BACKGROUND: Randomized clinical trials are blinded to prevent knowledge of treatment assignment from influencing outcomes and their assessments, thus protecting the trial's scientific integrity. Trials involving a warfarin treatment arm are difficult to blind due to the need to continuously adjust dose.
PURPOSE: We sought to examine the effectiveness of blinding secondary stroke prevention trials with a warfarin treatment arm in which the blinding system incorporates use of placebo warfarin dose modification schedules for patients in the placebo warfarin arm.

In a placebo-controlled double-blind comparative study conducted in 19 healthy volunteers, naftifin hydrochlorid cream was tested against clotrimazole 1%, clotrimazole 1%+hydrocortisone 1%, and various hydrocortisone preparations (0.5%; 1%; 2.5%) for erythema-suppressive effects. The anti-inflammatory effect of naftifine hydrochloride demonstrated earlier was confirmed in this study. The results prove a potency comparable to that of weak topical glucocorticosteroids.

We conducted a randomized, double-blind, placebo-controlled trial of two forms of topical interferon therapy for condyloma acuminata in women. Gel containing 10(6) IU of leukocyte interferon/g, with or without nonoxynol-9, was compared with treatment with gel base alone. One percent metronidazole in an emollient cream base was compared with 250 mg oral tetracycline taken twice daily for the treatment of seventy-five patients with rosacea. After eight weeks of treatment there was no statistically significant difference between the results of the two treatments. Tetracycline did have a more rapid onset of effect on papules and pustules. Both treatments were well tolerated.

A topical carbonic anhydrase inhibitor, 6-amino-2-benzothiazolesulfonamide (aminozolamide), which is an analogue of ethoxzolamide, was studied in 18 patients with ocular hypertension. Significant lowering of intraocular pressure was achieved with a single 50-microL gel application compared with the untreated control eye. The onset of action occurred within two hours and lasted at least eight hours. No systemic side effects were elicited. Topical carbonic anhydrase inhibitors may be an important alternative in the medical management of glaucoma.

A modified double blind crossover study was performed to assess the effect of 1% topical minoxidil as compared with placebo in 30 patients with alopecia areata and alopecia totalis. The active preparation produced a highly significant incidence of hair regrowth. A cosmetically acceptable response was noted in 16 patients. No side effects were seen. The study confirmed that topical minoxidil will induce new hair growth in alopecia areata but that it is less likely to do so in more severe and extensive disease. An open trial of local clotrimazole therapy in 56 pregnant women with vulvovaginal candidiasis is reported. The diagnosis was confirmed by mycotic culture. The patients were given one vaginal tablet daily and cream was applied to the vulva 2 or 3 times daily. Their male partners were treated with cream only. The duration of therapy was 6 days. In a randomized double-blind controlled trial of the value of intermittent topical idoxuridine in treating herpes zoster in 118 patients idoxuridine 5% in 100% dimethyl sulphoxide (DMSO), applied four-hourly for four days, significantly shortened the vesicular phase, healing time, and duration of pain; idoxuridine 25% applied two-hourly produced no greater benefit. The only side effects were transient tender erythema in three patients and "urticarial" oedema in two patients with dermographia.

A field trial was carried out with a vaccine containing 300x10(9) killed S. typhi (strain Ty58) per tablet. A total of 12 564 children aged 1-15 years were given 3 tablets of the vaccine or a placebo at the rate of one tablet on each of 3 consecutive days.
