Quinidine for pharmacological cardioversion of atrial fibrillation: a retrospective analysis in 501 consecutive patients.

BACKGROUND: Although quinidine has been used to terminate atrial fibrillation (AFib) for a long time, it has been recently classified to be used as a third-line-drug for cardioversion. However, these recommendations are based on a few small studies, and there are no data available of a larger modern patient population undergoing pharmacological cardioversion of AFib. Therefore, we evaluated the safety of quinidine for cardioversion of paroxysmal AFib in patients after cardiac surgery and coronary intervention. METHODS: In 501 consecutive patients (66 +/- 9 years, 32% women), 200-400 mg of qui

Clinical predictors of termination and clinical outcome of catheter ablation for persistent atrial fibrillation.

OBJECTIVES: This study evaluated the role of pre-procedural clinical variables to predict procedural and clinical outcomes of catheter ablation in patients with long-lasting persistent atrial fibrillation (AF). BACKGROUND: Catheter ablation of persistent AF remains a challenging task. METHODS: Catheter ablation was performed in 90 patients (76 men, age 57 +/- 11 years) with long-lasting persistent AF. The history of AF, echocardiographic parameters, presence of structural heart disease, and surface electrocardiogram (ECG) AF cycle length (CL) were assessed before ablation and analyzed with r

Use of vitamins C and E as a prophylactic therapy to prevent postoperative atrial fibrillation.

Oxidative stress has been strongly involved in the underlying mechanism of atrial fibrillation, particularly in the arrhythmia occurring in patients undergoing cardiac surgery with extracorporeal circulation (postoperative atrial fibrillation). The ischemia/reperfusion injury thus occurring in the myocardial tissue contributes to the development of tissue remodeling, thought to be responsible for the functional heart impairment. Consequently, structural changes due to the cardiac tissue biomolecules attack by reactive oxygen and/or nitrogen species could account for functional changes in ion c

Transseptal catheterization in the catheter ablation of atrial fibrillation in a patient with cor triatriatum sinister.

A 66-year-old man with cor triatriatum sinister underwent pulmonary vein (PV) isolation (PVI) for atrial fibrillation (AF) twice because of AF recurrence. Different transseptal approaches into the anterior chamber receiving the left atrial appendage and posterior chamber receiving the PVs, were achieved in two sessions. PVI of the left PVs and right superior PV was challenging via the anterior chamber, whereas PVI of the right inferior PV was challenging via the posterior chamber because of the disturbance of the membrane. Therefore, an intentional transseptal catheterization into the more app

Influence of electrical cardioversion on inflammation and indexes of structural remodeling, in persistent atrial fibrillation.

BACKGROUND: Electrical cardioversion (EC) remains the most effective treatment for the restoration of sinus rhythm in patients with atrial fibrillation (AF). During the development of AF, structural remodeling (besides electrical remodeling) and inflammation could play a pivotal role in perpetuation and recurrence of the arrhythmia after EC. The aim of this study was to evaluate the role of inflammation [as indicated by C-reactive protein (CRP)] and matrix degradation [matrix metalloproteinase type I, MMP-1, its tissue inhibitor, TIMP-1 and carboxy-terminal telopeptide of collagen type I, ICTP

Efficacy of pulmonary vein isolation in paroxysmal atrial fibrillation patients with a Brugada electrocardiogram.

BACKGROUND: Medical therapy of atrial fibrillation (AF) can be challenging in patients with Brugada electrocardiograms (ECGs). The purpose of this study was to investigate the efficacy of pulmonary vein (PV) isolation (PVI) in AF patients with Brugada ECGs. METHODS AND RESULTS: PVI was performed in 6 consecutive patients exhibiting Brugada ECGs (type I in 1, type II in 4, and type III in 1) at baseline. In all patients exhibiting type II or III Brugada ECGs but 1, the administration of sodium-channel blockers converted those ECG patterns to a type I. Five of 6 (83%) patients were free of symp

Clinical profile and outcomes of atrial fibrillation in elderly patients with acute myocardial infarction.

BACKGROUND: Atrial fibrillation (AF) occurs commonly in patients with acute myocardial infarction (MI) and is associated with an increased long-term mortality. This study aimed to investigate the clinical characteristics and outcomes of AF in in-hospital elderly Chinese patients with acute MI. METHODS: A total of 967 patients with acute MI, aged >/= 65 years, were categorized on the basis of the absence or presence of AF. Patients with documented AF were classified into two subgroups: the ongoing AF group and the new-onset AF group. We retrospectively evaluated the clinical profile, in-hospit

Brain natriuretic peptide as a surrogate marker for cardioembolic stroke with paroxysmal atrial fibrillation.

BACKGROUND: Cardioembolic stroke generally results in severer disability, since it typically has a larger ischemic area than the other types of ischemic stroke. However, it is difficult to differentiate cardioembolic from noncardioembolic stroke (atherothrombotic and lacunar stroke), whenever ischemic stroke patients have sinus rhythm at the time of presentation. METHODS: In this study, we evaluated the levels of plasma brain natriuretic peptide in acute ischemic stroke patients with cardioembolic or noncardioembolic stroke and assessed whether this could provide a basis for differentiating c

Atrial fibrillation in stroke-free patients is associated with memory impairment and hippocampal atrophy.

AIMS: To determine whether atrial fibrillation (AF) in stroke-free patients is associated with impaired cognition and structural abnormalities of the brain. AF contributes to stroke and secondary cognitive decline. In the absence of manifest stroke, AF can activate coagulation and cause cerebral microembolism which could damage the brain. METHODS AND RESULTS: We cross-sectionally evaluated 122 stroke-free individuals with AF recruited locally within the German Competence Network on AF. As comparator, we recruited 563 individuals aged 37-84 years without AF from the same community. Subjects un

Abrupt changes in fibrillatory wave characteristics at the termination of paroxysmal atrial fibrillation in humans.

AIMS: We investigated the process of spontaneous termination of atrial fibrillation (AF) to determine its time course from the surface ECG. METHODS AND RESULTS: We studied fibrillatory waves in Holter recordings of paroxysmal and sustained AF. Following QRS-T cancellation dominant frequencies (DFs) were computed and the relationship of DF to termination was scrutinized. For 57 episodes of paroxysmal AF (PAF) in 24 patients, DF ranged from 4.4 to 6.5 Hz (5.2 +/- 0.4 Hz) compared to 5.8 to 7.4 Hz (6.6 +/- 0.6 Hz) for sustained AF recordings. Comparison of the atrial frequency of the ultimate to

Inefficiency of renin-angiotensin inhibitors in preventing atrial fibrillation in patients with a normal heart.

AIM: Recent scientific evidence has emphasized the possible role of inhibitors of the renin-angiotensin system in preventing arrhythmic relapses in patients with paroxysmal or persistent atrial fibrillation and co-existing left ventricular hypertrophy or left ventricular dysfunction. METHODS: In order to verify the effects of these drugs on patients with a normal heart, we collected a series of 187 patients admitted to our division of cardiology for paroxysmal or persistent atrial fibrillation. All patients underwent cardioversion (with antiarrhythmic drugs and/or by electrical cardioversion)

An experimental sheep model used to develop an ablation procedure for chronic atrial fibrillation.

BACKGROUND: Atrial fibrillation is the most common form of serious arrhythmia in humans. The therapeutic options offered are medical, surgical, and interventional. The surgical approach is justified in cases of atrial fibrillation already subjected to cardiac surgery for an associated organic heart disease such as a valvular or ischemic disease. A minimally invasive surgical approach is needed to extend the possibility of surgical treatment to patients with lone atrial fibrillation and those who cannot be treated by interventional procedures. This study aimed to use sheep as an experimental mo

[Alterations in gene expression of calcium handling proteins in patients with chronic atrial fibrillation].

OBJECTIVE: To discuss the role of calcium-overloading in initiation and maintenance of atrial fibrillation (AF). METHODS: The right atrial appendages were obtained from 14 patients with AF and 12 patients with sinus rhythm. The mRNA expression of proteins influencing the calcium homeostasis was measured by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR) and normalized to the mRNA level of glyceraldehyde-3- phosphate dehydrogenase. The left atrial diameter (LAD), mitral valvular area (MVOA), and systolic pulmonary arterial pressure were obtained by echocardiography b

Atrial fibrillation in the Wolff-Parkinson-White syndrome: ECG recognition and treatment in the ED.

Estimated to occur in 0.1% to 0.3% of the population, Wolff-Parkinson-White syndrome (WPW) is a condition where atrial impulses bypass the atrioventricular node and activate the ventricular myocardium directly via an accessory pathway. Clinical clues to the diagnosis include a young patient with previous episodes of palpitations, rapid heart rate, or syncope. Although several different rhythm presentations are possible, atrial fibrillation is a not infrequent dysrhythmia seen in the WPW patient. Electrocardiographic features suggestive of WPW atrial fibrillation include irregularity of the rhy

Electrogram-guided isolation of the left superior vena cava for treatment of atrial fibrillation.

AIMS: Radiofrequency ablation targeting the pulmonary veins offers potential cure for patients with symptomatic paroxysmal atrial fibrillation (AF). Initiating ectopics can also arise from other sites including the major thoracic veins, vein of Marshall and more rarely, persistent left superior vena cava (LSVC). We report our experience with arrhythmogenic persistent LSVC initiating AF. METHODS AND RESULTS: The LSVC was present in four patients from an overall series of 204 patients undergoing AF ablation at our centre. All were males, mean age 50 +/- 11 years. All patients underwent pre-proc

Assessment of spatial organization in the atria during paroxysmal atrial fibrillation and adrenergic stimulation.

Non-linear parameters were computed to assess the extent of spatial organization in the atria in terms of coupling/synchronization between electrograms recorded in different atrial sites. Recordings of 9 patients suffering from paroxysmal atrial fibrillation were tested during four clinical experimental conditions: sinus rhythm and atrial fibrillation, both before and after isoproterenol infusion, a drug mimicking adrenergic activation. Two non-linear metrics were investigated: an index of non-linear association (NLA) and a synchronization (S) index based on the cross-conditional entropy. Resu
