Sinus node injury as a result of superior vena cava isolation during catheter ablation for atrial fibrillation and atrial flutter.

BACKGROUND: The ectopic foci originating from superior vena cava (SVC) may act as triggers in the occurrence and perpetuation of atrial fibrillation (AF). Unfortunately, the SVC isolation may result in potential dysfunction of sinus node. Furthermore, little is known about the occurrence of sinus node injury complicated by the SVC isolation. METHODS: Patients with AF or atypical atrial flutter experienced SVC isolation. The junctional rhythm or sinus arrest could be observed, if sinus node was injured. Atropin and dopamine administration ruled out the vagal irritation of sinus node, when junc

Late presentation of aorto-right ventricular fistula and associated aortic regurgitation following penetrating chest trauma.

We present a case in which a 39-year-old man was assaulted in 2002 and sustained penetrating chest trauma. He was treated conservatively, and presented six years later with exertional dyspnoea and a continuous murmur. Echocardiography detected an aorta-right ventricular fistula and an important aortic regurgitation. The aorta-right ventricular fistula was repaired and the aortic valve was replaced with a mechanical prosthesis. Traumatic aorto-right ventricular fistulas and aortic regurgitation are often masked by the primary injury and may take months or years to become clinically evident.

Spinal cord transection inhibits HR reduction in anesthetized rats immersed in an artificial CO2-hot spring bath.

Like humans, the heart rate (HR) of anesthetized rats immersed in CO(2)-water is lower than that when immersed in tap water at the same temperature. To investigate the afferent signal pathway in the mechanism of HR reduction, Wistar rats were anesthetized with urethane and then the spinal cord was transected between T(4) and T(5). The animals were immersed up to the axilla in a bathtub of tap-water (CO(2) contents: 10-20 mg.l(-1)) or of CO(2)-water (965-1,400 mg.l(-1)) at 35 degrees C while recording HR, arterial blood pressure, and arterial blood gas parameters (PaCO(2), PaO(2), pH). Arterial

[Phase analysis of gated blood pool scintigraphy in traumatic myocardial contusion].

It is not easy to make a diagnosis of myocardial contusion following blunt chest trauma, because most patients have many other concurrent injuries with diverse symptoms. The usefulness of phase analysis of gated blood pool scintigraphy (GBPS) for myocardial contusion following blunt chest trauma was evaluated. Thirty-eight patients who had been strongly suspected of having myocardial contusion from clinical symptoms and electrocardiograms underwent phase analysis of GBPS. The results of phase analysis were compared with those of two-dimensional echocardiography (2-D Echo) and CPK-MB fraction m

Lethal myocardial infarction subsequent to compression of the left anterior descending coronary artery induced by traumatic hematoma.

Post-traumatic myocardial infarction is a well-known although rare complication of blunt chest traumas. In cases of proximally located damage to the coronary arteries, modern vascular surgery such as bypass operations may be lifesaving. In this paper we describe a case, where a healthy 35-year-old man developed a lethal myocardial infarction 8 days after a chest trauma caused by a moped. The ECG obtained 4 h after the accident showed extensive transmural ischemia of the anterior wall. At autopsy a hematoma originating from a side branch was found to compress the left anterior descending corona

A pediatric survivor of left ventricular rupture after blunt chest trauma.

The case of a patient with left ventricular rupture as a result of blunt chest trauma from a motor vehicle accident is presented. This is the first reported case of a pediatric survivor of this rare and usually lethal injury and only the fourth report of a survivor. This condition can be treated if the patient survives long enough to reach a trauma center, if the diagnosis is entertained early, and if the appropriate diagnostic studies are obtained in an expeditious manner. We believe that echocardiography is the diagnostic method of choice for pericardial tamponade, whether it is the result o

Targeting E3 Ubiquitin Ligase Hrd1 Prevents Myocardial Ischemia-Reperfusion Injury Through Enhancing ALDH2 Enzymatic Activity.

BACKGROUND: Myocardial ischemia-reperfusion (I/R) injury presents a significant clinical challenge characterized by a complex pathological mechanism. The role of protein ubiquitination in I/R injury has not been systematically investigated. Global ubiquitinome profiling was conducted to identify the potential key players in myocardial I/R injury. METHODS: The ubiquitination levels of proteins in mouse hearts subjected to either sham surgery or I/R injury were analyzed using ubiquitinome. A combined analysis of ubiquitinome, single-cell RNA sequencing (RNA-seq), and proteomics data was employe

Radiation-Induced Coronary Artery Disease in Lung and Breast Cancer Patients: Insights from PET Imaging and Long-Term Risk Assessment.

Radiation-induced coronary artery disease (RI-CAD) is a significant cardiovascular complication for cancer survivors treated with thoracic radiation therapy (RT). Despite advances in RT techniques, exposure to the heart during treatment remains a critical factor influencing long-term cardiac outcomes, particularly in patients with breast and lung cancer. RI-CAD develops due to radiation-induced endothelial injury, inflammation, and accelerated atherosclerosis, presenting a unique and aggressive disease profile. This review explores the pathophysiology, risk factors, and diagnostic advancements

Congestive Hepatopathy: A Review of the Literature.

Congestive hepatopathy (CH), stemming from compromised hepatic venous flow or heightened intrahepatic pressure, represents a significant consequence of cardiovascular conditions like congestive heart failure (CHF). This review of literature encapsulates the core aspects of this condition, characterized by hepatic congestion, cellular injury, and impaired liver function. Diagnostic challenges arise due to symptoms mirroring primary liver diseases. Management revolves around addressing the underlying cause and mitigating fluid retention. This review of literature provides a snapshot of CH's comp

Hypertonic Saline Administration via Intraosseous Access During Symptomatic Hyponatremia.

Hyponatremia is a common lab finding. Symptomatology varies greatly and can depend on the degree of hyponatremia and its chronicity. Causes of hyponatremia are also vast and include heart failure, renal injury, liver disease, and gastrointestinal losses, or it can be induced by medication. Treatment depends on the suspected etiology. However, in life-threatening conditions such as seizures or coma, urgent 3% saline is required. Administration of 3% saline is usually through peripheral and central IV access. This case report highlights an alternative route in administering 3% saline, intraosseo

Potential Adjuncts to Therapeutic Hypothermia to Mitigate Multiorgan Injury in Perinatal Hypoxia-Ischemia.

Over the last 2 decades, therapeutic hypothermia has become the standard of care to reduce morbidity and mortality in neonates affected by moderate-to-severe hypoxic-ischemic encephalopathy (HIE). There is a significant interest in improving the neurologic outcomes of neonatal HIE, ranging from adjunctive therapy to therapeutic hypothermia. Importantly, the pathophysiologic mechanisms underlying HIE also affect multiple other organs, contributing to high morbidity and mortality in this patient population. This review focuses on the adjunct therapies currently under investigation to mitigate th

A Case of Spontaneous Perinephric Hematoma From Rivaroxaban.

Spontaneous perinephric hematoma is an uncommon but serious condition that is typically caused by tumor hemorrhage or vascular disease such as polyarteritis nodosa. We describe a 78-year-old Chinese gentleman with underlying chronic kidney disease, renal cysts, non-obstructive renal stones, hypertension, ischemic heart disease, and atrial fibrillation who was on rivaroxaban and clopidogrel. He developed spontaneous perinephric hematoma complicated by acute kidney injury, anemia, and myocardial infarction who underwent angioembolization and packed cell transfusion. He did not have further bleed

The Link Between Ferroptosis and Cardiovascular Diseases: A Novel Target for Treatment.

Ferroptosis is an iron-dependent cell death, which is characterized by iron overload and lipid peroxidation. Ferroptosis is distinct from apoptosis, necroptosis, autophagy, and other types of cell death in morphology and function. Ferroptosis is regulated by a variety of factors and controlled by several mechanisms, including mitochondrial activity and metabolism of iron, lipid, and amino acids. Accumulating evidence shows that ferroptosis is closely related to a majority of cardiovascular diseases (CVDs), including cardiomyopathy, myocardial infarction, ischemia/reperfusion injury, heart fail

A giant left ventricular pseudoaneurysm diagnosed after double-valve replacement.

A diagnostic coronary catheter injury to the subaortic region in a 41-year-old woman with rheumatic heart disease led to a pseudoaneurysm that later caused extrinsic left coronary compression. She subsequently underwent double-valve replacement, overlooking the pseudoaneurysm that enlarged to a giant size three months later following thrombolysis for mitral prosthesis thrombosis. A thrombolysis-induced large intracerebral hemorrhage posed a significant risk for reoperation, and mechanical prosthetic valves in the aortic and mitral positions allowed a catheter option only, through percutaneous

A care pathway for the cardiovascular complications of COVID-19: Insights from an institutional response.

The infection caused by severe acute respiratory syndrome coronavirus-2, or COVID-19, can result in myocardial injury, heart failure, and arrhythmias. In addition to the viral infection itself, investigational therapies for the infection can interact with the cardiovascular system. As cardiologists and cardiovascular service lines will be heavily involved in the care of patients with COVID-19, our division organized an approach to manage these complications, attempting to balance resource utilization and risk to personnel with optimal cardiovascular care. The model presented can provide a fram

Ischemic Model of Heart Failure in Rats and Mice.

Temporary or permanent left coronary artery (LCA) ligation is the most widely used model of heart failure. In the present protocol, we describe the materials necessary for the procedure, key steps of the LCA ligation, triphenyl tetrazolium chloride (TTC) staining, and calculation of myocardial infarction (MI) size after ischemia-reperfusion (I/R) injury (30 min/24 h) in rats and mice. We discuss precautions and tips regarding the operation before and after surgery, both in vivo and ex vivo. The aim of this chapter is to describe the details of LCA surgery and provide recommendations for curren
