Nail varnish allergy with far-reaching consequences.

Contact allergy to nail varnish is well-known, and toluene-sulphonamide formaldehyde resin (TSAfr) was identified as the main allergen in 1943. During the period October 1989-December 1991 we identified 18 cases of contact allergy to nail varnish. The aim of the study was to describe the clinical picture, patch-test results, course and socio-medical consequences. Seventeen of the 18 patients were patch-test positive to TSAfr and 17/18 were positive to their own nail varnishes. Fourteen of the 18 were also positive to one or more substances in the standard patch-test series. The lesions were sc

Chromium content in human skin after in vitro application of ordinary cement and ferrous-sulphate-reduced cement.

The amount of chromium found in human skin after in vitro application of cement suspensions on full-thickness human skin in diffusion cells was investigated. Cement suspensions made from ordinary Portland cement or Portland cement with the chromate reduced with added ferrous sulphate were used. The cement suspensions were either applied on the skin surface under occlusion for 48 h or applied repeatedly every 24 h for 96 h. No statistically significant difference in chromium content of skin layers between skin exposed to ordinary Portland cement, skin exposed to cement with added ferrous sulpha

Patch testing with pure palladium metal in patients with sensitivity to palladium chloride.

During a 15-month period, 536 patients being investigated for suspected contact dermatitis were patch tested with the European standard series and palladium chloride 1% pet. 13 patients (2.4%) had a positive allergic response to palladium chloride and all 13 were also allergic to nickel. 12 of these 13 patients consented to further patch testing with discs of pure palladium metal foil, and none reacted. We have shown previously that palladium chloride patch test material contains traces of nickel, and propose an explanation for these results in terms of the additive effect of allergens when te

Long-standing dermatological manifestations in a patient with chronic heavy metal intoxication.

A patient with chronic metal intoxication is described, presenting during four years after the cessation of her exposure to industrial substances, maculo-papular eruptions with several ulcerated lesions and excoriations on her abdomen and buttocks. She also had pallor of her face, greyish-dark discoloration of the hair, while the fingernails were brittle and sensitive. Scrupulous physical examination excluded further cutaneous involvement. The immunological workup revealed both phenotypic and functional defects in cellular immunity.

Atopic eczema, Langerhans cells and allergy.

While the pathomechanisms of respiratory atopy are rather well established, the role of IgE-mediated hypersensitivity in the elicitation and maintenance of eczematous skin lesions in atopic eczema is still controversial. Few diseases are characterized by an equally elevated production of IgE antibodies as atopic eczema. Many authors, however, regard this only as epiphenomenon. On the other hand, there is clearcut clinical evidence for exogenous elicitation of atopic eczema by contact with aero or food allergens. A variety of hypotheses may help to explain the participation of IgE antibodies in

[Occupational dermatoses among workers in slag separation plants of the low-carbon ferrichrome industry].

Slag separation shops in low-carbonaceous ferrochromium production, although provided with ecologically and economically justified closed waste-free technological scheme, are marked by occupational dermato-pathologies among the workers (epidermitis, contact dermatitis, allergic dermatoses) as well as by certain prevalence of some non-occupational skin and fat-tissue disorders caused by hazardous labour conditions. The article reveals the complex characteristics of slag's affecting the skin, which include primary irritating factors (alkali) and allergenic factors (chromium-hapten). The contribu

Occupational dermatoses and allergy to metals in Polish construction workers manufacturing prefabricated building units.

The incidence of dermatoses and allergy to metals (Cr, Co, Ni) was determined in 1782 workers exposed to cement, waste fly ash and asbestos cement. They were also exposed to reclaimed (used), mineral oils. Dermatitis was found in 23.6% of the subjects, and oil acne in 11.2%. Allergy to chromium was found in 23% of the subjects; the % of definitely positive patch test results (the total of positive +(+) and strongly positive +(+)+ was, however, 8.6%. Allergy to cobalt was found in 13.4% of the subjects examined (definite in 3.1%). Allergy to nickel was found in 2.7% of the subjects (definite in

[Cutaneous accidents caused by self-defense sprays].

The free sale of "self-defence sprays" and the use of such weapons by the police and riot squads account for the increase observed in the frequency of cutaneous accidents. Tear gases. 1. CN, or chloracetophenone, is a compound insoluble in water and soluble in alcohol, ether and carbon sulphide. MACE is CN dissolved in methylchloroform. 2. CS, or orthochlorobenzylidene malononitrile is also insoluble in water and can be used in sprays in concentrations of 2 to 8 p. 100, propelled by such gases as freons. Clinical effects. 1. Contact allergy to styrene and related chemicals.

One patient, sensitive to styrene, cross-reacted at patch testing to 2-,3-and 4-vinyltoluene (2-,3-and 4-methylstyrene) and to the metabolites styrene epoxide and 4-vinylphenol (4-hydroxy-styrene). It is assumed that styrene is a prohapten metabolized in the skin by aryl hydrocarbon hydroxylase (AHH) to styrene epoxide acting as the true hapten. Styrene occurs in nature and as a synthetic product. Vinyltoluenes (methylstyrenes) occur as synthetic products in plastics. 4-vinylphenol (4-hydroxystyrene) occurs frequently in different ciders, wines, foods and berries, e.g. cloudberry.

[Prophylaxis of patients with sensitization to chrome and nickel using a cosmetic preparation for topical use].

Firstly we showed the reason for the choice of each product used in the preparation of this cosmetic. After this we studied "in vitro" the effect of this preparation, and "in vivo" in 25 patients sensitive to Potassium dichromate. On seeing the null effect of this preparation, owing to the oxidation of ascorbic acid, on these 25 patients, we modified the formula, substituting the ascorbic acid and EDTA, respectively, for tartaric acid and glycine. The respective tests "in vitro" and "in vivo" on 20 patients, this time showed beneficial effect of this preparation in 60% of the patients tested.

[Clinical testing of a new local corticoid: desoximethasone].

In a double-blind comparative trial on 37 patients with symetric dermatoses, the clinical effect of desoximetasone (0.25%) has been compared to fluocinolone acetonide (0.2%). After 7 days of treatment an improvement or cure of the lesions could be seen in 34 patients treated with desoximetasone, and only on 22 patients treated with fluocinolone acetonide (0.2%). The better effect of desoximetasone was especially evident on erythema and pruritus. Differences between the drugs were statistically significant.

Legislation on prevention of occupational dermatoses.

Legislation is one way to eliminate sensitizers and irritants. Purity requirements, declaration and labelling, industrial health codes and pre-employment occupational aptitude examinations are useful in combating occupational and contact dermatitis by drugs and products of daily use. Chromium compounds, turpentine, balsam of Peru, benzocaine and other can be eliminated by administration and legislation.
