Children who receive treatment for medulloblastoma have a high survival rate, but also a high likelihood of developing posterior fossa syndrome, a condition that includes devastating balance and motor problems. This case series used 2 novel neuromodulation devices in conjunction with an intensive physical therapy intervention for 2 children who were 5 years post tumor treatment with a diagnosis of posterior fossa syndrome. Pre- and postclinical measures, in addition to magnetoencephalography brain imaging, describe positive behavioral and neuroplastic changes resulting from the intervention. T

Tobacco use results in numerous consequences for individuals with mental illnesses and other substance use disorders, yet it is not adequately addressed by behavioral health professionals, including psychiatrists. This column describes current inaction among behavioral health professionals and some possible reasons for it and recommends next steps. Psychiatrists should provide treatment for all patients with a co-occurring tobacco use disorder and provide leadership to change policies and practices in treatment centers. Psychiatrists can be vital leaders of the effort to reduce the toll of tob

Advances in surgical and medical management have dramatically improved the survival of individuals with congenital cardiac anomalies. Various neurologic complications occur in association with congenital heart disease, including cognitive impairment and ischemic stroke. The likelihood of stroke is greatest in individuals with severe structural cardiac defects such as tetralogy of Fallot, transposition of the great arteries, or hypoplastic left heart syndrome. A persistent foramen ovale adds little or no additional stroke risk unless it is associated with an atrial septal aneurysm or other anom

Angelman syndrome (AS) is a neurogenetic disorder, characterized by intellectual disability, movement or balance disorders, specific abnormal behaviors, and severe speech and language limitations. Due to its low incidence and the nonspecifity of developmental problems in newborns and young children, AS is not easily identified by clinical pediatricians. The aim of this paper is to present three cases of AS, reporting the orofacial characteristics and requisite dental care in these patients. Interestingly, this investigation found that certain typical features of mouth breathing syndrome, such

Today, considering their adverse side effects, the first-generation antipsychotics have been replaced by the new-generation antipsychotics (also known as second-generation antipsychotic agents). The superiority of new-generation antipsychotics compared with first-generation antipsychotic agents in terms of side effects, especially movement disorders, are acknowledged by clinicians. But in recent years during the use of second-generation antipsychotic agents, endocrine side effects have been noteworthy. In our study with a diagnosis of paranoid schizophrenia treated with risperidone for 14 year

In this study, we present two cases of different eye movement disorders with variable case histories but with the same end stage; abduction paresis of one of the eyes, which ceased when the other eye was covered. Our differential diagnosis is that either the ocular form of myasthenia gravis, convergence spasm or ocular myotonia could explain the symptoms. However, we hypothesize that the clinical picture corresponds to pseudo abducens palsy or focal dystonia of the extraocular muscle, which in turn could be the result of impaired inhibition of the tonic resting activity of the antagonistic med

Tremor is one of the most disabling symptoms in patients with many movement disorders including Parkinson's disease (PD) and essential tremor (ET). Neural tremor, which is measured by microelectrodes placed near nerve cells or obtained from brain cells during stereotactic neurosurgery, is due to the fluctuation of the firing rate of neurons. The frequency of this neural tremor varies over time and is nonstationary. We describe a frequency tracking method using the extended Kalman filter (EKF) to estimate the instantaneous tremor frequency (ITF) of binary spike trains detected from microelectro

The dynamic unconscious remains in contention in cognitive psychology. Academic settings may not provide the compelling evidence of unconscious dynamics regularly experienced by dynamic psychiatrists, psychoanalysts, and other physicians, including neurologists treating psychogenic motor disorders in the movement disorder clinic. An origin of unconscious motor expression is postulated in the intermodal connectivity of neuromental agencies, extending an idea that originated in Freud's (1950/1966) "Project for a Scientific Psychology." Motoric alexithymia is contrasted with synesthesia. Other ex

Parents and clinicians concerned about high-risk infants and children with motor delay or cerebral palsy seek information on cause, treatment, prognosis, and recurrence risk. Used in combination with history and examination, neuroimaging studies can improve diagnosis and management. In premature infants, cranial ultrasound is a reliable, noninvasive diagnostic modality. Nuclear magnetic resonance techniques including magnetic resonance imaging and diffusion weighted imaging can be used effectively in neonatal encephalopathies. In children with motor delay and cerebral palsy syndromes including

Primary torsion dystonia (PTD) is a clinically and genetically heterogeneous group of movement disorders, usually inherited in an autosomal dominant fashion. Three PTD loci (DYT1, DYT6 and DYT7) have been identified to date. However, in several PTD families linkage to the known loci has been excluded. We identified an Italian PTD family with 11 definitely affected members. Phenotype was characterised by juvenile or early-adult onset, prominent cranial-cervical and upper limb involvement, mild course and occasional generalisation. A genome-wide search performed in the family identified a novel

CASE REPORT: A 39-year-old man ingested 35 g carisoprodol. He developed agitation, tachycardia, myoclonus, and coma. The blood carisoprodol was 71 micrograms/mL; the meprobamate was 26 micrograms/mL.
DISCUSSION: Carisoprodol overdose is thought to induce simple central nervous system depression. This case demonstrates a severe overdose with symptoms more consistent with myoclonic encephalopathy. A review of cases presenting to the San Francisco Division of the California Poison Control System during 1997 suggests that carisoprodol is more commonly associated with agitation and bizarre movemen

From March 1986 to April 1995, 418 consecutive psychiatric consultations for patients aged 18 or below were completed. Ten of these patients met the Diagnostic and Statistical Manual of Mental Disorders, 4th. edition (DSM-IV) diagnostic criteria for a conversion disorder. The medical and psychiatric consultation records of these patients were reviewed. Chief conversion symptoms included pseudoseizure (four cases), gait disturbance (two cases), abnormal movement (one case), four limbs weakness (one case), aphonia (one case) and tunnel vision (one case). The psychosocial problems of the patients

The records of 100 patients with anorexia nervosa were reviewed. Neurologic complications were present in 47 patients. Neuromuscular abnormalities were most common and were present in 45% of patients. Generalized muscle weakness was detected in 43% of patients and peripheral neuropathies in 13%. Less common neurologic complications included headaches (6%), seizures (5%), syncope in the absence of orthostatic hypotension (4%), diplopia (4%), and movement disorders (2%). Neurologic problems due to chronic deficiency diseases were rare; only one patient had symptoms directly attributable to a vit

In routine clinical electronystagmographic (ENG) tests (postrotatory, optokinetic, caloric and tracking tests), eye movement signals were analyzed and a multiple discriminant analysis was carried out with the aid of a microcomputer. Six parameters were selected and, based on these, two functions for discriminating between peripheral and central disorders were established. Discrimination between 35 patients with peripheral lesions and 15 patients with central lesions was made with a correct classification rate of 97.1 and 86.7%, respectively. These rates are significantly higher than that of an

The hypothesis of a significant involvement of the cholinergic system in the pathogenesis of affective disorders still lacks strong experimental support. This is mainly because of missing specific peripheral markers of the central nervous activity of the cholinergic system and the lack of specific cholinergic agonists and antagonists without severe peripheral side effects. As the direct cholinergic agonist RS 86 seems to be more suitable because of its minor side effects, long half-life and oral applicability, it was tested for its antimanic property and its effect on the hypothalamo-pituitary

The cerebral blood flow time (CBFT) was examined in 100 patients with brain circulatory disorders with the aid of radiocirculography (RCG). Use was made of 99mCc-pertechnetate (37 MBq) and a two-channel "Gamma" radiograph manufactured in the Hungarian Republic. The rate of film movement amounts to 160 mm/min at the time constant 10. The time of 5.5-7.5 s is assumed as normal CBFT. On recording the arterial and venous phases one can see different alterations in RCG, which primarily manifest themselves in the form of cerebral blood flow deceleration (ischemic and hemorrhagic strokes, cerebral at

A 58-year-old male with spinocerebellar degeneration in association with choreiform movement is described. He possessed neurological, biochemical, immunological, as well as cytogenetic disorders predisposing to cancer to patients with ataxia telangiectasia (AT). Cytogenetic abnormalities included four abnormal clones: trisomy 14;t(13;14);t(7;14)(q32;q12); and t(7;14)(p13;q12), in addition to breaks, rings, and acentric chromosomes. The absence of telangiectasia, the late onset of neurological symptoms, and the prolonged survival in this case may separate it from the classic AT. Although the sy

Blepharospasm and oromandibular dystonia are clinically similar to other hyperkinetic movement disorders. Dopaminergic antagonist (neuroleptic) and purported cholinergic agonist (deanol) treatment improved symptoms, whereas dopaminergic agonist (carbidopa/levodopa) and cholinergic antagonist (benztropine) drugs worsened symptoms in two patients. This suggested that the syndrome is also pharmacologically related to the hyperkinetic dyskinesias. Symptoms worsened substantially during carbidopa/levodopa but temporarily resolved in one patient and improved in another when the drug was discontinued

Correlation of self-mutilation and sleep stage in the Lesch-Nyhan syndrome was studied by a polygraphic method. Five patients and three control boys were monitored with EEG, EOG, EMG, ECG and respiration throughout the night. The results were as follows: 1. The sleep-time of the patients was much disturbed during the night. 2. Decreased REM density was noticed with low DQ of the patients. 3. Self-mutilation during sleep-time was observed a lot in stages 1, 2, and REM in two cases. 4. No correlation was observed between body movement and self-mutilation in the Lesch-Nyhan syndrome. These data s

A group of 16 patients afflicted with involuntary movement disorders received subcutaneous injections of the direct dopamine agonist, apomorphine hydrochloride. Paradoxically, these injections were generally followed by a reduction of dyskinesia; this was most noticeable in patients with tardive dyskinesia and was only mild in some patients with spasmodic torticollis. Preferential stimulation by apomorphine of inhibitory dopamine presynaptic receptors (so-called dopamine autoreceptors) is proposed as the most likely explanation for the observed antidyskinetic effect of this drug. The results o
