A 53-year-old man with chronic myelogenous leukemia developed progressive proximal muscle weakness with electromyographic and histologic features consistent with polymyositis. Although the association of polymyositis with solid tumors is well recognized, an association with hematologic malignancies has not been firmly established. A survey of the world medical literature reveals one previously reported case of polymyositis and one of dermatomyositis associated with chronic myelogenous leukemia. We conclude that polymyositis does occur in association with chronic myelogenous leukemia. Recogniti

Children with spastic hemiplegia secondary to cerebral palsy show disrupted patterns of work and power in gait. A computer-assisted feedback system was used to deliver EMG feedback from the triceps surae muscle group to walking subjects in conjunction with amplitude and timing targets for muscle relaxation and activation. Biofeedback of triceps surae muscle activity during gait was compared with physical therapy (PT) in a two-period crossover design, with intervening biomechanical gait analyses to assess the effects of each type of treatment. Kinematic and kinetic gait variables were affected

The effect of botulinum toxin A treatment was studied in patients with idiopathic torticollis. Twenty patients with idiopathic torticollis received electromyographically guided intramuscular botulinum toxin A into the hyperactive neck muscles. In all, 48 treatments were given. The injections were repeated with intervals of 11 to 35 weeks (mean 18.7 weeks). The grade of improvement was estimated subjectively using a visual analogue scale. An overall improvement of 55% compared with the status before treatment was found. The best result obtained in each individual patient varied from 20% to 84%

The electrophysiologic and histologic effects of the Bard Argon Beam Coagulator (ABC) were investigated in the New Zealand White rabbit. Thirty-four rabbits were divided into three groups. Controls underwent simple femoral exploration and closure. The remaining rabbits' femoral nerves were spot coagulated with either the ABC or standard electrosurgical unit (ESU). Stimulus thresholds were recorded before treatment and again prior to sacrifice at 0, 30, 60, or 120 days. Thresholds were significantly elevated for the ABC and ESU compared to controls (P = .0077 and .0351, respectively). Changes i

Trained weight lifters lift heavy loads without a concomitant degree of acute low-back injuries. To study the process by which large loads are lifted with minimal injury, integrated electromyographic signals were recorded from four large muscle groups: gluteus maximus, quadriceps, latissimus dorsi, and erector spinae in 4 weight lifters and 11 asymptomatic control subjects. These signals were recorded during a floor-to-knuckle-height isokinetic lift (dead lift) at 30.5 and 45.7 cm/sec. The signals were normalized for the height of the lift and the maximal isokinetic integrated electromyographi

After low transection of the spinal cord mammalian quadrupeds can be trained to walk on a driven surface indicating that coordinating neuronal circuits persist in the spinal cord segments caudal to the lesion. We trained 8 persons with incomplete spinal cord lesion on a Laufband (driven treadmill) for 1 1/2 to 7 months (5 days a week, 30-60 minutes daily) starting 5 to 20 months after injury and found significant improvement in the utilisation of the paralysed limbs during locomotion. Locomotion is described in one additional patient who had trained independently on parallel bars for several y

A patient with a 5 year history of slow-progressive Lambert-Eaton Myasthenic Syndrome (LEMS) was treated for a period of 12 months with 3,4-diaminopyridine (3,4-DAP). The therapy led to an objective increase in muscle power. During the treatment period, there was no increase in muscle weakness, but attempts at withdrawal of the drug confirmed a progression. The mouth dryness disappeared and autonomic regulation disturbances were improved. All of the laboratory parameters remained unchanged. A neoplasia was excluded by extensive endoscopic and radiological investigations. Side-effects included

A case study is reported regarding a 57-year-old woman, chose chief complaint was weakness in her thumb that she had noted while gardening. The patient described difficulty pulling weeds out because of an inability to get a firm grip when using the thumb. Physical examination showed weakness of the flexor pollicis longus and flexor digitorum profundus to the index finger. There was no other weakness and no clinical sensory deficit. Electrodiagnostic studies revealed normal median motor and sensory nerve conduction studies with needle examination abnormalities noted only in the flexor pollicis

We report a 25 year-old woman with delayed finger opening caused by a midbrain lesion. Magnetic resonance imaging revealed an abnormal intensity area in the left midbrain in the vascular territory corresponding to the mesencephalic inferomedian branch and central posteromedian branch of posterior cerebral artery. Clinically, she suffered from Weber's syndrome, and in the course of recovery, she developed marked delayed finger opening. Her weakness was slight, this delayed finger opening was observed only after maximal grip, and she required about ten seconds to open her right fingers. She coul

Five horses with Australian stringhalt were treated with 15 mg/kg phenytoin orally for 2 weeks. During the second week of the trial, 3 of the horses were given an additional dose of 10 mg/kg phenytoin. The response to treatment was clinically assessed by grading the severity of the gait abnormality at the walk, trot, turning and backing twice daily. There was a significant (P less than 0.05) improvement in the gait abnormality when pre-treatment values were compared with the mean of the last 3 assessments before treatment stopped. When reassessed 2 weeks after treatment ceased, there remained

Motor evoked potential (MEP) examination is a new method. Ipsilateral facial muscles are excited by magnetic stimulation over the scalp when the coil is placed on the parieto-occipital region. Using magnetic stimulator and EMG instrument (1500C4 DISA), one can measure the CMAPs from orbicularis oris and frontal muscle. In 30 normal subjects, the CMAPs were found to be similar in form but of longer latency than those obtained with electrical stimulation (ENoG). In 20 patients with facial nerve paralysis due to Bell's palsy, Hunt's syndrome or trauma, the latencies on the affected sides were app

Clinical symptoms and signs and the frequency of abnormalities in the results of enzymatic, electromyographic, histological and serological tests were analysed in 50 patients, including 17 with polymyositis (PM) and 33 with dermatomyositis (DM), groups I and II according to Bohan's classification and followed-up for 25 years. Weakness of the proximal muscles of the extremities was present in both groups in all patients and in a high proportion of cases weakness involved also the distal muscles. Dysphagia was more frequent in DM patients (54.5%) than in PM (17.6%). In DM erythema and facial oed

The Authors describe three cases of neuralgic amyotrophy examined clinically and neurophysiologically at the onset of symptoms and after six months. At that time patients showed an objective improvement in strength and functions, though a few SEP and EMG alterations persisted. These findings are considered of out discrepancy between clinical and neurophysiological picture.

The aim of the study is to investigate the motor unit abnormalities in late postpolio muscular atrophy (PPMA) as compared to those found in patients who had polio 20-30 years prior to examination without any new clinical signs. The quantitative concentric needle EMG and a single fiber EMG techniques were employed. Spontaneous activity, the parameters of individual motor units potentials (MUP), number of complex potentials and their stability, jitter and blocking as well as fiber density (FD) have been evaluated. In PPMA patients (5 subjects) we found in newly weakened muscles: spontaneous acti

A 22-year-old woman admitted to the hospital on Mar. 19, 1985 because of syncopal attacks and numbness on the limbs. Examination revealed peripheral sympathetic as well as parasympathetic dysfunctions and loss of myelinated as well as unmyelinated fibers in a biopsied sural nerve. A diagnosis of acute autonomic and sensory neuropathy (AASN) was made. She was treated with prednisolone but did not respond to the therapy. Only L-DOPS was effective on orthostatic hypotension. Recovery of the neurological conditions was insufficient till May 1987, when she complained of easy fatigability when speak

In the routine analysis of patients with tremors, we have the routine analysis of patients with tremors, we have encountered three patients with severe cerebellar postural & kinetic tremors. Surface EMG study revealed alternating activity in antagonistic muscles with low frequency (3-5 cps) and wide duration (over 125msecs). They were different to typical essential tremor & parkinson's tremor in the clinical manifestation and tremogram. The tremor did not respond to high dose of propranolol (120-160mg/day), but responded to high dose of isoniazid (800-1200mg/day). Patients were evaluated befor
