Symptomatic treatment of brain tumor patients with sodium selenite, oxygen, and other supportive measures.

Patients (16 women and 16 men) with brain tumors previously treated conservatively by surgery, radiation, and/or chemotherapy with typical s … improvement of the general condition and a decrease in symptoms, such as nausea, emesis, headache, vertigo, unsteady gait, speech disorders, and Jacksonian seizures, were observed. In all treated patients, improvements of erythrocyte, hemoglobin, and thrombocyte counts were observed. Additional beneficial effects were noted in the patients receiving the …

On the role of the olivocochlear bundle in hearing: a case study.

A young patient with normal pure-tone thresholds in both ears underwent a unilateral vestibular neurotomy in January 1992 to relieve severe vertigo ascribed to Ménière's disease. Evidence is provided that the whole vestibular nerve including the olivocochlear bundle (OCB) was sectioned. Just prior to the surgery, the patient was examined in several psychoacoustic tests involving mainly signal detection and selective attention. Over the next 20 months, he was reexamined in those same tests. The patient's ability to detect expected tones in the quiet (including audiograms) or in noise was the sa…

[A case of intracranial hypoglossal neurinoma with no preoperative hypoglossal nerve palsy].

A case of intracranial hypoglossal neurinoma without hypoglossal nerve palsy is reported. A 43-year-old housewife was admitted to our hospital with vertigo and left occipital headache. Neurologically, no cranial nerve deficits were present. CT scan and cerebral angiography showed a mass in the lower left posterior fossa. MRI also revealed a well circumscribed extra-axial mass compressing brain stem and cerebellum to the right. Left suboccipital craniotomy was performed and the tumor was removed subtotally. From the operative findings, the 8th to 11th cranial nerves were not related to the tumo…

[Surgical treatment of otosclerosis. Effects and risks].

Surgical treatment of otosclerosis involves an opening of the labyrinth and, accordingly, a risk of complications from that organ: vertigo and sensorineural hearing loss or anacusis. During the past decade, the number of new patients with otosclerosis has stabilized at a low level in Danish ENT-departments. Thus, typically the number of stapedectomies per year per department is between 10 and 25. The study elucidates effects and risks at the operation, focusing in particular on the question whether this treatment ought to be centralized. In a seven-year period, a total of 75 stapedectomies in …

Generalized argyrosis in man: neurotological, ultrastructural and X-ray microanalytical findings.

Generalized argyrosis can produce a number of abnormalities, including skin discoloration, liver and kidney dysfunction. We describe a patient with generalized argyrosis following long-term self-treatment with oral silver intake, in whom skin discoloration, progressive taste and smell disorders, vertigo and hypesthesia were observed. These findings were confirmed by chemosensory tests and electrophysiological investigations. The development of hypogeusia was assessed by subjective tests, while the progression of hyposmia was followed by recording olfactory evoked cortical potentials. Light and…

[Diagnosis and course of tuberculosis especially from the viewpoint of clinically unknown deaths].

Every time the tuberculosis is present and it is to be included in the differentialdiagnosis if the occasion arrises. In the anamnesis it is necessary to pay attention to specific diseases and the risk groups like patients with "21-day-cough", silicotics, "Contrast-articularis bronchitics", diabetics, so-called "persons with fibrotic lesions" and patients with frequent influenzal infections. The symptoms unclear gastric distress, want of appetite, indifferent loss in weight, uneasiness, slight vertigo and fast tiredness already give further references. Breath-pain, haemoptysis and subfebrile t…

A case of cysticercosis cerebri. Neurotologic implications.

Cysticercosis is an unusual disease entity in the United States. A case is reviewed in which a young Mexican-American female presented to an otologist's office with a headache and vertigo. Her symptoms were a manifestation of a single larval cysticercus cyst which had become lodged in the fourth ventricle resulting in an obstructive hydrocephalus and a fatal course. The epidemiology of this disease traces the life cycle of the pork tapeworm from man, normally the definitive host, to the pig, the intermediate host. It is noteworthy that the disease cysticercosis in humans is a deviation from th…

[The acute vestibular paralysis (author's transl)].

Acute vestibular paralysis may not be considered as a nosologic entity but as a syndrome. Symptomatology (vertigo, spontaneous and provoked vestibular nystagmus, absence of cochlear signs) shows an uniform picture. The results of the caloric test as well as the nystagmic responses induced by galvanic stimulation and the development of central vestibular compensation however indicate that the site of the lesion is not only confined to the labyrinth but may also occur at the level of the peripheral neuron or even the vestibular nuclei. Etiology and pathology are still vague. Our own clinical obs…

Labyrinthine hemorrhage: practical lessons from a rare cause of acute vestibular syndrome.

Labyrinthine hemorrhage (LH) is a rare cause of sudden sensorineural hearing loss (SNHL) and acute vestibular syndrome (AVS), which may mimic both peripheral and central etiologies. We report a case of a 76-year-old male on edoxaban therapy presenting with sudden right-sided SNHL and vertigo. Brain magnetic resonance imaging (MRI) performed 2 weeks later showed T1 and fluid-attenuated inversion recovery (FLAIR) hyperintensity of the right inner ear, consistent with LH. Evaluation of AVS in the emergency setting remains challenging, as LH may present as a rare stroke mimic, often indistinguisha…
