Hemodynamic Improvement in Acute Respiratory Distress Syndrome Patients After Venovenous Extracorporeal Membrane Oxygenation Implantation.

OBJECTIVE: Severe acute respiratory distress syndrome (ARDS) is often complicated by hemodynamic instability requiring pharmacological support. Venovenous extracorporeal membrane oxygenation (VV ECMO) is a well-established technique that contributes to improved outcomes in this population. However, the effects of VV ECMO on inotropic and vasoconstrictor requirements have never been addressed in a large case series. DESIGN: Observational study. SETTING: University hospital. PARTICIPANTS: Consecutive adult ARDS patients treated with VV ECMO. MEASUREMENTS AND MAIN RESULTS: From June 2009 to O

Hemodynamic effects of levosimendan in low cardiac output syndrome post-CABG refractory to standard inotropic therapy.

BACKGROUND: Low cardiac output syndrome (LCOS) following coronary artery bypass grafting (CABG) remains a major cause of morbidity and mortality. Standard inotropes frequently provide only marginal hemodynamic benefit and are associated with increased myocardial oxygen consumption and arrhythmogenic risk. Levosimendan, a calcium sensitizer with inodilatory properties, may offer more favorable hemodynamics without these adverse effects. METHODS: In this retrospective study, we evaluated 41 patients with LCOS post-CABG who failed to respond to standard catecholamines, defined by < 20% improveme…

Hemodynamic investigations on the portal hypertension and treatment of transjugular intrahepatic portosystemic shunt (TIPS) based on CFD simulation.

Hemodynamic processes from the portal vein(PV) to the inferior vena cava(IVC) were mimicked for three patients with portal hypertension(PH) … omes. Energy loss of the TIPS stents and distributary effects of the PV bifurcation dominate the PV hemodynamics and the TIPS outcomes. For stents with large diameter or proper placement, the energy loss is low therefore the PV pressure reduction and stent shunt rate are high. Stents inserted on the left and right branches of the PV are able to utilize distrib …

Perinatal risk factors for hemodynamically significant patent ductus arteriosus in very low birth weight infants.

BACKGROUND: Multiple perinatal factors influence hemodynamically significant patent ductus arteriosus (HS PDA) in preterm infants. PURPOSE: This study aimed to identify the risk factors associated with HS PDA in very low birth weight infants (VLBWIs) and determine the predictors of surgical ligation. METHODS: This retrospective study included VLBWIs born at 23-32 weeks' gestation whose HS PDA properties could be identified using echocardiography. The infants were stratified into 2 groups based on gestational age (23-27 and 28-32 weeks). RESULTS: Among the 496 included VLBWIs, 171 had no PDA

Lactate orchestrates metabolic hemodynamic adaptations through a unique combination of venocontraction, artery relaxation, and positive inotropy.

AIM: H+ facilitates metabolic blood flow regulation while negatively impacting cardiac contractility. Cardiovascular consequences of conjugate bases accumulating alongside H+ remain unclear. Here, we evaluate the cardiovascular effects of nine prominent carboxylates-particularly lactate, 3-hydroxybutyrate, and butyrate-linked to metabolic and microbial activity. METHODS: Comparing the actions of pH-adjusted Na-carboxylates to equiosmolar NaCl, we study arteries and veins isolated from healthy rats and humans with ischaemic heart disease, isolated perfused rat hearts, and rat cardiovascular fu

"Bedside" percutaneous device closure of hemodynamically significant ductus arteriosus in an extreme low birth weight preterm.

Percutaneous device closure is fast gaining ground in the field of preterm ductus arteriosus intervention, post the introduction of specialized devices. However, percutaneous device closure in a preterm duct without fluoroscopic assistance has not been reported in India. We report the successful "bedside" Amplatzer PiccoloTM device closure of a hemodynamically significant ductus in an extremely low birth weight baby using only echocardiographic and portable X-ray assistance.

Cardiac Complications in Women With Hypertensive Disorders of Pregnancy Undergoing a Cesarean Section: Role of Intraoperative Hemodynamic Monitoring.

Introduction Hypertensive disorders of pregnancy (HDP) are major contributors to maternal morbidity and mortality, predisposing to significant cardiac complications during cesarean section. Hemodynamic fluctuations under anesthesia further increase the risk of myocardial ischemia, arrhythmia, and pulmonary edema. Continuous intraoperative monitoring may help identify early hemodynamic instability and improve maternal outcomes. Objective This study aimed to evaluate the frequency, pattern, and predictors of maternal cardiac complications in women with HDP undergoing cesarean section and to dete

Short-term reduction in sleep irregularity modulates cardiac autonomic function and central hemodynamic parameters at rest.

Fluctuation in sleep/wake timing, often referred to as sleep irregularity, is associated with elevated risk for cardiovascular disease. We sought to determine the effect of reducing sleep irregularity on cardiac autonomic function and hemodynamic parameters. Sixteen adults (35 ± 10 years) were randomly assigned to 12 nights of regular sleep timing or habitual sleep (control) completed in a crossover fashion. Sleep was assessed using wrist actigraphy. Laboratory testing took place the morning after each 12-night period. Short-term EKG recordings were used to evaluate heart rate variability (HRV

Clinical Outcomes of Different Angioembolization Techniques and Materials for Hemodynamically Unstable Pelvic Fractures: An Overlap Weighted Study.

BACKGROUND: Internal iliac artery embolization (IIAE) is a critical hemostatic intervention for controlling hemorrhage in patients with hemodynamically unstable pelvic fractures (HUPF). However, the optimal technique-nonselective versus selective embolization-and the choice of embolic materials remain debated. This study compares the efficacy, safety, and clinical outcomes of nonselective embolization (NSE) and selective embolization (SE) techniques in patients with HUPF. A secondary analysis examined the impact of different embolic materials on patient outcomes. METHODS: This multicenter ret

Is the Corrected Carotid Flow Time a Clinically Acceptable Surrogate Hemodynamic Parameter for the Left Ventricular Ejection Time?

OBJECTIVE: The corrected left ventricular ejection time (cLVET) comprises the phase from aortic valve opening to aortic valve closure corrected for heart rate. As a surrogate measure for cLVET, the corrected carotid flow time (ccFT) has been proposed in previous research. The aim of this study was to assess the clinical agreement between cLVET and ccFT in a dynamic clinical setting. METHODS: Twenty-five patients with severe aortic valve stenosis (AS) were selected for transcatheter aortic valve replacement (TAVR). The cLVET and ccFT were derived from the left ventricular outflow tract (LVOT)

Natural course of hemodynamically stable hemispheres contralateral to operated hemispheres in adult patients with ischemic moyamoya diseases.

The necessity of bilateral bypass in adult moyamoya disease (MMD) remains unclear despite its recommendation for pediatric and hemorrhagic cases. We aimed to investigate the natural course of hemodynamically stable unoperated hemispheres after bypass surgery for symptomatic and hemodynamically unstable hemispheres in adult patients with ischemic MMD. Among 288 patients, the mean age at the first operation of the unstable hemispheres was 40.8 ± 12.2 years. The mean follow-up period was 62.9 ± 46.5 months. 45 patients (15.6%) experienced stroke events in the unoperated hemisphere, consisting of

Is initial unilateral revascularization acceptable in pediatric patients with bilateral moyamoya disease with mild contralateral hemodynamic disturbance?

OBJECTIVE: Although asymmetrical vascular involvement between hemispheres is common in pediatric patients with bilateral moyamoya disease, whether hemispheres with mild vascular changes and hemodynamic impairment require immediate surgical revascularization or whether they can be observed until disease progression remains unclear. The authors evaluated the long-term outcomes of their strategy to initially perform unilateral surgery and withhold surgery to the contralateral hemispheres with mild vascular changes and hemodynamic impairment. METHODS: The authors retrospectively evaluated Japanes
