Hantaviruses are a diverse and widespread group of zoonotic viruses under the Orthohantavirus genus that can cause severe acute cardiopulmonary or renal disease. Hantavirus species are grouped generally as either New World or Old World based on geographic distribution, which is influenced by their respective host reservoir including rodents. Although there is no specific treatment for hantaviruses, recognizing clinical presentation and interpreting laboratory results for case identification can improve patient management. Here we review the epidemiology, clinical manifestations, and diagnostic

The clinical association between Coxsackievirus B (CVB) and methicillin-sensitive Staphylococcus aureus (MSSA) has not been well established in the current literature. Here, we report a case of a 29-year-old male who presented with fever and malaise 24 hours after noticing a pruritic lesion on the anterior foreleg that resembled a mosquito bite. After multiple ED visits, laboratory studies, and imaging tests, the patient was admitted for treatment of high fevers and pancytopenia. The final diagnosis was viral sepsis complicated by co-infection with MSSA.

A 42-year-old man presented with painless, left inguinal lymphadenopathy, which was suspicious of malignant lymphoma. Multiple left-sided foci of markedly increased metabolic activity were observed on PET/CT (SUVmax up to 22.3), located at the inguinal, iliac, and para-aortic lymph nodes along with small-sized right inguinal lymphadenopathy. Laboratory tests revealed increased inflammation markers and neutrophilic leukocytosis. Pathological examination from dissected inguinal lymph node was consistent with granulomatous disease. Infection by chlamydia trachomatis was made serologically establi

Isolated superior oblique myositis is a rare variant of idiopathic orbital myositis. We are reporting for the first time the case of a 19-year-old woman who had isolated superior oblique myositis with sinusitis that mimics a subperiosteal abscess. Despite the typical history of upper respiratory tract infection and laboratory test results and initial radiological findings suggestive of orbital cellulitis secondary to sinusitis, the initial response to systemic steroid with subsequent imaging changes and the relapse after cessation of steroid therapy helped us reach the diagnosis.

Actinomycosis is a chronic suppurative infection that can develop almost anywhere in the body. Cervicofacial actinomycosis is the most common form of the disease. We report a case of tonsillar actinomycosis that causes massive, asymmetric enlargement of tonsil and mimics the neoplasia. The most common cause of asymmetric tonsil hypertrophy is tonsillar squamous cell carcinoma and lymphomas. Tonsillar actinomycosis is a rare entity that should be kept in mind, if cases cannot be clarified with clinical and laboratory findings, when radiological findings are suspicious about malignancy.

In humans, infection with the coronavirus, especially the severe acute respiratory syndrome coronavirus (SARS-CoV) and the emerging Middle East respiratory syndrome coronavirus (MERS-CoV), induces acute respiratory failure, resulting in high mortality. Irregular coronavirus related epidemics indicate that the evolutionary origins of these two pathogens need to be identified urgently and there are still questions related to suitable laboratory animal models. Thus, in this review we aim to highlight key discoveries concerning the animal origin of the virus and summarize and compare current anima

Onychomycosis is a fungal nail infection caused primarily by dermatophytes. Several other nail disorders, including psoriasis, can simulate onychomycosis. Accurate diagnosis is therefore vital for the ongoing treatment and management of onychomycosis and to avoid misdiagnosis and treatment delay, which can be both lengthy and costly. Often, a combination of histologic and laboratory techniques is used to obtain an accurate diagnosis. The potential diagnostic challenges associated with the differential diagnosis of onychomycosis caused by dermatophytes and the most common techniques used to con

BACKGROUND: Lyme disease, a tick-borne illness transmitted to humans, has become increasingly recognized.
OBJECTIVE: Given the myriad neurovegetative and neuropsychiatric symptoms that can be associated with Lyme disease, we sought to review the epidemiology, evaluation, and treatment of the disorder.
METHODS: A case vignette that highlights manifestations of the condition and dilemmas associated with its treatment is presented.
CONCLUSIONS: Knowledge about its symptoms, its abnormal laboratory tests, and its course can help to guide the timely and appropriate management of this spirochetal

The epidemiology, etiology and outcome of neonatal sepsis are changing over time. While monitoring longitudinal trends in neonatal sepsis in our institution, we encountered a case of late-onset neonatal sepsis due to Leclercia adecarboxylata. A Gram-negative rod previously not encountered in the clinical setting, L. adecarboxylata has recently emerged as a human pathogen, primarily in immunosuppressed patients. This report describes the clinical and laboratory features of this case of late-onset L. adecarboxylata sepsis, and reviews significant features of infection associated with this emergi

Emphysematous pyelonephritis (EPN) is a rare fulminating, gas-forming infection of the renal parenchyma. Diagnosis of EPN is difficult if it is based only on the history, physical examination and laboratory results. But the radiological evidence is valuable for diagnosis. CT scan is the most valuable method for diagnosis. This is a case report of EPN diagnosed by plain film of the abdomen and confirmed by non-contrast CT. The patient underwent nephrectomy and EPN was confirmed by pathological finding. Diagnostic investigation of this condition was reviewed.

Rickettsia conorii is endemic in Greece, though only a few cases of infection have been published to date. The case of a 58-year-old man from northern Greece with a severe form of Mediterranean spotted fever and rapid neurological deterioration is presented here. The patient received antibiotic treatment with doxycycline, showing immediate clinical and laboratory improvement. Diagnosis was confirmed later, during the second week after disease onset, by detection of elevated titres of IgM and IgG antibodies against R. conorii using an indirect immunofluorescence assay.

On March 30, CDC was notified by the Pennsylvania Department of Health of an acute febrile respiratory illness characterized by fever, chills, dry cough, chest pain, and headache among college students who traveled to Acapulco during March 2001. Initial laboratory testing indicated that most students had histoplasmosis, an infection caused by the soil-inhabiting fungus, Histoplasma capsulatum. While in Acapulco, most ill students had stayed at the Calinda Beach Hotel. This report updates the investigation of the outbreak and presents possible evidence of ongoing transmissions.

We report the case of an infant in whom congenital syphilis was diagnosed at the age of 5 weeks. The case is remarkable because of (a) the negative venereal disease laboratory test from the cord blood, (b) the incidental diagnosis of the disease in the fifth week of life, (c) pneumonia alba being one of the symptoms, (d) the occurrence of a mild Jarisch-Herxheimer reaction after initiation of penicillin therapy and (e) the successful treatment of infection related anaemia with recombinant human erythropoietin.

Emergency physicians are often required to evaluate febrile infants and children with no obvious source of infection. Most have uncomplicated viral illnesses. It is imperative, however, to identify the child with potentially life-threatening bacterial meningitis. The clinical presentation of meningitis is variable and depends on many factors. These variables are discussed as well as the indications for lumbar puncture and guidelines for laboratory evaluation. Recommendations for antibiotic selection and dosages are provided. The clinical trials regarding the use of dexamethasone are also revie

A rapid micro-agglutination test (RMAT) for the detection of antibody to Legionella pneumophila serogroup 5 is described. It was found to be both sensitive and specific when compared with the indirect immunofluorescence test. Evaluation of 89 paired sera from patients with respiratory symptoms showed that the incidence of L. pneumophila serogroup 5 respiratory infection in East Anglia is low: only one case was found in this study. The RMAT would be easy to perform as a screening test in a routine serological laboratory.

The authors report a case of bilharzia localised to one of the ureters in a case of double renal pelvis and ureter. The parasitic infection was identified only at the time of exploratory surgery (laboratory investigations and preoperative endoscopy negative). The "providential" double ureter made it possible to combine excision of the pathological ureter with a termino-lateral anastomosis between the lower renal pelvis and the ureter of the upper renal pelvis. The result remains very satisfactory with a follow-up of 13 months.

A patient with lymphoma developed central nervous system dysfunction and the diagnosis of cryptococcal meningitis was made. Because of discordant clinical and laboratory responses to appropriate treatment, further investigations were performed and the additional diagnosis of progressive multifocal leukoencephalopathy (PML) was established. Computerized axial tomography (CT scanning) proved helpful in pointing toward the latter disorder. Clinical, neuropathological, and microbiologic features are presented. The diagnostic approach to central nervous system illness in patients with impaired immu

Skin and soft tissue infections result from microbial invasion of the skin and its supporting structures. Management is determined by the severity and location of the infection and by patient comorbidities. Infections can be classified as simple (uncomplicated) or complicated (necrotizing or nonnecrotizing), or as suppurative or nonsuppurative. Most community-acquired infections are caused by methicillin-resistant Staphylococcus aureus and beta-hemolytic streptococcus. Simple infections are usually monomicrobial and present with localized clinical findings. In contrast, complicated infections

BACKGROUND: Klebsiella variicola has been implicated in neonatal intensive care unit (NICU) outbreaks previously and can be misidentified as Klebsiella pneumoniae. An increased incidence of K. pneumoniae bacteremia on the NICU of our institution was notified to the infection prevention and control (IPC) team in May 2024. The four isolates involved displayed wild-type susceptibility, so had not been detected via multidrug-resistant organism surveillance. This triggered investigation with a nanopore-based decentralized whole-genome sequencing (dWGS) system in operation at our laboratory.
