Sutimlimab for cold agglutinin disease: an updated perspective from approval to real-world clinical treatment.

INTRODUCTION: Sutimlimab, a classical complement pathway (CP) inhibitor, was approved in 2022 in the US, EU, and Japan for the treatment of cold agglutinin disease (CAD), a rare form of autoimmune hemolytic anemia (AIHA) characterized by CP‑mediated extravascular hemolysis and circulatory symptoms related to IgM‑mediated red blood cell (RBC) agglutination. This review reexamines the clinical trial data and compares those findings with published real‑world experience (RWE), providing clinicians with efficacy and safety data that extend beyond the clinical trial experience in this rare AIHA. On a Penalized Likelihood Approach for Joint Modeling of Longitudinal Covariates and Partly Interval-Censored Data-An Application to the Anti-PD1 Brain Collaboration Trial.

This paper considers the joint modeling of longitudinal covariates and partly interval-censored time-to-event data. Longitudinal time-varying covariates play a crucial role in obtaining accurate clinically relevant predictions using a survival regression model. However, these covariates are often measured at limited time points and may be subject to measurement error. Further methodological challenges arise from the fact that, in many clinical studies, the event times of interest are interval-censored. A model that simultaneously accounts for all these factors is expected to improve the accura

A Practical Guide to Optimal Adaptive Two-Stage Designs for Various Endpoint Types.

Adaptive designs are frequently used to adjust sample sizes based on interim data, ensuring reliable conclusions while protecting resources. To maximize trial efficiency, design parameters can be optimized to minimize an objective function (e.g., the expected sample size) while rigorously controlling overall error rates or further operating characteristics. However, existing methodological guidance for optimal adaptive designs mainly focuses on normally distributed data or discrete optimizations for binary data, leaving a limited practical framework to practitioners who work with other common

[From phase I-IV-Correctly understand and assess study designs in ophthalmology].

How reliable is the evidence for new forms of treatment in ophthalmology and what does each study phase actually demonstrate? Phases I-IV not only stand for different developmental stages but also particularly for different targets: from the first safety assessment and dosage results to the proof of efficacy and to data from treatment practice; however, especially in ophthalmology the assessment of clinical trials is particularly challenging. Bilaterality, invasive application routes, complex functional measurement and the increasing importance of imaging biomarkers make the planning, performa

Computational pharmacology and in silico clinical trials for the development of drugs from natural products: A review of advances, opportunities, and challenges.

Natural products have inspired the discovery of several drug candidates and US Food and Drug Administration-approved drugs. However, the conventional drug development pathway has several limitations, necessitating innovative strategies. Computational pharmacology and in silico clinical trials (ISCTs) have emerged as a potential to optimize the drug discovery process. Current literature rarely discusses these 2 domains together, leaving limited guidance on how computational tools used in natural product research can complement ISCT frameworks. This review addresses this gap by summarizing key c

Drug re-purposing to improve outcomes in the management of prostate cancer - aims, outcome measures and design of current phase III trials.

Prostate cancer remains a major cause of cancer morbidity and mortality and is rising in incidence across the world. Although a succession of randomised controlled trials have improved outcomes across all stages of disease, there remain significant clinical challenges and unmet needs. Repurposed drugs have attracted interest in the treatment of prostate cancer, where a number of agents have been proposed with a range of mechanisms of action suggesting potential benefits. A major advantage of such drugs is the wealth of pre-existing patient safety data typically available, meaning repurposed ag

Pharmacological properties, safety, pharmacokinetic and clinical trials of kukoamine A and B.

Kukoamine A (KuA) and kukoamine B (KuB) are spermine alkaloids characterized by polyamine backbones conjugated with phenolic moieties, primarily isolated from Lycium chinense root bark (Lycii Cortex). This review comprehensively analyses their pharmacological properties, mechanistic pathways, safety profiles, and clinical potential. KuA and KuB exhibit potent antioxidant activity by scavenging reactive oxygen species, enhancing superoxide dismutase and catalase activity, and chelating Fe2+. Their anti-inflammatory effects involve neutralizing pathogen-associated molecular patterns (e.g., LPS,

Advancing Cancer Cachexia Drug Development: Leveraging Biomarkers and Functional Endpoints to Optimize Trial Design.

BACKGROUND: Cancer-associated cachexia is a complex, multistage metabolic disorder that markedly contributes to morbidity and mortality in patients with advanced cancer, yet effective pharmacological interventions are lacking. Cachexia drug development has expanded significantly in recent years, with multiple repurposed agents and novel therapies targeting specific pathways entering clinical trials. However, clinical benefit has been limited not only by a lack of promising agents but also, to a large extent, by suboptimal trial design. RECENT FINDINGS: This article reviews the current evidenc

Limited inclusion in clinical trials and access to poly (ADP-ribose) polymerase inhibitors in racial and ethnic minority patients with cancer: a narrative review.

BACKGROUND: Poly ADP-ribose polymerase (PARP) inhibitors (PARPi) have revolutionized cancer care across multiple solid tumor types for patients with BRCA mutations and homologous recombination deficient malignancies. Sentinel trials for PARPi, however, have had limited participation from non-European populations, limiting interpretation of results, dosing strategies, and quality-of-life implications in these patients. Our objective was to review participation by race and ethnicity in sentinel PARPi trials across disease sites. METHODS: A literature search was conducted using PubMed/MEDLINE. C

Clinical implications of loss of interruption variants for diagnosis, genetic counselling, and clinical trials in Huntington's disease.

Age of onset in Huntington disease (HD) is influenced by cis-acting genetic variants, particularly the loss of interrupting codons in the HTT CAG and CCG repeats (CAG-CCG LOI variant). The CAG-CCG LOI variant is not detectable by current diagnostic assays, leading to underestimation of CAG repeat length, misdiagnosis, and inaccurate prediction of risk of symptom onset in the reduced penetrance range. In clinical trials, unidentified CAG-CCG LOI variants may affect interpretation of results, particularly for small trials. Accurate ascertainment and reporting of the CAG-CCG LOI genotype therefor

Vaccine efficacy estimands and power considerations.

The ICH E9(R1) addendum stresses the importance of clearly pre-specifying clinically interpretable treatment effect measures (estimands) and proposes different strategies to deal with intercurrent events. In this paper, we consider different estimands of the vaccine efficacy that are interpretable even when the proportional hazard assumption is not met, including estimands based on the average hazard ratio, the cumulative incidence ratio, and the restricted mean time lost ratio. Under the assumption that different estimands target relevant clinical questions, power becomes a crucial factor in

Evaluating the influence of vaccine archetypes and epidemiological conditions on clinical trial metrics for tuberculosis vaccine candidates.

BACKGROUND: Conducting tuberculosis vaccine trials in high-transmission settings offers potential gains in efficiency, yet the impact of repeated exposure in the presence of incomplete protection on measured vaccine efficacy is uncertain. We evaluated the effect of vaccine characteristics and epidemiological context on estimates of vaccine efficacy and requisite sample size for tuberculosis vaccine trials. METHODS: We simulated clinical trials of hypothetical tuberculosis vaccines using a stochastic compartmental model of tuberculosis natural history. We considered vaccines that confer "leaky

A data-driven atlas of CAR-based immunotherapy in lung cancer: Insights from multidatabase integration.

Lung cancer remains a leading cause of cancer-related mortality worldwide, and the clinical translation of chimeric antigen receptor (CAR)-based immunotherapy into solid tumors continues to be constrained by antigen heterogeneity, an immunosuppressive microenvironment, and stromal barriers that impede immune cell trafficking and curtail efficacy. Here, we present a comprehensive bibliometric analysis of CAR-based immunotherapy for lung cancer, drawing on publications indexed in the Web of Science Core Collection and Scopus between 2016 and 2025. Bibliometric and network analyses elucidate glob
