A multicenter, double-blind, randomized, controlled study of 203 Israeli patients with endoscopically proven duodenal ulcer is described. The study compares the efficacy (i.e., ulcer healing and relief of symptoms) and safety of 20 mg omeprazole once daily in the morning, with those of 300 mg ranitidine once daily at night. 80 liquidators of Chernobyl disaster with aggravation of duodenal ulcer were examined, as well as a control group of 70 patients. In the group of liquidators some characteristic features were disclosed, namely peculiarities in the state of abdominal mucosa and clinical course of the disease. We investigated the effect of phytohemagglutinin and the neuropeptide beta-endorphin on the in vitro lymphocyte proliferative response in patients with chronic ulcer disease and upper gastrointestinal bleeding or in remission, acute ulcer or hemorrhagic gastritis, and in healthy persons without upper gastrointestinal disease. We found that the addition of beta-endorphin produced a strongly enhanced proliferative response to phytohemagglutinin in lymphocytes from patients with chronic ulcer disease, and in particular from those with recent bleeding.

The purpose of this study was to compare duodenal ulcer healing, symptom relief, and safety of lansoprazole (a new proton pump inhibitor) given at doses of 30 mg and 60 mg, in the morning with ranitidine 300 mg at bedtime. Two hundred and eighty nine patients were enrolled over a 20 month period in a double blind randomised parallel group comparative study set in outpatient endoscopy units of six United Kingdom medical centres. Patients were randomised to receive lansoprazole 30 mg in the morning (n = 95), 60 mg in the morning (n = 96), or ranitidine 300 mg at bedtime (n = 98) for four weeks.

The relationship between gastric mucosal blood flow and acid secretion was studied in 39 patients. In the active stage of gastric ulcer, the volume of gastric mucosal blood flow (GMBF) was decreased, and that of GMBF at the ulcer margin was decreased significantly and increased markedly at the healing stage. An analysis was carried out of the efficiency of using sound waves, drugs and electrostimulation on the motor activity of the duodenum in 45 patients with chronic duodenal obstruction of functional genesis. Excellent and good results were obtained in 80% of patients treated with sound waves complexly with other methods, in 40% when only drugs were used and 53% when only electric stimulation was used. The advantages of sound in the treatment are emphasized as concerns early effect, long-term results, absence of side-effects, low incidence of recurrences of the disease.

The authors analyse the results of esophago-gastro-duodenal fibroscopy in 930 symptomatic patients. Ninety one per cent of them had lesions. Inflammatory pathology was predominant: esophagitis, gastritis and duodenitis were seen in 21.5%, 47% and 29.08% respectively of the patients investigated. Overnight gastric secretion was studied in 32 patients with acute duodenal ulcers before treatment and whilst taking cimetidine 400 mg b.d. The synchronous change in the antral release of gastrin and somatostatin into a vein draining the stomach was studied during acidic and alkaline intragastric pH in six anaesthetised duodenal ulcer patients and six controls after atropinisation. Seventy-three patients with pathologic liver function tests were examined using ultrasound one day prior to liver biopsy. The ultrasound findings were compared with the histologic findings. In 23 patients enlarged lymph nodes were found in the hepatoduodenal ligament and 21 of these had active immune-mediated liver disease.
