[The eye as a mirror of the heart-retinal findings in heart failure and cardiovascular diseases].

BACKGROUND: Due to its unique anatomical and functional characteristics, the retina represents a promising organ for the evaluation of systemic vascular diseases. Modern imaging techniques, particularly optical coherence tomography angiography (OCT-A), enable noninvasive and quantitative assessment of retinal microcirculation. The aim of this review is to present current findings on the significance of retinal biomarkers in cardiovascular diseases, with a focus on heart failure. RESULTS: Numerous studies show associations between retinal vascular changes and arterial hypertension, coronary he

Lard Intake Reduced Tetralinoleoyl Cardiolipin and Attenuated the Cardioprotective Effects of Exercise in Mice With Pressure Overload-Induced Heart Failure.

BACKGROUND: Exercise-based cardiac rehabilitation is effective for improving the prognosis of patients with heart failure (HF), but little is known about its interactions with dietary components. We investigated the impacts of dietary fat intake and its source on the cardioprotective effects of exercise in mice with HF. METHODS AND RESULTS: Endurance exercise and diets with different fat composition, including a normal diet (NE: 11% fat), a soybean oil-rich diet (SE: 32% fat) and a lard-rich diet (LE: 32% fat), were provided for 4 weeks to mice with HF induced by transverse aortic constrictio

Paradoxical improvement of mitral regurgitation associated with hyperadrenocorticism-related cardiac hypertrophy in a dog with myxomatous mitral valve disease.

In this case report, we describe an atypical clinical course in a dog with preexisting myxomatous mitral valve disease (MMVD) subsequently complicated by the development of hyperadrenocorticism. Typically, hyperadrenocorticism can exacerbate cardiac conditions; however, this dog demonstrated paradoxical improvement. A retrospective analysis of the clinical, radiographic, echocardiographic, and hormonal data was conducted to investigate the marked reduction in cardiac size in a Maltese dog with MMVD following the diagnosis of hyperadrenocorticism. A 10-year-old spayed female Maltese dog present

Apical Hypertrophic Cardiomyopathy: A Rare and Hidden Cause of Stroke.

Hypertrophic cardiomyopathy (HCM) is an important but under-recognised cause of cardioembolic stroke, due to impaired myocardial contraction, leading to a region of blood stasis and thus associated thrombus formation. Stroke remains the second leading cause of death worldwide, with increasing incidence even among patients without conventional vascular risk factors and those younger in age. Apical hypertrophic cardiomyopathy (ApHCM), characterised by hypertrophy of the left ventricular apex, is associated with a higher risk of stroke than other HCM variants due to apical cavity obliteration, im

Mitral Annular Calcification: Understanding the Disease and Treatment Options.

Mitral annular calcification (MAC) is a degenerative process that causes calcium deposits along the mitral annulus. The pathophysiological mechanisms contributing to the development of MAC are not fully understood. The main risk factors for MAC are advanced age, female gender, chronic kidney disease, and conditions predisposing to left ventricular hypertrophy, which increases mitral annulus tension and subsequent annular degeneration. The prevalence of MAC varies widely among studies, from 5% to 42%, depending on the characteristics of the study population and the imaging modality used to make

Early diagnosis of cardiac involvement in Anderson-Fabry disease using cardiac MRI parameters.

Anderson-Fabry disease (AFD) is a rare, X-linked, lysosomal storage disorder caused by the absence or deficiency of alpha-galactosidase A activity, leading to the progressive accumulation of sphingolipids in multiple organ systems, including the heart. Cardiac involvement accounts for > 50% of AFD-related mortality and is a primary determinant of disease prognosis. AFD cardiomyopathy is heterogenous with key features that include left ventricular hypertrophy, conduction disturbances, myocardial fibrosis and valvular disease. Early diagnosis and treatment are crucial to prevent progressive and

An analysis regarding the article "Artificial intelligence-enhanced electrocardiogram for the diagnosis of cardiac amyloidosis: A systemic review and meta-analysis".

Cardiac Amyloidosis (CA) occurs when misfolded proteins accumulate in the heart muscle, leading to restrictive cardiomyopathy and possibly escalating to heart failure, impaired conduction system function, and sudden cardiac arrest. It is a significant clinical challenge due to its high rates of underdiagnosis and misdiagnosis. Research indicates that about 35% of individuals with CA have been incorrectly diagnosed with other prevalent cardiovascular diseases. CA is differentiated into various subtypes depending on the protein involved in its pathogenesis, with Transthyretin Amyloidosis (ATTR)

The sympathetic nervous system in heart failure revisited.

Several attempts have been made, by the scientific community, to develop a unifying hypothesis that explains the clinical syndrome of heart failure (HF). The currently widely accepted neurohormonal model has substituted the cardiorenal and the cardiocirculatory models, which focused on salt-water retention and low cardiac output/peripheral vasoconstriction, respectively. According to the neurohormonal model, HF with eccentric left ventricular (LV) hypertrophy (LVH) (systolic HF or HF with reduced LV ejection fraction [LVEF] or HFrEF) develops and progresses because endogenous neurohormonal sys

Targeting mAKAPβ expression as a therapeutic approach for ischemic cardiomyopathy.

Ischemic cardiomyopathy is a leading cause of death and an unmet clinical need. Adeno-associated virus (AAV) gene-based therapies hold great promise for treating and preventing heart failure. Previously we showed that muscle A-kinase Anchoring Protein β (mAKAPβ, AKAP6β), a scaffold protein that organizes perinuclear signalosomes in the cardiomyocyte, is a critical regulator of pathological cardiac hypertrophy. Here, we show that inhibition of mAKAPβ expression in stressed adult cardiomyocytes in vitro was cardioprotective, while conditional cardiomyocyte-specific mAKAP gene deletion in mice pr

Npt2a as a target for treating hyperphosphatemia.

Hyperphosphatemia results from an imbalance in phosphate (Pi) homeostasis. In patients with and without reduced kidney function, hyperphosphatemia is associated with cardiovascular complications. The current mainstays in the management of hyperphosphatemia are oral Pi binder and dietary Pi restriction. Although these options are employed in patients with chronic kidney disease (CKD), they seem inadequate to correct elevated plasma Pi levels. In addition, a paradoxical increase in expression of intestinal Pi transporter and uptake may occur. Recently, studies in rodents targeting the renal Na+/

Oxidative Stress-Related Susceptibility to Aneurysm in Marfan's Syndrome.

The involvement of highly reactive oxygen-derived free radicals (ROS) in the genesis and progression of various cardiovascular diseases, including arrhythmias, aortic dilatation, aortic dissection, left ventricular hypertrophy, coronary arterial disease and congestive heart failure, is well-established. It has also been suggested that ROS may play a role in aortic aneurysm formation in patients with Marfan's syndrome (MFS). This syndrome is a multisystem disorder with manifestations including cardiovascular, skeletal, pulmonary and ocular systems, however, aortic aneurysm and dissection are st

Hyperuricaemia and gout in cardiovascular, metabolic and kidney disease.

During the last century, there has been an increasing prevalence of hyperuricaemia noted in many populations. While uric acid is usually discussed in the context of gout, hyperuricaemia is also associated with hypertension, chronic kidney disease, hypertriglyceridaemia, obesity, atherosclerotic heart disease, metabolic syndrome, and type 2 diabetes. Here we review the connection between hyperuricaemia and cardiovascular, kidney and metabolic diseases. Contrary to the popular view that uric acid is an inert metabolite of purine metabolism, recent studies suggest serum uric acid may have a varie

Phenotypic Characterization of a Family With An In-frame Deletion in the DMD Gene and Variable Penetrance.

Duchenne muscular dystrophy is a disorder with variable expression caused by framedisrupting mutations in the dystrophin gene. It is characterized by progressive muscle weakness and dilated cardiomyopathy. In-frame dystrophin mutations cause a clinically moderate disorder named Becker muscular dystrophy. Our aim was to study the clinical and genetic characteristics of a family with inherited cardiomyopathy and Becker muscular dystrophy. The index case was diagnosed with psychomotor retardation at 5 years of age. Asymmetric left ventricular hypertrophy and a long QT interval were evidenced at t

Hypertension and cardiovascular risk: General aspects.

Hypertension is the strongest or one of the strongest risk factors for almost all different cardiovascular diseases acquired during life, including coronary disease, left ventricular hypertrophy and valvular heart diseases, cardiac arrhythmias including atrial fibrillation, cerebral stroke and renal failure. The continuous relationship between blood pressure and cardiovascular and renal events makes the distinction between high normal blood pressure and hypertension based on arbitrary cut-off values for blood pressures. Overall the prevalence of hypertension in different European countries app

Effects of valproic acid on sympathetic activity and left ventricularmyocardial remodelling in rats during pressure overload.

Background/aim: Pressure overload induces cardiac remodelling and results in heart failure. Enhanced sympathetic outflow participates in the development of cardiac remodelling for the duration of pressure overload as an independent factor. Valproic acid has recently been shown to reduce neuronal injury and have antiinflammatory and antiapoptotic effects as a histone deacetylase inhibitor. We speculate that the drug plays a specific role in alleviating cardiac remodelling by inhibiting sympathetic activity. Materials and methods: Surgery of partial abdominal aortic constriction was performed on

Cardiovascular magnetic resonance imaging in hypertrophic cardiomyopathy: Current state of the art.

Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiomyopathy with a prevalence of 1:500 (0.2%) in the general population. Sudden cardiac death (SCD) is the most feared presentation of HCM. Therefore, it is essential to identify individuals at high risk in order to prevent SCD. The absence of conventional risk factors does not nullify the risk of HCM related SCD. Although echocardiography is currently the most widely used imaging modality, cardiac magnetic resonance (CMR) allows detailed characterization of the HCM phenotype, which makes it possible to differentiate HCM from othe

Endocardial Remodeling in Heart Failure Patients with Impaired and Preserved Left Ventricular Systolic Function--A Magnetic Resonance Image Study.

Left ventricular (LV) trabeculation has been studied in certain forms of cardiomyopathy. However, the changes of LV endocardial trabeculation during the remodeling process leading to heart failure (HF) are unclear. Seventy-four patients with systolic heart failure (SHF), 65 with heart failure with preserved ejection fraction (HFpEF) and 61 without HF were prospectively enrolled. All subjects received magnetic resonance imaging (MRI) study including cine, T1 and late gadolinium enhancement (LGE) images. Cardiomyopathies and the Armed Forces.

Cardiomyopathies are a group of heterogeneous myocardial diseases that are frequently inherited and are a recognised cause of premature sudden cardiac death in young individuals. Incomplete expressions of disease and the overlap with the physiological cardiac manifestations of regular intensive exercise create diagnostic challenges in young athletes and military recruits. Early identification is important because sudden death in the absence of prodromal symptoms is a common presentation, and there are several therapeutic strategies to minimise this risk. This paper examines the classification

Effects of mtDNA in SHR-mtF344 versus SHR conplastic strains on reduced OXPHOS enzyme levels, insulin resistance, cardiac hypertrophy, and systolic dysfunction.

Common inbred strains of the laboratory rat can be divided into four major mitochondrial DNA (mtDNA) haplotype groups represented by the BN, F344, LEW, and SHR strains. In the current study, we investigated the metabolic and hemodynamic effects of the SHR vs. F344 mtDNA by comparing the SHR vs. SHR-mt(F344) conplastic strains that are genetically identical except for their mitochondrial genomes. Altogether 13 amino acid substitutions in protein coding genes, seven single nucleotide polymorphisms in tRNA genes, and 12 single nucleotide changes in rRNA genes were detected in F344 mtDNA compared
