Therapeutics Beyond Delivery: Repurposed Drugs and Biologic Pathways.

PURPOSE OF REVIEW: Pre-eclampsia remains a leading cause of maternal and perinatal morbidity and mortality worldwide, yet no disease-modifying therapy currently exists. This review examines the challenges inherent in developing therapeutics for pre-eclampsia, and evaluates the evidence for repurposing existing drugs and developing novel biologic strategies to treat preterm pre-eclampsia. RECENT FINDINGS: Metformin has demonstrated a trend towards prolongation of pregnancy in a randomized controlled trial, with larger trials ongoing. Proton pump inhibitors and sulfasalazine show compelling pre

Mondor's Disease of the Penis: Clinical Presentation, Diagnosis, and Conservative Management.

Mondor's disease of the penis is a rare, benign thrombophlebitis of the superficial dorsal vein, often presenting as a painful, cord-like induration along the penile shaft. Though alarming to patients, it is a self-limiting condition that responds well to conservative management. A 54-year-old male presented with pain and swelling over the dorsal shaft of the penis for 15 days. Examination revealed a tender, cord-like induration without ulceration or discharge. Laboratory parameters were normal. Doppler ultrasonography demonstrated a dilated, thrombosed superficial dorsal vein with loss of ven

Stereotactic Body Radiotherapy for Refractory Ventricular Tachycardia: From Serendipity to Precision Biological Ablation.

Ventricular arrhythmias (VAs), particularly refractory ventricular tachycardia (VT), pose a significant therapeutic challenge and are associated with high morbidity and mortality. For patients who do not respond to conventional therapies, including antiarrhythmic drugs and catheter ablation (CA), stereotactic body radiotherapy (SBRT)-also known as stereotactic arrhythmia radioablation (STAR)-has emerged as a promising non-invasive alternative. This review synthesizes current clinical evidence from prospective trials, retrospective series, and case reports that collectively demonstrate the effi

Heart failure in autoimmune rheumatic diseases.

Heart failure (HF) is an increasingly important cause of morbidity and mortality in patients with autoimmune rheumatic diseases. Despite advances in cardiovascular prevention and treatment, HF incidence continues to rise in this population, driven by chronic systemic inflammation, immune-mediated myocardial injury, microvascular dysfunction, fibrosis, and treatment-related cardiotoxicity. Epidemiological studies consistently demonstrate a markedly increased HF risk across a broad spectrum of rheumatic diseases—including rheumatoid arthritis, systemic lupus erythematosus, systemic sclerosis, id

Ribosomal architecture and rRNA modification landscape in the tick-borne parasite Babesia divergens.

Babesia are tick-borne intracellular apicomplexan parasites that infect a wide range of wild and domestic animals (e.g., cattle), resulting in significant economic losses to the livestock industry. Humans are considered accidental hosts for a few Babesia species. Babesia microti and B. divergens are the most prevalent causes of human babesiosis that are showing a broadening geographic distribution. Due to the complex life cycle of Babesia species, their survival depends on the precise control of gene expression, which is primarily regulated by epigenetic, transcriptional, and post-transcriptio

Non-Drug therapeutic strategies to combat antimicrobial resistance in livestock: a review.

BACKGROUND: Antimicrobial resistance (AMR) represents an escalating global public health crisis, projected to cause 10 million deaths annually by 2050. Pathogens such as bacteria, viruses, fungi, and parasites evade treatments in animals and humans due to overuse and misuse of antimicrobials, particularly antibiotics, in medical and veterinary practices. This drives a worldwide surge in resistant infections, disproportionately burdening livestock health and productivity. OBJECTIVES: This review examines the global rise of AMR and its impacts on livestock and public health, underscoring the ne

Mouse Models for Use in Cryptosporidium Infection Studies, Quantification of Parasite Burden Using Flow Cytometry, qPCR and Histopathology, and Confocal Imaging of Oocysts.

Cryptosporidiosis threatens the lives of young children in developing countries and newborn calves around the world. No vaccine or therapy can prevent or cure this diarrhea-inducing enteric disease caused by Cryptosporidium spp. protozoan parasites. There is an essential need to discover new therapeutic drugs efficient in reducing parasite burden in infected individuals. Research therefore relies on reliable small animal models of cryptosporidiosis. Here, we present excellent mouse models which can efficiently mimic pathogenesis of human and bovine cryptosporidiosis. We also describe methods t

Fluoroquinolone-Induced Achilles Tendon Damage: Structural and Biochemical Insights into Collagen Type I Alterations.

Fluoroquinolones, one of the most frequently used antibiotics, despite their wide spectrum of beneficial activity, are linked to serious adverse effects such as tendinopathies. Tendon injuries connected to the use of the group of drugs frequently affect the Achilles tendon-an anatomical structure, crucial to the proper mobility of lower limb, that is made of collagen fibers and extracellular matrix (ECM). Fluoroquinolones derive and decrease collagen and proteoglycans synthesis; they also disturb tendon regeneration by downregulating activity of metalloproteinases, enzymes essential for the pr

A Triple-Action Inhibitory Mechanism of Allosteric TYK2-Specific Inhibitors.

Deucravacitinib is a highly selective allosteric inhibitor of the protein TYK2. It targets the Jak-signal transducer and activator of transcription pathway. Despite its selectivity, the structural basis for its inhibition mechanism remains poorly understood. In this study, we analyze available atomic resolution structures relevant to the Jak-signal transducer and activator of transcription pathway to investigate the TYK2 inhibition mechanism. Our computational analysis suggests a mechanistic hypothesis for the relatively rapid IFN-induced gene expression mediated by TYK2 compared with that of

Targeting Various Pathogenic Pathways for the Development of Anti-alzheimer's Drugs: An Update.

Alzheimer's disease (AD) has been recognized as the most important cause of dementia, which is estimated to contribute more than 2 trillion USD in medical costs. AD patients encounter progressive neurodegenerative dementia associated with behavioural, linguistic, and visuospatial deficits. Although studies on the discovery of amyloid β (Aβ) and tau (the essential elements of plaques and tangles in AD) have shed light on the molecular pathological processes of AD, the exact cause of the condition is still largely unknown. The involvement of various proteins, such as amyloid-β, prion protein, ta

[Molecular pathogenesis and management of myeloma bone disease].

The advent of novel anti-multiple myeloma (MM) agents has led to dramatic improvement in patient survival. Nevertheless, the majority of patients with MM have bone lesions, and destructive bone lesions significantly reduce quality of life. Progressive destructive bone lesions develop when osteoblast differentiation from bone marrow stromal cells is inhibited and osteoclasts are activated in the bone marrow microenvironment in patients with MM. Recent research has also shed light on the functions and roles of osteocytes, which account for the majority of bone cells. Since MM cells mainly invade

N-Alkyl Sulfamates as a New Class of nsP2 Cysteine Protease Inhibitors with Broad-Spectrum Antialphaviral Activity.

The emergence of mosquito-borne alphaviruses that cause chronic arthritis or encephalitis underscores the urgent need for broad-spectrum antiviral therapeutics. The viral nsP2 cysteine protease, which is essential for alphavirus replication, is a promising antiviral target. Vinyl sulfone covalent inhibitors potently inhibit nsP2 protease but suffer from glutathione reactivity and species-dependent systemic clearance catalyzed by glutathione S-transferase. To address these liabilities, we explored alternative electrophilic warheads and identified acetamide inhibitors bearing N-alkyl sulfamate w

Leveraging magnetic resonance imaging to assess biocompatible scaffolds diffusion and flow-driven perfusion for lab-on-a-chip systems.

Biocompatible scaffolds play a crucial role in creating 3D in vitro models for precision medicine. Understanding liquid transport dynamics is essential for ensuring timely delivery of nutrients, drugs or imaging agents to cells embedded within the scaffold. Therefore, it is imperative to thoroughly characterise the physical properties of these scaffolds. In this study, we leverage nuclear magnetic resonance imaging (MRI) to examine the diffusivity and flow-driven perfusion properties of commonly used cryogels and hydrogels. We used deuterium oxide (i.e. heavy water) as a contrast agent to moni

Discovery of novel diarypyrimidine derivatives as potent HIV-1 NNRTIs by exploiting the tolerant region I within NNRTI binding pocket.

Non-nucleoside reverse transcriptase inhibitors (NNRTIs) serve as an essential component of antiretroviral treatment regimens, but they encounter challenges such as resistance mutations, poor solubility, and adverse effects. This study presents the design, synthesis, and biological evaluation of novel diarylpyrimidine (DAPY)-typed derivatives targeting the tolerant region I within the NNRTI binding pocket (NNIBP). We introduced aliphatic ring structures with high Fsp3 value, aiming to enhance target affinity while simultaneously improving water solubility. Among them, compound 9k demonstrated

Thrombotic Thrombocytopenic Purpura During Anti-Tuberculosis Therapy: A Case Report and Literature Review.

INTRODUCTION: Thrombotic thrombocytopenic purpura (TTP) is a rare, life-threatening thrombotic microangiopathy and a high mortality rate if untreated. While TTP can be primary or secondary to factors like drugs, reports linking it to anti-tuberculosis (anti-TB) therapy are scarce. This case highlights the diagnostic challenges and need for vigilance in TB patients receiving standard regimens. CASE PRESENTATION: A 76-year-old male on anti-TB therapy (isoniazid, rifampicin, pyrazinamide, ethambutol) presented with acute neurological symptoms, thrombocytopenia (platelets 9×10^9/L), microangiopat

Pseudocellulitis Due to Antineoplastic Therapy: A Case of Atypical Presentation and Recent Developments.

The term "pseudocellulitis" refers to a non-infectious, non-necrotizing inflammation of the dermis and hypodermis. A skin reaction seen in antineoplastic therapy-treated patients, it mimics cellulitis by displaying symptoms of erythema and swelling that are not caused by infection or trauma. Previously considered rare, the incidence of pseudocellulitis has been increasing due to the continuous expansion of antineoplastic armamentarium. The most common drugs associated with pseudocellulitis are gemcitabine and pemetrexed, but some of the newer agents have also been reported as potential culprit

Ophthalmic Toxicities in BEACON Triplet Therapy for BRAF V600E-Mutant Colorectal Cancer: A Case Series.

The BEACON regimen, comprising cetuximab, encorafenib, and binimetinib, is a critical treatment for BRAF-mutant metastatic colorectal cancer (mCRC). While effective, this regimen may be associated with a high frequency of serous retinal detachment (SRD) as an ocular toxicity, which is a less recognized risk in colorectal cancer. Here, we report three patients with mCRC who developed SRD shortly after starting BEACON therapy. Each patient experienced acute visual disturbances, and optical coherence tomography (OCT) confirmed SRD without other retinal abnormalities. Temporary discontinuation of

From Synaptic Plasticity to Neurotoxicity: Endocannabinoid Influence on Addiction and Neurodegeneration.

The endocannabinoid system (eCBS) is a versatile neuromodulatory network that orchestrates synaptic plasticity, reward processing, and neuronal homeostasis. Increasing evidence implicates eCBS dysregulation in both addiction and neurodegenerative (ND) disorders, suggesting overlapping molecular and cellular mechanisms underlying these conditions. This review synthesizes recent advances in understanding how eCBS components-cannabinoid receptors (CB1 and CB2), endogenous ligands (anandamide and 2-arachidonoylglycerol), and their metabolic enzymes-modulate dopaminergic and glutamatergic signaling
