The pharmacological treatment of Menière's disease.

The management of Menière's disease often provides a formidable clinical challenge largely because its precise aetiology is unknown. There is no known 'cure' once the condition is established, but drugs may be helpful in treating both the acute attacks of vertigo, and in more long-term management, particularly in the earlier fluctuant stage. These remedies are largely symptomatic and there are few if any properly controlled studies of their efficacy. Suppressant drugs may act centrally at neurotransmitter sites, or peripherally on the labyrinth. Conventional diuretics and osmotically acting ag

["Cerebro-active" drugs].

The prevention and treatment of age related mental diseases, including cerebrovascular dementia and Alzheimer's disease, are complicated by diagnostic problems and by the lack of specificity of the so-called "cerebroactive" drugs. These agents are frequently prescribed at the patient's or the relatives' request, without clear-cut evidence for their therapeutic efficacy. The economic consequences of these usually long term treatments should also be taken into consideration. A few "cerebroactive" agents may marginally slow down or modulate the progression of the various neuropathological process

N,O-diacylated-N-hydroxyarylsulfonamides: nitroxyl precursors with potent smooth muscle relaxant properties.

N,O-Diacylated-N-hydroxyarylsulfonamides are capable of slowly releasing nitroxyl (HNO) by simple, non-enzymatic hydrolysis in Krebs solution at 37 degrees C. Release of nitric oxide (NO) was not seen. These compounds were also found to elicit vasorelaxation in rabbit thoracic aorta in vitro, presumably as a result of their ability to release HNO. This effect was enhanced by the addition of superoxide dismutase (SOD). Thus, these results are consistent with previous work indicating that HNO is a potent vasorelaxant.

Cutaneous responses to topical methyl nicotinate in human forearm and vulvar skin.

In order to identify and define differences in percutaneous absorption and microcirculatory sensitivity between forearm and vulvar skin we studied the response of human forearm and vulvar (labium majus) skin to topical methyl nicotinate (MN) in 11 healthy premenopausal volunteers. MN-induced erythema was assessed by laser Doppler velocimetry (LDV). The following parameters were compared: 1) basal cutaneous blood flow, 2) the time to peak response, 3) the magnitude of LDV peak response, 4) the area under the LDV response-time curve and 5) the decay time to 75% of peak response. Basal cutaneous

[Drug therapy of angiolopathies].

The treatment of angiolopathies is rather difficult, at this time, at least concerning functional angiopathies. It is a fact that protection against cold represents, in all angilopathies, one of the main elements of the treatment besides medications which are essentially vasoactive drugs, myorelaxants or alpha-blockers, and possibly venous tonics. Fluorescein or Sodium fluoresceinate at 5%, administered in slow intra-venous injections, represents one of the best treatment of acrocyanosis, in combination with vitamins A and D, given at the beginning of fall. Other angiolopathies, especially tho

Intracoronary adenosine enhances myocardial reactive hyperemia after brief coronary occlusion.

Adenosine is a prime candidate for the role of mediator between myocardial metabolic state and coronary blood flow. However, there are few reports concerning the direct effects of exogenously added adenosine on coronary autoregulation. The present investigation in the open-chest dog studied the effects of a threshold dose of intracoronary adenosine infusion on reactive hyperemia following brief coronary occlusions. The infused dose did not increase nonocclusive flow by greater than 10%. Adenosine enhanced total hyperemic flow at all occlusions tested (5, 10, 15, 20, and 30 s). Aminophylline pr

Severe electrocardiographic abnormalities during arfonad administration.

Severe and reversible electrocardiographic abnormalities (first degree atrioventricular block, left bundle branch block, ventricular fibrillation), were induced by the administration of Arfonad in a patient with type III acute aortic dissection previously chronically treated with alpha-methyl-dopa. Any other possible cause of the electrocardiographic changes was excluded on the basis of clinical findings and laboratory studies. The explanation of the electrocardiographic abnormalities induced by Arfonad are not readily apparent, also on careful review of the literature. We suggest a strict ele

Adenosine causes dilatation and constriction of hypothalamic blood vessels.

The effect of adenosine on intraparenchymal cerebral blood flow was examined in conscious rabbits with the 133Xe clearance technique. Perivascular application of 10(-3) and 10(-4) M adenosine to hypothalamic blood vessels increased hypothalamic blood flow by approximately 50% (p less than 0.005). This vasodilatation was attenuated by the intrahypothalamic injection of the beta-adrenergic receptor antagonist propranolol, but was unaffected by alpha-adrenoreceptor blockade with phenoxybenzamine, or depression of neuronal activity with barbiturate. 2-Chloroadenosine, a stable analogue of adenosin

[Histopathologic study of perioral dermatitis].

38 biopsies of perioral dermatitis have been examined. The following has consistently been observed: follicular hyperkeratosis; vasodilatation and oedema (sometimes very pronounced) of the papillary dermis; perivascular and parafollicular infiltrates consisting predominantly of lymphocytes, histiocytes and polymorphonuclear leucocytes. Occasionally, there have been epithelioid or giant cells, sarcoid-like infiltrates or vasculitis of the leucoclastic type. Demodex folliculorum was observed in only two sections and the follicles that contained them were not inflamed. This histological picture i

Secretory, motor and vascular effects in the sublingual gland of the rat caused by autonomic nerve stimulation.

The influence of the autonomic nerves on sublingual glands of rats was studied. Stimulation of the chorda-lingual nerve evoked a lively flow of saliva and was also thought to contract the myoepithelial cells in the gland. Sympathetic nerve stimulation, on the other hand, usually evoked no secretion and did not cause any motor responses in the sublingual gland. The glandular blood flow was increased by chorda-lingual nerve stimulation, and this vasodilatation persisted also when atropine had been administered. Sympathetic nerve stimulation decreased the sublingual blood flow; this vasoconstrict
