A rare complication of vascular surgery - subclinical esophageal perforation secondary to supra-aortic debranching and TEVAR: a case report.

INTRODUCTION AND IMPORTANCE: Combined supra-aortic debranching and thoracic endovascular aortic repair (TEVAR) are well-established therapeutic options for complex aortic arch pathologies. Although rare, late complications such as esophageal injury can be life-threatening and require prompt multidisciplinary management. Several cases of delayed esophageal perforation complicated by mediastinitis have been reported in the literature. However, perforation presenting solely as a graft infection causing microembolizations remains exceedingly rare. CASE PRESENTATION: A 56-year-old male patient wit

The prevalence of comorbidities and differences in noncommunicable diseases and nonrandom associations of comorbidities between HIV-infected and -uninfected individuals in Guangdong Province, China.

BACKGROUND: Globally, uncertainty persists regarding the prevalence of comorbidities in noncommunicable diseases (NCDs) among people living with HIV (PLWH) compared those without HIV. This uncertainty extends to the degree of nonrandom associations between comorbidities in both populations, particularly in resource-limited settings. METHODS: This cross-sectional study involved 343 HIV-infected individuals (cases) and 686 HIV-uninfected counterparts (controls), with a 1:2 individual matching ratio. Nonrandom associations between comorbidities were assessed using Somers' D statistic. RESULTS:

Vascular Endothelial Protection Effects of Curcuma wenyujin Root Aqueous Extracts on LPS-induced Rat Vascular Damage.

Sepsis caused by bacterial infection can also induce vascular endothelial damage through endotoxins secreted by bacteria such as lipopolysaccharides (LPS). The mechanism of LPS induce vascular endothelial damage is mainly through the release of pro-inflammatory factors and activation of matrix metalloproteinases (MMPs), then MMPs further cause the glycocalyx layer to damage endothelial cells and finally lead to hyperosmolar vascular abnormalities and eventually microcirculatory disorders in general clinical practice, and are used in clinical treatment to prevent the release of pro-inflammatory

Current standard of care for septic shock.

Sepsis is a syndrome of life-threatening organ dysfunction that results from dysregulated host response to infection, with septic shock defined as persistent hypotension despite fluid resuscitation, a serum lactate > 2 mmol/L and the need for a vasopressor infusion to maintain a mean arterial pressure of at least 65 mmHg. Approximately, 49 million cases of sepsis are recorded worldwide annually, with 11 million sepsis-related deaths, the majority occurring in patients with septic shock. A substantial proportion of survivors suffer from moderate to severe functional limitations including physic

High-dose versus low-dose sufentanil anesthesia for patients undergoing off-pump coronary artery bypass grafting: A retrospective study from single cardiovascular center.

High-dose opioid anesthesia has been the mainstay of cardiac anesthesia for decades. Yet emerging evidence has shown that low-dose opioid anesthesia may be of equal safety and effectiveness in cardiac surgery. We compared the outcomes of 2 different sufentanil dosing regimens (<4 μg/kg vs > 4 μg/kg) in off-pump coronary artery bypass grafting (OPCAB) surgery. The study identified all patients undergoing elective OPCAB at a single large-volume cardiovascular center between January 2020 and December 2020. Based on total intraoperative sufentanil dosing, patients were divided into 2 groups: low-d

Do Long COVID and COVID Vaccine Side Effects Share Pathophysiological Picture and Biochemical Pathways?

COVID affects around 400 million individuals today with a strong economic impact on the global economy. The list of long COVID symptoms is extremely broad because it is derived from neurological, cardiovascular, respiratory, immune, and renal dysfunctions and damages. We review here these pathophysiological manifestations and the predictors of this multi-organ pathology like the persistence of the virus, altered endothelial function, unrepaired tissue damage, immune dysregulation, and gut dysbiosis. We also discuss the similarities between long COVID and vaccine side effects together with poss

Unraveling the resurgence of syphilis: a deep dive into the epidemic sweeping across the United States; a mini review.

Syphilis, a resurging public health concern in the United States, has witnessed a staggering rise in cases over the past decade. This highly contagious sexually transmitted infection caused by Treponema pallidum presents significant challenges due to its infectious nature and potential for severe complications. Despite the successful syphilis elimination plan launched in the early 2000s, which showed a decline in syphilis cases in highly funded states, syphilis has rapidly reemerged, with incidence rates steadily climbing across many states. Transmitted through sexual contact and vertically fr

Helicobacter pylori and Pro-Inflammatory Protein Biomarkers in Myocardial Infarction with and without Obstructive Coronary Artery Disease.

Myocardial infarction (MI) with obstructive coronary artery disease (MI-CAD) and MI in the absence of obstructive coronary artery disease (MINOCA) affect different populations and may have separate pathophysiological mechanisms, with greater inflammatory activity in MINOCA compared to MI-CAD. Helicobacter pylori (Hp) can cause systemic inflammation and has been associated with cardiovascular disease (CVD). We aimed to investigate whether Hp infection is associated with concentrations of protein biomarkers of inflammation and CVD. In a case-control study, patients with MINOCA (n = 99) in Sweden

Transient constrictive pericarditis: A complication of COVID-19 infection or first presentation of systemic lupus erythematous? A case report.

Cardiovascular disorders are significantly associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Herein, we describe a case of myopericarditis and subsequent transient constrictive pericarditis after coronavirus disease 2019 (COVID-19). Three weeks following a mild SARS-CoV-2 illness, a 53-year-old woman was referred to the hospital with acute pleuritic chest pain, which was not attributable to any known cause and was only temporarily relieved. The pain persisted for the next few weeks until her second COVID-19 infection, which occurred 5 months after her firs

Incidence and outcomes of hospital-associated respiratory virus infections by viral species.

BACKGROUND: Although the incidence of hospital-associated respiratory virus infection (HARVI) is well recognized, the risk factors for infection and impact on patient outcomes are not well characterized. METHODS: We identified a cohort of all inpatient admissions ≥24 hours duration at a single academic medical center from 2017 to 2020. HARVI were defined as respiratory virus detected in a test ordered after the 95th percentile of the virus-specific incubation period. Risk factors for HARVI were assessed using Cox proportional hazards models of the competing outcomes of HARVI and discharge. Th
