Demographic-Based Personalized Left Ventricular Hypertrophy Thresholds for Hypertrophic Cardiomyopathy Diagnosis.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a leading cause of sudden cardiac death. Current diagnosis emphasizes the detection of left ventricular hypertrophy (LVH) using a fixed threshold of ≥15-mm maximum wall thickness (MWT). This study proposes a method that considers individual demographics to adjust LVH thresholds as an alternative to a 1-size-fits-all approach. METHODS: Left ventricular MWT was measured in 3 cohorts: a Reference Cohort of healthy adults (n = 5,067, no comorbidities), a Population Cohort (n = 43,239, with comorbidities), and an HCM Cohort from 6 international cent

Mechanistic Insights Into Qidan Yixin Decoction for Diabetic Cardiomyopathy via Macrophage Polarization.

Background: Diabetic cardiomyopathy (DCM) has a multifactorial etiology, and no specific treatment is available for its management. Qidan Yixin decoction (QDYXD) demonstrated encouraging clinical results in treating DCM; however, its underlying mechanics are yet unclear. Methods: Network pharmacology was applied to determine the active ingredients and targets of QDYXD. The GeneCards and GEO databases were used to retrieve genes associated with DCM and macrophage polarization. These targets were subjected to GO and KEGG enrichment analyses, immune infiltration study, and PPI network design. The

Transthyretin stabilizer targeting for transthyretin amyloid cardiomyopathy: A systematic review and meta-analysis.

INTRODUCTION: Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive disease caused by cardiac amyloid deposition. Transthyretin stabilizers (TTRS) are a potential treatment, but their efficacy and safety remain uncertain. This study aims to evaluate the effects of TTRS on all-cause mortality, functional capacity, quality of life, and adverse events in ATTR-CM patients. METHODS: A systematic review and meta-analysis was conducted in February 2025 using Cochrane Central, PubMed, and Embase to identify clinical trials and observational studies comparing TTRS versus no-TTRS therapies. Prognostic impact of late gadolinium enhancement granularity in non-ischemic dilated cardiomyopathy.

OBJECTIVES: We aimed to assess the additional prognostic value of the concept of "late gadolinium enhancement (LGE) granularity" in non-ischemic dilated cardiomyopathy (DCM) patients to predict all-cause death. METHODS: Between 2008 and 2021, we conducted a bicentric retrospective study including all consecutive DCM patients referred for Cardiovascular Magnetic Resonance (CMR). The primary outcome was all-cause death. Cox regressions were performed to determine the prognostic value of LGE findings. RESULTS: Of 1668 DCM patients recruited (age 52 ± 8 years, 54% male), 268 (16%) died after a m

Sauchinone preserves cardiac function in doxorubicin-induced cardiomyopathy by inhibiting the NLRP3 inflammasome.

BACKGROUND: Doxorubicin (Dox)-induced cardiomyopathy (DIC) is characterized by severe myocardial damage that can progress to dilated cardiomyopathy and potentially lead to heart failure. No effective prevention or treatment strategies are available for DIC. Sauchinone, a diastereomeric lignan isolated from Saururus chinensis, is known for its notable anti-inflammatory effects. However, a paucity of research on sauchinone in relation to heart disease exists, particularly regarding its role in DIC, which remains unclear. PURPOSE: This study aimed to assess the therapeutic potential of sauchinon

A Proposed Genetic Risk Score for Dilated Cardiomyopathy Susceptibility in the Chinese Han Population.

BACKGROUND: Dilated cardiomyopathy (DCM) is one of the most common types of cardiomyopathies. The current study constructs a proposed genetic risk score and assesses its potential as a predictive tool for individual DCM susceptibility. METHODS: A total of 1,000 patients with idiopathic DCM and 828 control participants from the study institution were enrolled. The top 25 up-regulated and down-regulated genes from tandem mass spectrometry analysis of 6 hearts with DCM and 7 control hearts (from a study by Chen et al) were selected for logistic analysis using whole-exome sequencing data. Statist…

Trends in diagnostic testing in Medicare patients with wild-type transthyretin amyloid cardiomyopathy.

BACKGROUND: Most patients with wild-type transthyretin amyloid cardiomyopathy (ATTRwt-CM) are diagnosed noninvasively, using nuclear imaging and monoclonal protein testing. However, concerns have been raised that some may receive an ATTRwt-CM diagnosis based on incomplete evaluations (not based on current consensus recommendations). Using a cohort of US Medicare Fee-for-Service patients, we aimed to examine the frequency and cadence of diagnostic testing for ATTRwt-CM. METHODS: In this retrospective observational cross-sectional study, administrative de-identified claims data from 2018 to 202

Coronary embolisation of a left ventricular fibroelastoma mimicking a thrombus and triggering Takotsubo cardiomyopathy.

We describe a case of a woman in her 70s who presented with acute chest pain and ST-segment elevation on ECG. Multimodality cardiac imaging confirmed Takotsubo cardiomyopathy alongside a pedunculated left ventricular mass, presumed to be a thrombus, with suspected embolisation to the left anterior descending artery, triggering an acute myocardial infarction. She received guideline-directed medical therapy for heart failure and anticoagulation for the thrombus. Her ejection fraction subsequently recovered. However, after 7 months of anticoagulation, serial imaging showed persistent ventricular

Mitochondrial Complex I Deficiency: Unraveling the Relevance of NDUFAF1 in Pediatric Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is rare in childhood, but it is associated with significant morbidity and mortality. Genetic causes of HCM are mostly related to sarcomeric genes abnormalities; however, syndromic, metabolic, and mitochondrial disorders play an important role in its etiopathogenesis in pediatric patients. We here describe a new case of apparently isolated HCM due to mitochondrial assembly factor gene NDUFAF1 biallelic variants (c.631C > T and an intragenic deletion encompassing exon 3, NM_016013.4). Alterations of this nuclear gene have been associated to Mitochondrial complex

Sex-Specific Clinical and Genetic Factors Associated With Adverse Outcomes in Hypertrophic Cardiomyopathy.

BACKGROUND: Females with hypertrophic cardiomyopathy present at a more advanced stage of the disease and have a higher risk of heart failure and death. The factors behind these differences are unclear. We aimed to investigate sex-related differences in clinical and genetic factors affecting adverse outcomes in the Sarcomeric Human Cardiomyopathy Registry. METHODS: Cox proportional hazard models were fit with a sex interaction term to determine if significant sex differences existed in the association between risk factors and outcomes. Models were fit separately for females and males to find t

Calcium handling remodeling in dilated cardiomyopathy: From molecular mechanisms to targeted therapies.

Calcium ions play a crucial role in cardiac excitation-contraction (EC) coupling, and disruptions in Ca2+ homeostasis are a key factor in the development of dilated cardiomyopathy (DCM). This review aims to systematically analyze how structural and functional remodeling of Ca2+-handling proteins drives DCM progression and to evaluate therapeutic strategies targeting these pathways. The movement of intracellular Ca2+, which is regulated by transporters like SERCA2a, ryanodine receptor 2 (RYR2), and L-type Ca2+ channels, affects the heart's contraction and relaxation. In DCM, both structural and

Exploring potential therapeutic targets for cardiomyopathy: A proteome-wide Mendelian randomization analysis.

Cardiomyopathy often results in heart failure and mortality, significantly impairing patients' quality of life. Advancements in genomics and proteomics now enable the identification of proteins associated with cardiomyopathy, offering valuable insights for its diagnosis and treatment. However, numerous potential pathogenic proteins remain unidentified, underscoring the need for further exploration of novel drug targets for cardiomyopathy. This study aims to employ Mendelian randomization (MR) to explore genetic associations between plasma proteins and cardiomyopathies, with the objective of id…
