[Electrophysiological evaluation of a case of sensory-motor multifocal acquired demyelinating neuropathy. The effects of immunoglobulin therapy].

INTRODUCTION: Sensory motor multifocal acquired demyelinating neuropathy (SMMADN) is a variant of the chronic multifocal neuropathies. Several reports have been published about the clinical and electrophysiological progression in motor multifocal neuropathy (MMN) prior to and following immunoglobulin (Ig) therapy, but we have found no reports that deal with SMMADN. AIMS: We describe a case of SMMADN in which we conducted a clinical and electrophysiological evaluation before and after therapy with Ig UNC Hemoderivados. CASE REPORT: Female, 40 years of age, who presented asymmetrical weakness

Body equilibrium at the end of gait initiation: importance of ankle muscular force as evidenced in clubfoot children.

This study investigates the importance of ankle muscular force and foot shape on body equilibrium at the end of a first step by means of a comparison between healthy children and children with unilateral clubfoot who showed Triceps surae atrophy. Subjects were asked to initiate gait at different velocities and to perform a few forward-oriented steps. In healthy children, there was a significant increase in the number of positive values of the centre of gravity vertical acceleration at foot contact (Z"HC) as velocity increased without any difference between the preferred and non-preferred stepp

Low bone mineral density in non-ambulatory adolescents relates to lower correction of neuromuscular scoliosis after posterior spinal fusion.

PURPOSE: Non-ambulatory adolescents with neuromuscular scoliosis (NMS) often require surgical intervention. The purpose of this study was to examine whether low bone mineral density (BMD) affects the results of posterior spinal fusion. METHODS: Volumetric BMD (vBMD) was measured in preoperative computed tomography (CT) scans of scoliotic patients with Duchenne Muscular Dystrophy (DMD), Spinal Muscular Atrophy (SMA), Cerebral Palsy (CP) and Rett syndrome (RTT). CT scans of healthy age-matched adolescents were used to define reference vBMD values and to calculate Z-scores. Scoliosis angles were…

U.S. health plan coverage of Neuromuscular Disease Therapies: An assessment of policy availability and restrictions.

BACKGROUND: Health plan policies managing neuromuscular disease (NMD) therapies may impose burdensome requirements on patients and providers. While prior research has explored payer restrictions for rare NMD treatments, limited evidence exists on policies specifically governing therapy initiation and reauthorization. OBJECTIVES: To examine initial and reauthorization coverage policies for therapies targeting five NMDs-Duchenne muscular dystrophy, amyotrophic lateral sclerosis, spinal muscular atrophy, generalized myasthenia gravis, and Lambert-Eaton myasthenic syndrome-across all U.S. state M…

Severe burn increased skeletal muscle loss in mdx mutant mice.

BACKGROUND: Severe burn causes muscle mass loss and atrophy. The balance between muscle cell death and growth maintains tissue homeostasis. We hypothesize that preexisting cellular structural defects will exacerbate skeletal muscle mass loss after burn. Using a Duchenne muscular dystrophy (mdx) mutant mouse, we investigated whether severe burn caused more damage in skeletal muscle with preexisting muscle disease. METHODS: The mdx mice and wild-type (WT) mice received 25% total body surface area scald burn. Gastrocnemius (GM), tibialis anterior, and gluteus muscles were obtained at days 1 and

Peptide nanoparticle delivery of charge-neutral splice-switching morpholino oligonucleotides.

Oligonucleotide analogs have provided novel therapeutics targeting various disorders. However, their poor cellular uptake remains a major obstacle for their clinical development. Negatively charged oligonucleotides, such as 2'-O-Methyl RNA and locked nucleic acids have in recent years been delivered successfully into cells through complex formation with cationic polymers, peptides, liposomes, or similar nanoparticle delivery systems. However, due to the lack of electrostatic interactions, this promising delivery method has been unsuccessful to date using charge-neutral oligonucleotide analogs.

Mechanisms of green tea-induced attenuation of osteoporosis and skeletal muscle atrophy: current status and future study recommendations for clinical application.

Epigallocatechin gallate (EGCG), have been reported to attenuate skeletal muscle atrophy and osteoporosis. This review first examines the relationship between the beneficial effects of EGCG and its ability to enhance stem cell function by improving cellular antioxidant capacity. It subsequently identifies key areas warranting further investigation to support the clinical application of EGCG. Stem cell function is essential for the maintenance of muscle and bone mass; however, it declines with aging, largely due to the excessive accumulation of reactive oxygen species and dysregulation of Yes-a

Gait analysis for functional evaluation in a surgical hindlimb suspension model of muscle atrophy.

Disuse-induced skeletal muscle atrophy, commonly resulting from bedrest, immobilisation or spaceflight, leads to rapid loss of muscle mass and impaired mobility. Although muscle mass and contractile force are standard assessments in experimental models, these measures often fail to capture neuromuscular co-ordination deficits essential for effective movement. To better characterise these deficits, we employed a mouse hindlimb suspension (HLS) model for 14 days to induce disuse atrophy, confirmed by reductions in muscle mass, fibre type remodelling and satellite cell depletion, all of which wer

Increased 1-year mortality rates after hip fracture surgery in geriatric patients with skeletal muscle atrophy: a propensity score-matched study.

PURPOSE: To investigate the relationship between skeletal muscle atrophy (SMA), assessed using chest CT-derived skeletal muscle index (SMI) at the T12 vertebra level, and 1-year mortality in geriatric patients following hip fracture surgery. METHODS: After applying exclusion criteria, 1,243 patients (514 SMA) were analyzed. SMA was diagnosed by the skeletal muscle index at the T12 vertebral level (males: T12-SMI < 25.75 cm2/ m2; females: T12-SMI < 20.16 cm2/m2). The association between T12-SMI and 1-year mortality risk was assessed using multivariate logistic regression and propensity score-m

Sciatic denervation differentially remodels skeletal muscle and deep fascia in the lower limb of male mice.

The deep fascia surrounds skeletal muscles and contributes to force transmission and mechanical coordination, but its response to denervation remains unclear. This study investigated morphological and transcriptional adaptations of the deep fascia following sciatic nerve denervation in mice. Male C57BL/6J mice (n = 5-6/group) underwent unilateral sciatic nerve denervation or sham surgery. Fourteen days later, tibialis anterior (TA) muscle and overlying deep fascia (anterior crural fascia) were analyzed by histology and quantitative PCR. Denervation induced significant TA muscle atrophy with up

Sema4A Protects Against Muscle Atrophy and Promotes Repair by Regulating Intracellular Metabolic Signalling.

BACKGROUND: Skeletal muscle atrophy is a debilitating condition associated with diverse diseases and clinical interventions. Although triggers such as glucocorticoid excess or direct muscle injury disrupt the balance between protein homeostasis and tissue repair, the upstream molecular signals that actively preserve muscle integrity remain largely unknown. Semaphorin 4A (Sema4A) is a Class IV transmembrane semaphorin traditionally recognized for its roles in neural development and immune regulation; however, its function in skeletal muscle maintenance has not been explored. METHODS: We first

Age, cancer, and the dual burden of cancer and doxorubicin in skeletal muscle wasting in female rats: which one to blame?

Sarcopenia and cancer cachexia are two life-threatening conditions often misdiagnosed. The skeletal muscle is one of the organs most adversely affected by these conditions, culminating in poor quality of life and premature mortality. In addition, it has been suggested that chemotherapeutic agents exacerbate cancer cachexia, as is the case of doxorubicin. Herein, we sought to investigate markers of inflammation and neuromuscular junction (NMJ) remodeling during aging and in response to cancer or cancer with chemotherapy. To address this, we utilized female rats across three age groups - young,

Muscle Dysfunction and Physical Recovery After Critical Illness.

During critical illness, patients experience significant and rapid onsets of muscle wasting and dysfunction with loss of strength, mass, and power. These deficits often persist long after the ICU, leading to impairments in physical function including reduced exercise capacity and increased frailty and disability. While there are numerous studies describing the epidemiology of impaired muscle and physical function in the ICU, there are significantly fewer data investigating mechanisms of prolonged and persistent impairments in ICU survivors. Additionally, while several potential clinical risk f

Comparative Analysis of Primary Sarcopenia and End-Stage Renal Disease-Related Muscle Wasting Using Multi-Omics Approaches.

BACKGROUND: Age-related primary sarcopenia and end-stage renal disease (ESRD)-related muscle wasting are discrete entities; however, both manifest as a decline in skeletal muscle mass and strength. The etiological pathways differ, with aging factors implicated in sarcopenia and a combination of uremic factors, including haemodialysis, contributing to ESRD-related muscle wasting. Understanding these molecular nuances is imperative for targeted interventions, and the integration of proteomic and metabolomic data elucidate these intricate processes. METHODS: We generated detailed clinical data a
