Whole-body hypothermia in late preterm and early term infants: a retrospective analysis from a neurocritical care unit.

BACKGROUND: To compare the outcomes of whole-body hypothermia (WBH) in infants with neonatal encephalopathy born at 340/7-356/7 versus 360/7-376/7 weeks' gestation. METHODS: Retrospective analysis of 122 outborn infants in a single unit: Group 1 (n = 63; 340/7-6/7 weeks n = 17, 350/7-6/7 weeks n = 46) and Group 2 (n = 59; 360/7-6/7 weeks n = 25, 370/7-6/7 weeks n = 34). Clinical, electrographic, neuroimaging, neurodevelopmental data at 18 months were assessed. RESULTS: Group 1 had more hemodynamic instability (67% vs. 32%, p < 0.001), hypoglycemia (54% vs 36%, p = 0.04), and higher white-mat

Opioids for neonates receiving mechanical ventilation.

BACKGROUND: Mechanical ventilation is a potentially painful and discomforting intervention widely used in neonatal intensive care units. Newborn babies (neonates) demonstrate increased sensitivity to pain, which may affect clinical and neurodevelopmental outcomes. The use of drugs that reduce pain might be important in improving survival and neurodevelopmental outcomes. OBJECTIVES: To determine the effect of opioid analgesics (pain-killing drugs derived from opium e.g. morphine), compared to placebo, no drug, or other non-opioid analgesics or sedatives, on pain, duration of mechanical ventila

Prospective audit of perinatal mortality among inborn babies in a tertiary health center in Lagos, Nigeria.

BACKGROUND: The perinatal mortality rate remains an important indicator of maternal care and maternal health and nutrition, and also reflects the quality of obstetric and pediatric care available. The causes of most of the perinatal deaths are preventable, thus making it important to identify the risk factors in each health environment. OBJECTIVE: The aim was to prospectively audit the perinatal mortality and associated risk factors in a tertiary health facility in a developing country. MATERIALS AND METHODS: Data for all consecutive deliveries in the labor ward complex of Lagos University T

Improvements in nutritional management as a determinant of reduced mortality from community-acquired lower respiratory tract infection in hospitalized children from rural central Africa.

BACKGROUND: In-hospital mortality from lower respiratory tract infections (LTRI) is unacceptably high in developing countries where LTRI are still a leading cause of death. OBJECTIVE: To identify new approaches to reduce in-hospital mortality of LRTI through the improvement of its management. METHODS: The prospectively collected database of children admitted during an 11-year period with LRTI in a pediatric rural hospital in Central Africa was reviewed to determine the predictors of death and to evaluate the impact on mortality of 4 different protocols for the management of malnutrition. Health of Hmong in Thailand: risk factors, morbidity and mortality in comparison with other ethnic groups.

Demographic and health conditions among Hmong in Thailand were examined in comparison with other ethnic groups which closely resemble the ethnic origins of Southeast Asian refugees in the U.S. Thailand Hmong have very large extended family households, very high birth rates, low use of contraception, very young age at first marriage, and, compared with other highland minorities, relatively low infant and crude mortality rates. Hmong use of tobacco and alcohol and other stimulants is lower than other ethnic groups, and is much more frequent among men than among women. Opium was used by 15% of th

Early anemia and neonatal morbidity in extremely low birth-weight preterm infants.

BACKGROUND: Extremely low birth-weight (ELBW) preterm infants remain at high risk for mortality and major morbidities, and nearly all need packed red blood cell transfusions within their first weeks of life. The overall objective of this study was to assess the association between anemia at admission to neonatal intensive care unit (NICU) with the neonatal morbidity and mortality in ELBW infants. METHODS: This retrospective study was conducted on 106 patients with ELBW admitted at our level III NICU from January 2006 to December 2015. The subjects were divided into two groups: (1) patients wi

Early treatment versus expectative management of patent ductus arteriosus in preterm infants: a multicentre, randomised, non-inferiority trial in Europe (BeNeDuctus trial).

BACKGROUND: Much controversy exists about the optimal management of a patent ductus arteriosus (PDA) in preterm infants, especially in those born at a gestational age (GA) less than 28 weeks. No causal relationship has been proven between a (haemodynamically significant) PDA and neonatal complications related to pulmonary hyperperfusion and/or systemic hypoperfusion. Although studies show conflicting results, a common understanding is that medical or surgical treatment of a PDA does not seem to reduce the risk of major neonatal morbidities and mortality. As the PDA might have closed spontaneou

Outcomes of multistage palliation of infants with functional single ventricle and heterotaxy syndrome.

BACKGROUND: Management of infants with heterotaxy syndrome and functional single ventricle is complicated due to associated cardiac and extracardiac anomalies. We report current era palliation results. METHODS: Between 2002 and 2012, 67 infants with heterotaxy syndrome underwent multistage palliation. Competing risks analyses modeled events after surgery (death vs Glenn procedure) and examined factors associated with survival. In addition, early and late outcomes following first-stage palliation surgery were compared with a matched contemporaneous control group of patients with nonheterotaxy

Intramuscular penicillin for the prevention of early onset group B streptococcal infection in newborn infants.

BACKGROUND: Early-onset group B streptococcal disease (EOGBSD) is the most frequent cause of serious infection in the newborn period. Current strategies used to prevent EOGBSD are focused upon maternal antibiotic prophylaxis to reduce transmission of GBS to the infant. Observational studies have suggested that the administration of intramuscular penicillin to the newborn immediately following delivery may be an effective strategy to reduce the incidence of EOGBSD. OBJECTIVES: To determine if the administration of intramuscular penicillin to newborns at birth is a safe and effective method to

Evolution of infant nutrition.

Homo sapiens has developed during the course of over two million years. The social and physical conditions of life, the availability of milk and infant foods as well as the presence of diseases have all undergone radical transformations from the Stone Age, at first without and then with fire, to the hunter-gatherer, farmer-herder, agricultural and, now, developed societies. These changes in the human environment may have induced modifications in the length of pregnancy, the development of the neonate at birth, the duration of lactation, the composition of breast milk and use of weaning foods a

Video vs. traditional laryngoscopy for tracheal intubation at birth or in the neonatal unit: A systematic review and meta-analysis.

BACKGROUND: Videolaryngoscopy may increase neonatal intubation success when used by neonatologists and anesthesiologists. It is not known if this is true for intubations by neonatal clinicians only in neonatal units or on delivery suites. OBJECTIVE: To critically appraise evidence on the success of tracheal intubation with video laryngoscopy compared to traditional laryngoscopy in infants at birth or in a neonatal unit. DATA SOURCES: Systematic review of studies identified by MEDLINE, Embase, Cochrane Library, CINAHL and Clinical Trial Databases searched from inception to August 22, 2024, wi

The Characterization of Postoperative Mechanical Respiratory Requirement in Neonates and Infants Undergoing Cardiac Surgery on Cardiopulmonary Bypass in a Single Tertiary Institution.

OBJECTIVES: Although neonates and infants undergoing cardiac surgery on cardiopulmonary bypass (CPB) are at high risk of developing perioperative morbidity and mortality, including lung injury, the intraoperative profile of lung injury in this cohort is not well-described. Given that the postoperative course of patients in the pediatric cardiac surgical arena has become increasingly expedited, the objective of this study was to characterize the profiles of postoperative mechanical ventilatory support in neonates and infants undergoing cardiac surgery on CPB and to examine the characteristics o
