Individual circulating microRNAs and continuous cognitive performance in a mixed inpatient-outpatient geriatric cohort: A cross-sectional pilot study.

BACKGROUND: Circulating microRNAs have been proposed as minimally invasive markers of cognitive impairment and neurodegenerative disease, but evidence for individual miRNAs in heterogeneous older populations remains limited. OBJECTIVE: We examined cross-sectional associations between eight circulating serum miRNAs and continuous cognitive performance in a mixed inpatient-outpatient geriatric cohort. METHODS: This cross-sectional analysis included 65 enrolled participants from a single-center, non-consecutive, mixed inpatient-outpatient geriatric cohort; 63 formed the analytic cohort (32 inpa

Longitudinal study of psychiatric symptoms, disability, mortality, and emigration among Bosnian refugees.

CONTEXT: Evidence is emerging that psychiatric disorders are common in populations affected by mass violence. Previously, we found associations among depression, posttraumatic stress disorder (PTSD), and disability in a Bosnian refugee cohort. OBJECTIVE: To investigate whether previously observed associations continue over time and are associated with mortality emigration to another region. DESIGN, SETTING, AND PARTICIPANTS: Three-year follow-up study conducted in 1999 among 534 adult Bosnian refugees originally living in a refugee camp in Croatia. At follow-up, 376 (70.4%) remained living i

Association of a Physician-Nurse Collaborative Multimodal Analgesic Pathway with Early Pain Trajectories and Institution-Recorded Opioid Use After Total Knee Arthroplasty: A Propensity Score-Matched Retrospective Cohort Study.

BACKGROUND: Multimodal analgesia is widely used after total knee arthroplasty (TKA), but evidence regarding the real-world effectiveness of bundled opioid-sparing pathways remains limited. This study compared a standardized physician-nurse collaborative pathway with conventional care delivered during an earlier historical period. METHODS: This single-center retrospective cohort study included adults undergoing primary unilateral TKA. The modified pathway combined physician-prescribed scheduled celecoxib and tramadol/paracetamol rescue analgesia with nurse-delivered cryotherapy, limb elevation

Concussion Nondisclosure in Youth Sports.

CONTEXT: Researchers conducting studies about sport-related concussion (SRC) reporting behaviors and reasons for nondisclosure primarily focus on older athletic populations. Youth athletes participating in contact sports are also at risk for SRC; however, little is known about their SRC disclosure patterns and reasons for nondisclosure. OBJECTIVE: To examine the self-reported SRC history and reasons for SRC nondisclosure of youth athletes aged 8-14 years. DESIGN: Cross-sectional study. SETTING: Youth athletes in organized football, ice hockey, lacrosse, or soccer. PATIENTS OR OTHER PARTICI

Cardiac surgery report cards: comprehensive review and statistical critique.

Public report cards and confidential, collaborative peer education represent distinctly different approaches to cardiac surgery quality assessment and improvement. This review discusses the controversies regarding their methodology and relative effectiveness. Report cards have been the more commonly used approach, typically as a result of state legislation. They are based on the presumption that publication of outcomes effectively motivates providers, and that market forces will reward higher quality. Numerous studies have challenged the validity of these hypotheses. Furthermore, although stat

State-of-the-Art management of knee osteoarthritis.

Osteoarthritis (OA) is the most common type of arthritis found in the United States' population and is also the most common disease of joints in adults throughout the world with the knee being the most frequently affected of all joints. As the United States' population ages along with the increasing trends in obesity prevalence in other parts of the world, it is expected that the burden of OA on the population, healthcare system, and overall economy will continue to increase in the future without making major improvements in managing knee OA. Numerous therapies aim to reduce symptoms of knee O

Burden evaluation and prediction of osteoarthritis and site-specific osteoarthritis coupled with attributable risk factors in China from 1990 to 2030.

OBJECTIVE: This study aimed to estimate and predict the burden of osteoarthritis (OA) and site-specific OA (hip, knee, hand, and others) from 1990 to 2030 and their attributable risk factors in China. METHOD: Data were obtained from the Global Burden of Diseases 2019. The burden was estimated by analyzing the trends of prevalence, incidence, and disability-adjusted life years (DALY). Population attributable risk (PAR) was calculated to assess the impact of high body mass index (BMI). The prediction from 2020 to 2030 was implemented by Bayesian age-period-cohort analysis. RESULTS: In China, p

Optimism and Longevity Among Japanese Older Adults.

BACKGROUND: Optimism has been linked to better physical health across various outcomes, including greater longevity. However, most evidence is from Western populations, leaving it unclear whether these relationships may generalize to other cultural backgrounds. Using secondary data analysis, we evaluated the associations of optimism among older Japanese adults. METHODS: Data were from a nationwide cohort study of Japanese older adults aged ≥65 years (Japan Gerontological Evaluation Study; n = 10,472). In 2010, optimism and relevant covariates (i.e., sociodemographic factors, physical health c

Skating on Thin Ice: Craniofacial Injuries in Amateur Ice Hockey.

INTRODUCTION: Hockey is a sport of high speeds, projectiles, and slick surfaces. A scenario is ripe for craniofacial injuries. Annually, over 1 million Americans, with many more abroad participate in organized hockey. This number continues to grow at youth, amateur and professional levels. Following the characterization of groups with the highest incidence of hockey-related craniofacial injuries, our goal is to propose guidelines for the acute management of hockey-related craniofacial injuries in amateur settings. METHODS: This study follows a 10-year retrospective cohort design, examining ho

Symbolic and non-symbolic geometric abilities across development and in individuals with dyscalculia and dyslexia.

Geometry is fundamental to mathematical thinking, yet its foundations and difficulties in learning disorders remain poorly understood. Here, we investigated the development of geometric abilities and their relationship with developmental dyscalculia and dyslexia. Seventy-nine children and adults, including individuals with and without learning difficulties, completed behavioral measures of non-symbolic geometry and a paper-and-pencil assessment of symbolic geometry. Geometric abilities continued to develop from early adolescence into young adulthood and non-symbolic geometry predicted symbolic

Educator's Blueprint: Six Tips for Navigating What Emergency Physicians Cannot Control.

Emergency physicians repeatedly encounter problems that clinical expertise alone cannot resolve, such as a patient with decision-making capacity leaving against advice, a consultant declining to come in, admitted patients boarding for hours, or a family asking for continued resuscitation after the team concludes that it cannot succeed. Residency prepares us well for what we can control but offers little guidance about the rest. Some of these encounters produce moral distress; others are frustrating limitations. This Educator's Blueprint presents six tips to face such situations adapted from St

Advancing Perioperative Medicine: A Multistakeholder Perspective.

Perioperative medicine spans the entire surgical care continuum, improving patient outcomes while reducing morbidity and mortality. Anesthesiology is uniquely positioned to influence the continued development of perioperative medicine. Recognizing this opportunity, the American Society of Anesthesiologists (Schaumburg, Illinois) established the Center for Perioperative Medicine as an organizational framework to actively implement perioperative medicine. Whereas earlier efforts focused on why anesthesiology must evolve, the Center emphasizes how the specialty is advancing through rigorous, incl

Rational Use of Benzodiazepines in Modern Healthcare: Evidence-Based Strategies.

Benzodiazepines (BZDs) are among the most widely prescribed psychotropic drugs in modern healthcare, primarily used as anxiolytics, hypnotics, anticonvulsants, and muscle relaxants. However, rising global consumption and prolonged use raise significant concerns about safety and dependence. Recent studies report that up to 35.8% of patients continue BZD therapy beyond three months, with long-term use observed in over 5% of the general population. These patterns highlight the need for evidence-based strategies to improve prescribing practices. BZDs are recommended primarily for short-term manage

The Use of Sexual Grooming by Attorneys in Court.

Child sexual abuse (CSA) cases rely on attorneys, judges, and jurors having an accurate understanding of complex abuse dynamics, including sexual grooming. While sexual grooming is widely used in research and legal contexts, little is known about how attorneys understand and use the construct. This study examined attorneys' knowledge, perceptions, and utilization of sexual grooming in CSA cases. Fifty-one attorneys from 23 U.S. states completed an online survey. Attorneys viewed sexual grooming as useful (M = 8.82/10), with 88% having used it in their cases. Around half (55%) used expert testi
