Transvaginal color Doppler was used to analyse a group of 56 patients including 41 with proven pelvic masses and 15 infertility patients with normal pelvic anatomy. Radical surgery was performed in 971 (86.0%) cases of rectal cancer, while palliative resection or extirpation of the rectum--in 158 (14.0%). The postoperative lethality rates were 5.8 and 11.4%, respectively. Twenty-four women who had undergone total salpingectomy due to ectopic pregnancy and who subsequently underwent a laparotomy for a second ectopic pregnancy in their opposite tube were treated by the author. Treatment consisted of linear salpingostomy (n = 20) and partial salpingectomy (n = 4). The intrauterine pregnancy rate after linear salpingostomy in women who attempted to conceive was 50%, and the incidence of a third ectopic pregnancy was 27.8%. A variety of benign liver tumors associated with the use of oral contraceptives has been described. However, there is controversy regarding the possible relation of focal nodular hyperplasia of the liver to oral contraceptive therapy. Over a ten-year period at the Soroka Medical Center, two young women were found to have hepatic tumors diagnosed as focal nodular hyperplasia. Two cases of biliary peritonitis due to intra hepatic bile duct rupture are reported. The first occurred in a seven year old girl with a right lateral hepatic duct. Outcome was favorable spontaneously after simple surgical drainage. The second was observed in a seven year old boy with rupture of the origin of the left hepatic duct under the falciformis. The case history of a 53-yr-old woman exhibiting the progressive appearance of several hundreds of subcutaneous leiomyomatous tumors in a 15-yr period is presented. The occurrence of multiple tumors in the gastrointestinal tract, mesentery, and retroperitoneum, necessitated several laparotomies because of pain and chronic blood loss. A total of 300 patients (66% women, 34% men) had carcinoma of the bile ducts; 64% were located in the gallbladder and 36% in the bile ducts. In 77.3% the carcinomas had metastasized or were conglomerate tumors, while in 22.7% a curative resection was performed. In 35.3% a palliative operation or resection was indicated. An exploratory laparotomy was performed in 42%, but 75% of the patients died within 6 months. After 1 year 10.2% had survived. After curative resection 46 patients survived on the average over 17.6 months, while 12 patients survived an average of 5.1 years.

Fascioliasis is rarely reported in humans although it is endemic in sheep and cattle. We describe the illness of a 60-year-old widow who ate wild watercress which is the usual source of infestation. Laparotomy for suspected liver abscesses revealed necrotic tracts on the surface of the liver left by the invasion of numerous flukes. Diagnosis was made during the latent phase by the detection of serum antibody to fasciola hepatica antigen obtained from a sheep. Symptoms returned during the cholestatic phase. A rare case of gastrointestinal fistula secondary to gastric cancer is presented. Fluoroscopic examination visualized the passage of contrast medium from the stomach to the small intestine. Gastric fiberscopy revealed cancer on the greater curvature of the corpus perforating into the jejunum. The scope could be inserted readily into jejunum through this perforation. The experience with surgical treatment of 866 patients with malignant neoplasms of the pancreato-duodenal zone is analyzed. Radical operations were carried out upon 220 (25.4%) patients, palliative ones-upon 459 (52.8%) and laparotomies-upon 187 (22.7%) patients. A series of 456 patients with malignant epithelial tumours of the ovary clinical stage III is presented. Of these, 96 were initially inoperable, and 49 had a clinical diagnosis of inoperable disease at the first examination. The rationale of undertaing operative interventions in pregnant women with mitral stenosis of the III and IV stages is analyzed. The authors report a case of Hodgkin's disease at the subdiaphragmatic II B b stage with cell type III, revealed by thrombopenic purpura. Laparotomy and splenectomy confirmed the invasion of the spleen whereas there was no clinical splenomegaly and splenic radio-isotope scan was normal.
