In vivo and in vitro evaluation of a solid dispersion system of gliclazide:PEG 6000.

OBJECTIVE: Gliclazide is a potent antidiabetic agent because of its capability to decrease blood glucose level via stimulating endogenous insulin secretion from beta-pancreas cells. Gliclazide is insoluble in water and has low dissolution rate. In this study, polyethylene glycol (PEG) 6000 was used as a matrix to disperse gliclazide in the solid state, and the pharmacokinetic profile of this solid dispersion was studied in rats. DESIGN: The solid dispersion of Gliclazide:PEG 6000 (1:4) was prepared by solvent evaporation method. MAIN OUTCOME MEASURES: Samples characterization included differ

Sustained ocular drug delivery from a temperature and pH triggered novel in situ gel system.

Various ocular diseases like glaucoma, conjunctivitis, and dry eye syndrome require frequent drug administration. Poor ocular bioavailability of drugs (< 1%) from conventional eye drops is due mainly to the precorneal loss factors that include rapid tear turnover, nonproductive absorption, transient residence time in the cul-de-sac, and the relative impermeability of the drugs to corneal epithelial membrane. These problems may be overcome by the use of in situ gel-forming systems that are instilled as drops into the eye and undergo a sol-gel transition in the cul-de-sac. Our present work descr

Influence of food on the bioavailability of an enteric-coated tablet formulation of omeprazole 20 mg under repeated dose conditions.

The objective of this study was to investigate the influence of food on the bioavailability of omeprazole (20 mg) given as an enteric-coated tablet under repeated dose conditions. This open randomized crossover study consisted of three seven-day treatment periods, each separated by a drug-free period. During each treatment period an enteric-coated tablet of omeprazole was taken once daily either under fasting conditions, or immediately before or after a standardized breakfast. On the last day of each treatment period, blood samples for the determination of plasma omeprazole concentrations were

Is the tissue availability of circulating insulin-like growth factor I involved in organ damage and glucose regulation in hypertension?

BACKGROUND: Besides its capacity to regulate organ and tissue growth, the insulin-like growth factor I exerts biologic actions that resemble those of insulin. Tissue access of the factor depends on the distribution of the circulating bound factor between its binding protein 3 that remains within the intravascular space and its binding protein 1 that is able to cross the endothelium. OBJECTIVE: To investigate whether the distribution of the circulating factor between its binding proteins is altered in patients with essential hypertension and whether this is related to changes in organ damage a

Neoral--new cyclosporin for old?

Cyclosporin A is now well established as an effective second-line drug to treat rheumatoid arthritis. In April 1995, the microemulsion-based formulation of cyclosporin (Neoral) was introduced based on its increased bioavailability at 'no extra cost'. There may have been concerns that with increased bioavailability of Neoral, some patients might experience increased toxicity, particularly if transferring from Sandimmun to Neoral at the same dose. We describe our experience of 51 patients treated with Neoral--39 with rheumatoid arthritis, six with psoriatic arthritis and the remainder with a var

Feeding of lactic acid-fermented high nutrient density weaning formula in paediatric settings in Ghana and Nigeria: acceptance by mother and infant and performance during recovery from acute diarrhoea.

A period of acute diarrhoea in infants is often associated with anorexia and weight loss. Catch-up growth is, therefore, necessary and this depends very much on the type of food, in terms of nutrient density, protein quality and digestibility, given to the child. Fermented (F) and non-fermented (NF) maize-soybean porridges of increased nutrient density were formulated and compared with traditionally fermented maize-only porridge (P) for acceptability using mothers in peri-urban Ghana as well as urban Nigeria as subjects. Subsequently, infants hospitalised in paediatric wards for acute diarrhoe

[Retardation of the excitation conduction by propaphenone. Duration and dosage effect relation after oral administration (author's transl)].

After oral administration, and depending on dosage, propaphenone produces a uniform delay of the excitation conduction in the region of the atrium, ventricle and atrioventricular conduction. At a dosage of 5 or 10 mg/kg body-weight (1 or 2x 300 mg capsules) propaphenone causes an average delay of excitation conduction of + 11% or + 19% of the initial value, and of + 39% (capsule) or + 30% (sugar coated tablet) at 14 mg/kg body-weight. The maximum effect is attained 2.5 to 4 hours after administration. A prolongation of conduction times and consequently of the anti-arrhythmic efficacy, is still

Optimizing therapy in hidradenitis suppurativa: Combination, sequencing and treat-to-target.

Hidradenitis suppurativa (HS) is a chronic, recurrent and progressively destructive inflammatory disease associated with substantial physical, psychological and social burden. The therapeutic landscape of HS has rapidly evolved with the introduction of biologics targeting TNF-α and IL-17 pathways, increasing use of metabolic and hormonal interventions and growing integration of surgery with systemic therapy. However, despite expanding treatment options, many patients continue to experience incomplete disease control, progressive tunnel formation and cumulative disability. This narrative review

Window of Opportunity and Predictors of Sustained Remission in Patients with Hidradenitis Suppurativa Treated with Adalimumab: A 540-Patient Cohort Study.

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease. Predicting responses of "prolonged disease-free remission" to biological therapy is essential for identifying a "window of opportunity" for disease modification. METHODS: A retrospective multicenter cohort study (2015-2023) of 540 patients with HS treated with adalimumab (ADA) was performed. A super-response to ADA (ADA-SR; ADA super-responders) was defined as sustained complete remission (absence of nodules, abscesses, and inflamed or draining tunnels) for ≥ 12 months leading to drug discontinuation. Predicto

TraNce: Type-aware hypergraph neural network with biological mediators for drug repositioning.

Computational drug repositioning (DR) accelerates pharmaceutical discovery by identifying novel therapeutic indications for approved drugs. Recently, to capture higher-order biological interactions, hypergraph-based DR approaches are proposed. Nevertheless, existing studies exclusively rely on drug and disease similarity networks, or drug-disease association network, to construct hypergraph structures, while neglecting other diverse biological mediators. Furthermore, conventional hypergraph neural networks (HGNNs) encounter the intractable type-agnostic aggregation challenge, wherein heterogen

From amyloid PET to plasma biomarkers: A multimodal imaging framework for early diagnosis and staging of Alzheimer's disease.

INTRODUCTION: Alzheimer's disease (AD) is biologically defined by amyloid beta and tau pathology. Available biomarkers enable early diagnosis but differ in targets, accessibility, and utility. This review synthesizes evidence for plasma biomarkers, magnetic resonance imaging (MRI), and positron emission tomography (PET) to develop a pragmatic, stage-aware framework for early diagnosis and staging. METHODS: We conducted a narrative review of recent literature and professional guidance (2024-2026) covering plasma biomarkers; structural/functional MRI; and amyloid, tau, and fluorodeoxyglucose (F

Favorable clinical outcomes of biologic and targeted therapies in refractory eosinophilic fasciitis: a case-based systematic review.

Eosinophilic fasciitis (EF) is a rare autoimmune disease characterized by fascial inflammation, skin induration, eosinophilia, and progressive functional impairment. Although glucocorticosteroids (GCs) remain the first-line therapy, some patients develop refractory or steroid-dependent disease, highlighting the need for alternative treatment strategies. This case-based review summarizes current evidence on biologic and targeted therapies in severe EF and presents a case of successful treatment with tocilizumab (TCZ). A systematic review of PubMed, Scopus, and Web of Science databases identifie

Hematopoietic cell transplant compared with standard care in adolescents and young adults with sickle cell disease.

Disease-modifying therapies are standard of care (SOC) for sickle cell disease (SCD), but hematopoietic cell transplantation (HCT) has curative potential. We compared outcomes prospectively through 2 years after biologic assignment to a donor or no donor (SOC) arm based on the availability of an HLA-matched sibling or unrelated donor (BMT CTN 1503). A donor search was commenced after eligibility confirmation. The primary end point was a comparison of survival between the treatment arms 2 years after biologic assignment. Power calculations required 60 participants in the donor arm and 140 in th

Mepolizumab in Severe Pediatric Asthma: Certainties and Doubts through a Single-Center Experience and Review of the Literature.

BACKGROUND: Although, in most children with asthma, good symptom control is achieved with a low to moderate dose of inhaled corticosteroids, a small group of patients still experiences frequent symptoms, and even severe exacerbations, impairment of lung function, and reduced quality of life. Some of these subjects with severe asthma require biologic drugs as add-on therapy. In the past decade, numerous monoclonal antibodies have been approved for children or adolescents with severe asthma, in addition to their increasing use in adult asthma. However, the available evidence on how to select the
