Fatal pediatric head injury due to toppled television: does the injury pattern overlap with abusive head trauma?

Pediatric head injuries can occur from abusive head trauma (AHT) or accidents. Accidental pediatric head injuries caused by cathode-ray tube televisions (CRTT) toppling have become a 'silent epidemic'. Differentiation between a fatal case of AHT and CRTT toppling could be vexing when the historical and scene evidence are subtle. A 2-year-old girl was found unresponsive in her house and could not be resuscitated. A 27" CRTT was found fallen from its stand onto the floor at the scene. The siblings report that the deceased was climbing on the television stand when it toppled. Autopsy revealed a s

Closed trauma, Mycoplasma hominis osteomyelitis, and the elusive diagnosis of Good's syndrome.

Mycoplasma hominis septic arthritis has a well-established association with hypogammaglobulinaemia, but is rarely seen in immunocompetent hosts. An association also exists with a closed trauma and a predisposition to M hominis bone infections. In this clinical case report, a patient with M hominis osteomyelitis following a closed trauma was diagnosed using 16S ribosomal studies, and led to the diagnosis of a severe underlying immunodeficiency syndrome known as Good's syndrome.

Does scaphoid bone bruising lead to occult fracture? A prospective study of 50 patients.

INTRODUCTION: Bone bruising of the scaphoid is a term reported when magnetic resonance imaging (MRI) is carried out for scaphoid injury. The aim of our study was twofold: to see if bone bruising alone without fracture of the scaphoid bone seen on initial MRI, in a clinically symptomatic (tender) patient at 10-14 days, progressed to fracture, and to define how this entity of bone bruising should be managed. METHODS: This was a prospective study looking at 170 patients with scaphoid injuries, of which 50 had bone bruising without fracture. These were followed up for at least 8 weeks to ascertai

Validation of a clinical score to predict skull fracture in head-injured infants.

OBJECTIVES: To validate a previously derived clinical score that uses clinical signs to determine which head-injured infants are at risk of skull fracture. The clinical score is calculated on the basis of the patient's age, the scalp hematoma size, and the location of the hematoma, with a total value between 0 and 8. METHODS: We performed a prospective observational study of children younger than 2 years with blunt head trauma presenting to an urban pediatric emergency department. Among subjects who had head imaging performed (validation set), we assessed the utility of our clinical score to

Hoffa-mimicking fracture of the tibial plateau.

We report the case of a teenage male who had a slip and fall from a motorcycle and sustained injury to his left knee. A clinicoradiological evaluation revealed a closed displaced osteochondral fragment of the posterolateral tibial plateau. The patient underwent open reduction and internal fixation with two cannulated headless screws. The patient recovered uneventfully on follow-up. The Hoffa-like displaced posterolateral tibial plateau fracture with no associated ligament damage is a rare injury. Further, there are very few reported cases on the surgical approach and fixation techniques in thi

Derotational Corrective Osteotomy for Pediatric Forearm Malunion: A Case Report.

In this case report, we describe an eight-year-old boy with both-bone forearm diaphyseal fracture that was treated conservatively after closed reduction with manipulation. Nine months after the injury, he returned to consultation, presenting a rotational deformity of the forearm and 20° of pronosupination limitation. He was submitted to corrective osteotomies, using three-dimensional (3D) planning and templating, using a double approach, and fixation with a four screw holes plate in each osteotomy. At one year of follow-up, there are signs of bone consolidation and complete recovery of range o

Modified Extension Block Technique for Bony Mallet Injury of the Hallux.

Bony mallet injury of the hallux is uncommon. In the few reports of this injury, authors have described surgical treatments such as closed reduction with percutaneous pinning and open surgical fixation with Kirschner wires or a suture anchor. However, the appropriate surgical management for this injury remains controversial. In this article, we describe a case of bony mallet injury of the hallux repaired with the modified extension block techniqueusing 3 Kirschner wires. This method is an effective and simple treatment to allow anatomic reduction of the displaced articular fracture fragment wi

Intramedullary nails in the treatment of the distal radius fractures.

The distal radius fracture is a common injury in old persons. Its treatment remains a critical challenge because of number of cases, the final cost of the procedure, the level of X-ray irradiation, and the demand of technique of fixation in osteoporotic bone. The entire closed procedure requires a strict percutaneous nailing. This offers advantages in terms of postoperative pain, per-operative irradiation, and cost. The main problem was so far the ability to insure a stable reduction in time. This point is discussed with the introduction of the "Nail-o-Flex®" nail. A continuous series of 83 pa

Anterior subtalar dislocation with comminuted fracture of the anterior calcaneal process.

Anterior subtalar dislocation is a very rare injury. We report a case of an 81-year-old woman who had her right foot injured during a motor vehicle accident. Radiographs showed anterior subtalar dislocation with comminuted fracture of the anterior calcaneal process. The dislocation was closely reduced and protected by a short leg cast. One year postinjury, the patient had only mild pain when walking on uneven ground. There was mild tenderness over the lateral heel. Subtalar motion was mildly painful. There was no pain with ankle motion.

Treatment of Nonunions After Malleolar Fractures.

Ankle fracture nonunion is a rare occurrence following closed or operative intervention. When it does occur, patients can experience debilitating symptoms that limit daily function. Malleolar nonunion can be caused by patient factors, such as smoking, malnutrition, or vascular insufficiency. Surgeon factors, such as insufficient or inappropriate operative fixation, also play a role. Several adjuncts, such as bone grafting, bone morphogenic protein, and bone stimulation, are useful in treating nonunions. Through a multimodal approach, malleolar nonunions are reliably treated with operative fixa

Surgical Correction of Cubitus Varus.

Cubitus varus can arise from distal humerus fractures in childhood as a result of malunion, nonunion, or overgrowth. Several types of distal humerus osteotomies have been described to treat this deformity, each with its own benefits, drawbacks, and complications. This article details the surgical technique and expected outcomes for 4 of the most commonly used types of distal humerus osteotomies in the treatment of cubitus varus. Specifically, we will describe the techniques for the lateral closing-wedge osteotomy, the step-cut osteotomy and its variations, the dome osteotomy and its variations

[Percutaneous stabilization of traumatic spondylolisthesis in a child].

A 12-year-old boy suffered a rare occurrence of a traumatic spondylolisthesis (L5/S1) without neurological alterations after being partially buried underneath a collapsing brick wall. Additionally he sustained a third degree open fracture of the left distal fibula and epiphysiolysis of the left distal tibia. A closed reduction and percutanous dorsal instrumentation L5/S1 as well as an open reduction and osteosynthesis of the tibia and fibula were performed. After 6 months the instrumentation was completely removed and an unrestrained range of motion of the lumbar spine and the upper ankle join

[Bilateral radiocarpal dislocation with fracture of the radial styloid. A case report].

INTRODUCTION: Radiocarpal dislocation is a rare injury and thus the number of cases described is limited. The course of the injury is favorable when the treatment provided is appropriate. CLINICAL CASE: A 21-year-old male patient who sustained radiocarpal dislocation and fracture of the radial styloid process in both wrists. He was treated with closed reduction and external fixation. The evaluation at 3 months showed favorable results. DISCUSSION: This case is one of the few similar cases reported because it is an uncommon pathology and it is even less frequent because the injury involves bo

Flexor tendon problems after volar plate fixation of distal radius fractures.

Four cases of flexor tendon problems which developed after volar plate fixation of distal radius fractures are presented. All cases were associated with close contact of the screws or distal edge of the plate with the flexor tendons. Poor bone stock or multiple bone fragments allowing loosening of the plate or non-locking screws cause the hardware to irritate the flexor tendons and ultimately lead to rupture. The flexor tendons involved include the flexor carpi radialis, flexor pollicis longus and flexor digitorum superficialis, and flexor digitorum profundus to the index and long fingers.

Complicated crown fracture of an unerupted permanent tooth--a case report.

Trauma to primary teeth may result in direct damage to underlying developing permanent teeth because of the close relationship that exists between the apices of the primary teeth and their permanent successors. Injuries to developing teeth have been classified into ten different categories, using a classification that is largely based on developmental alterations to the permanent teeth. However, this classification does not include other types of trauma that may occur to developing teeth, such as crown fractures. Although apparently rare, such injuries can occur by direct contact of the impact

Management of infected defect nonunion of the metacarpals.

A 30-year-old man sustained a displaced closed fracture of the fourth metacarpal bone, which was treated by open reduction and internal fixation with Kirschner wires. Severe postoperative infection led to a segmental defect with shortening of the fourth metacarpal bone and infected defect nonunion of the fifth metacarpal bone. After serial debridements and intravenous antibiotics, the infection was controlled. An AO external minifixator was applied to restore the length of the fourth metacarpal bone and to stabilize the fourth and fifth metacarpals, and iliac bone grafting was performed, leadi

Scapulothoracic dissociation: a case report.

Scapulothoracic dissociation has been described as a "closed traumatic forequarter amputation" and is characterized by massive soft tissue swelling of the shoulder, lateral displacement of the scapula, neurovascular injuries (brachial plexus, subclavian artery), an osseous-ligamentous injuries. In addition to the lateral displacement of the scapula, an acromioclavicular separation, a displaced fracture of the clavicle or sternoclavicular separation can be identified on plain X-rays. This injury pattern has been infrequently reported. We present a patient who had a previously unreported combina

Management of gunshot wounds to the tibia.

The management of tibial gunshot wounds is unique because of the close proximity of the medial aspect of the bone to the skin. The minimal soft-tissue sleeve covering the medial tibia allows bullets to enter medially and strike the bone with greater energy and allows surface contaminants to reach the bone readily unlike areas such as the thigh where soft tissue can dissipate much of the energy. The consequence of this anatomic difference may affect infection rates, fracture stability, and surgical options. For these reasons, tibial gunshot wounds should be treated independently from other guns

Dental implants placed in a patient with multiple myeloma: report of case.

A 69-year-old male, under active treatment for multiple myeloma, fractured his left maxillary second premolar palatal abutment tooth and requested treatment because his upper partial denture no longer fit. Several other teeth were deemed hopeless, and construction of a maxillary complete denture presented problems. There are general contraindications regarding dental implant surgery in the terminally ill patient; however, in this case, the patient wished to maximize his quality of life and opted to undergo implant surgery. With close management, the treatment was successful.
