Health care providers who care for patients with seizure disorders should be able to recognize progressive myoclonus epilepsy. Progressive myoclonus epilepsy is a syndrome confused with myoclonic seizures and other epilepsies. The main symptom is myoclonus, a brief involuntary muscle jerk of varying intensity that can throw a patient against a wall or to the ground. This article describes major types of progressive myoclonus epilepsy, a typical case presentation and two clinical drug trials available for these patients. The focus of clinical drug trials is to identify a drug that controls the

Coccidioidomycosis is an acute, generally mild, respiratory disease that rarely becomes chronic or disseminated and fatal. The aetiologic agent is Coccidioides immitis, a dimorphic fungus growing in soil. The fungus is endemic in hot semi-arid areas in the USA and South America. Human beings and animals become infected by inhaling dust with arthroconidia. The fungus gives rise to either a purulent or a granulomatous inflammation or both. Clinically and radiologically, it can be confused with neoplasms and tuberculosis. The disease may be fatal in immuno-deprived persons who are also at risk of

The effect of prenatal X-irradiation on the ontogenesis of corticospinal tract (CST) neurons was examined in rats using retrograde labeling with Fast blue and intracellular Lucifer yellow staining. In prenatally irradiated rats, the cortical laminar architecture of the CST neurons was confused and many cells demonstrated migratory disturbances. Migratory-disordered CST neurons at deeper cortical levels resembled pyramidal cells, but their apical dendrites were oriented in various directions and the development of their dendrites was poor. Migratory-disordered CST neurons near the ependymal lay

Behavioral interventions can augment the effects of antipsychotic medication in alleviating hallucinations, delusions, and conceptual disorganization. Such interventions may be based on operant conditioning and reinforcement strategies and on training in coping skills. Reinforcement strategies have been used to decrease the rate of confused speech, delusional talk, and other psychotic behaviors, but they appear to have little effect on the subjective distress patients experience as a result of such symptoms. Strategies that teach patients skills for coping with psychotic symptoms include cogni

Acalvaria is a rare malformation usually regarded as a postneurulation defect. It consists of absence of the calvarial bones, dura mater and associated muscles in the presence of a normal skull base and normal facial bones. The condition is frequently confused by prenatal ultrasonography with anencephaly or an encephalocele. Whereas the cerebral hemispheres are absent in anencephaly, the cranial contents in acalvaria are generally complete, though some neuropathological abnormality is often present. The presumed pathogenesis of acalvaria is faulty migration of the membranous neurocranium with

Neospora caninum is a recently identified protozoan parasite which was previously confused with Toxoplasma gondii. Neosporosis in puppies commonly causes signs referable to parasitic encephalomyelitis and myositis. The parasite was identified in tissues from 3 9-week-old Labrador littermates which showed signs of progressive hind limb hyperextension and paralysis. The diagnosis was confirmed, utilising a specific avidin-biotin immunohistochemical technique. Attempts to treat the pups with clindamycin were unsuccessful. One case was additionally treated with immunosuppressive doses of corticost

A 61-year-old woman was admitted to our hospital because of hoarseness and abnormal shadow on chest X-ray. We diagnosed this patient as large cell carcinoma of the right upper lobe of the lung; T3N3M1 Stage IV. She was treated with OK-432, CDDP and CQ. On the 6th day after 2nd cycle chemotherapy, she was confused, and we diagnosed her as a case of hyperosmolar nonketotic coma (HNC) on the 7th day. Unfortunately, she died on the 8th day, after 20 hours of treatment for HNC. She suffered from chronic dehydration due to trouble with left recurrent nerve palsy. Although continuous intravenous hype

Infection with Mycobacterium bovis was diagnosed in a small privately owned herd of Sika deer. After postmortem examination of a deer with progressive pulmonary disease, diagnosis of infection with M bovis was confirmed by bacteriologic culture. The 2 remaining deer in this herd were euthanatized, necropsied, and confirmed to be infected with M bovis. Three cats in contact with the deer were also euthanatized and necropsied. One of these cats had lesions suggestive of mycobacterial infection in the colon and mesenteric lymph nodes. Infection of this cat with M bovis was not confirmed by bacter

Most important features of puberty are described. The puberty is the age of development of sexual characters. In females it appears between 10 and 15 years +/- 1 year whereas in males signs of puberty are visible between 11 and 15 years. In our country menarche appears at the mean age of 12 years and 2 months +/- 1 year and 2 months. Precocious puberty in females is that which appears before 8 years and 6 months and in males before 10 years. Delayed puberty is that which appears in females after 15 years and in males after 16. Nowadays is possible to keep off puberty when it is too soon by mea

Parvoviruses have long been associated with disabling and even fatal illnesses in animals. The discovery of the human parvovirus B-19 in 1975 (1) and subsequent studies of its effects in humans identified this virus as the causative agent of erythema infectiosum ("fifth disease") in children. (2). Erythema infectiosum (EI) is a common, self-limited infectious disorder in children, easily recognized by the classic "slapped cheek" facial erythema and fine reticular rash. Only in the 1980s have further investigations linked HPV B-19 infection with more significant clinical syndromes, among which

Myofibromatosis is a rare but well recognized entity which was originally thought to affect only neonates and infants. It is now apparent however that adults may also be affected. Solitary cases affecting the oral cavity appear to be rare and only two cases of solitary lesions in adults appear in the literature. This report documents three solitary cases; two typical lesions in infants and a lesion from the tongue of an adult. The lesions were composed of small polygonal cells and large elongated cells in a scant fibrous stroma. The elongated spindle cells were identified as myofibroblasts by

Six children had retinoblastoma, six Coats' disease, five retinopathy of prematurity, four persistent hyperplastic primary vitreous, one cysticercosis and one retinal detachment. Retinoblastoma and Coats' disease was only seen in children over eight months, whereas persistent hyperplastic primary vitreous and retinopathy of prematurity all presented before the age of six months. Differentiation between Coats' disease and retinoblastoma is not always easy. Most helpful for the diagno

The term "cluster vertigo" was originally used by Gilbert to describe episodes of vertigo in patients with Ménière's syndrome. Since these patients also had co-existing cluster headache, he suggested that both disorders could have had a common pathophysiology. There is no evidence in the literature for an increased incidence of Ménière's syndrome in cluster headache patients, so the argument that cluster headache and Ménière's syndrome may have a common pathogenesis cannot be supported. Subsequent authors have used the term "cluster vertigo" to denote a variant form of cluster headache and hav

Although intracranial hypertension may cause autonomic disturbances, as well as alterations in the regulation of body temperature, an acute hyperthermic syndrome with autonomic disturbance as a consequence of hydrocephalus has not been described previously. Two subjects presented with such a syndrome, with each of several episodes of acute shunt failure and hydrocephalus. With correction of the hydrocephalus, the autonomic disturbances and fever immediately cleared. Observations from human and experimental studies suggest some potential mechanisms for the development of the syndrome. One of th

A renal, pancreatic and hepatic dysplasia sequence (RPHD sequence) was found in a male premature baby who died a few minutes after birth. Autopsy documented multicystic dysplastic kidneys, a dysplastic pancreas with dilated ducts, cysts, fibrosis and inflammatory infiltrates, prominent portal tracts containing dilated bile ducts and hypoplastic lungs. Other organs were normal. This triad constitutes a "dysplastic sequence" and was first reported by Ivemark et al. as "familial dysplasia of kidneys, liver and pancreas". Since then, this combination of abnormalities has been named "polycystic dys

The myeloproliferative and dysmyelopoietic syndromes are complex disorders that are interrelated and not always easily diagnosed on morphologic grounds alone. In particular, in the case of the myeloproliferative diseases, the diagnostic pathologist should avoid the pitfall of attempting to make a definite diagnosis in the absence of appropriate laboratory and clinical data. The morphologic features in given myeloproliferative diseases are not always specific; and, as the disorders evolve, they may closely mimic one another. With respect to the dysmyelopoietic syndromes, care must be taken not

Because of obvious therapeutic and prognostic implications, as definitive a pathologic diagnosis as possible is necessary for necrotizing lesions of the midfacial mucous membranes. In the beginning of the diagnostic process, the lesions are placed under the noncommittal rubric of midfacial necrotizing lesions, thus forever avoiding confusing nomenclature such as lethal midline granuloma. Following this, the lesions are approached with the full appreciation they may be (1) localized or part of a systemic illness; (2) neoplastic or non-neoplastic; (3) caused by a specific agent or organism or be

Muscle and joint pains and/or weakness are not usually stressed as central symptoms in hypothyroidism. Two cases of long-standing hypothyroidism presenting with prominent myopathic symptoms are described. The first patient presented with a 12-year history of proximal myopathy, arthropathy and skin abnormalities, and florid primary myxedema was diagnosed. No evidence for a systemic autoimmune process was found. The second patient had been treated with irradiation to the neck 23 years before admission and presented with clinical and laboratory signs of both proximal myopathy and hypothyroidism.
