urological diseases are discussed on the basis of a pilot study of 35 patients. The images (4 s-32 s acquisition time) are extremely T2-weighted and display unbounded free water (urine). They reveal valuable information, not available with ultrasound or conventional radiology without application of contrast agents in the presence of urine retention. S/N problems reduce the accuracy of this method if the urinary tract contains only small amounts of urine.

A 75-year-old woman with transitional cell carcinoma of the renal pelvis was revealed to have sarcoid granulomas within the kidney as well as in the renal pedicle lymph nodes. Furthermore, noncaseating granulomas had been found in the pelvic lymph nodes in a histological study following previous total cystectomy for bladder cancer. The association of sarcoidosis with genitourinary malignancies is briefly discussed.

Urothelial tumours of the upper urinary tract are rare, but their incidence increases in subjects suffering from phenacetin nephropathy, Balkan nephropathy and infected pyelic calculi. In order to act on the urothelium through the urinary way, a cancerigenic substance must (1) be eliminated with the urine; (2) be absorbed by the urothelium; (3) remain in the tissue for a certain length of time. Urinary hydroxyproline has been measured in 150 patients, 70 of them with urinary diseases. High values were found in chronic hyperazotaemia and in carcinoma with bone metastasis. Gray scale transrectal ultrasonography has proved to be a valuable adjunct in the evaluation of patients with tumors of the prostate and bladder. Its application in patients with prostatic disease is in the detection of early asymptomatic tumors, in the accurate staging of local disease and in the following of patients after the institution of specific treatment. Although it is not being used for the detection of bladder tumors the procedure is a reliable means to evaluate the degree of tumor invasion so that proper therapy can be instituted.

For patients with recurrent prostate cancer after initial external beam radiation salvage cryotherapy is considered as an alternative to salvage prostatectomy. We report a serious complication of salvage cryotherapy in a 72-year-old man suffering from a severe recto-vesico-cutaneous fistula 6 weeks after salvage cryotherapy. To manage this situation salvage cystoprostatectomy and continent urinary diversion with creation of an ileocaecal pouch with cutaneous stoma had to be performed.
